321 The effect of chlorimipramine on the spontaneous contraction of isolated duodenal smooth muscle of rabbits

Objective To investigate the effect of chlorimipramine (CIMP) on the spontaneous contraction of duodenal smooth muscle of rabbits in vitro and its mechanism.Methods The isolated intestinal tubes of rabbits were made.The technique of perfusion of isolated duodenum was used to record duodenal smooth muscle contraction curve.The influence of CIMP on the spontaneous contraction of duodenal smooth muscle was observed and the mechanism of CIMP was studied with pharmacological methods.Results CIMP reduced the amplitude of spontaneous contraction of duodenal smooth muscle in a dose-dependent manner,while the frequency was not obviously influenced.CIMP inhibited the enhanced contraction of duodenum smooth muscle induced by acetylcholine.CIMP also inhibited the contraction of duodenum smooth muscle induced by calcium chloride application in the calcium-free Tyrode’s solution.ACh and CaCl2 were added successively into calcium-free Tyrode’s solution,and CIMP was not effective on the release of internal calcium, but inhibited external calcium flow in the dose-dependent manner.Conclusion CIMP obviously inhibits the spontaneous contraction of duodenal smooth muscle of rabbits in vitro. Suppression of M receptor and inhibition of the extracellular calcium influx via cell membrane might be involved in the process.

326 Significance of expression of LC3,Raptor and Rictor in papillary thyroid carcinoma

Objective To investigate the expression of LC3,Raptor and Rictor in papillary thyroid carcinoma and elucidate the relationship between LC3,Raptor,Rictor and cancer clinical pathological factors.Methods Immunohistochemical method was used to detect LC3,Raptor and Rictor expression.The relationship with expression of LC3,Raptor, Rictor and clinicopathological factors was analyzed.Results The positive rates of LC3,Raptor and Rictor in papillary thyroid carcinoma were 76.7, 82.5 and 41.7% respectively, but they were 4.9,16.5 and 7.8%, respectively in the adjacent tissues.The difference was statistically significant.LC3 was correlated with tumor size and psammoma bodies,High expressions of Raptor and Rictor were significantly correlated with TNM grade, and Rictor was correlated with lymph node metastasis.Conclusion The expression of LC3,Raptor and Rictor were up-regulated in papillary thyroid carcinoma, and they were considered to have potential value for the diagnosis and prognosis of papillary thyroid carcinoma.

330 Expression and clinical significance of Axl and Gas6 in colon cancer

Objective To investigate the expression and clinical significance of receptor tyrosine kinases (Axl) and growth arrest-specific 6 (Gas6) in colon cancer,and discuss the role of Axl and Gas6 during the development and metastasis of colon cancer,and analyze their relationship.Methods The Axl and Gas6 expression was detected in 87 cases of normal,adjacent, and colon cancer tissues using immunohistochemical method to analyze the difference of Axl and Gas6 in three different tissues and the corresponding clinical pathological significance.Results The expression rate of Axl in 87 cases of colon cancer was 52.9%, which was significantly higher than that in normal tissues 33.3% and adjacent tissues 34.5%.The expression rate of Gas6 in 87 cases of colon cancer was 60.9%,which was significantly higher than that in normal tissues 37.9% and adjacent tissues 42.5%,and the expression of Axl and Gas6 was correlated with TNM stage and lymph node metastasis.Axl expression was positively correlated with Gas6 expression in colon cancer. Conclusions Axl, Gas6 protein were highly expressed in colon cancer tissues,they played an important role in the occurrence, development, invasion and metastasis of colon cancer.

334 Expression and the clinical significance of SCIN in ovarian cancer

Objective To investigate the expression and the clinical significance of SCIN in epithelial ovarian cancer.Methods Immunohistochemistry assay and western blot were used to detect the expression of SCIN in epithelial ovarian cancer tissues and normal ovarian tissues.Results The expression of SCIN in epithelial ovarian cancer tissues was significantly higher than that in normal ovarian tissues, and there was statistical significance between the two groups. The expression of SCIN was correlated with the clinical stage, grade of differentiation, lymph node metastasis and pritoneal seeding, while it is not correlated with age. The gray level of SCIN in epithelial ovarian cancer cells was significantly higher than that in normal ovarian cells, and there was statistical significance between the two groups.Conclusion The expression level of SCIN in epithelial ovarian cancer tissues is high, which indicates that it may participate in the occurrence and development of ovarian cancer, and it may become a tumor marker for clinical early diagnosis and a new target gene for treatment of ovarian cancer.

339 Expression and clinical significance of FAP and FSP1 in papillary thyroid carcinoma

Objective To observe the expression of fibroblast-activated protein (FAP) and fibroblast-specific protein 1 (FSP1) in carcinoma-associated fibroblasts (CAFs) of papillary thyroid carcinoma (PTC),and investigate the correlation between CAFs and PTC.Methods The expression of FAP and FSP1 in PTC,normal thyroid was detected by immunohistochemistry, and the clinicopathologicaI correlation was analyzed.Results FAP and FSP1 didn’t express in normal thyroid,but did in PTC.The positive expressions of FAP and FSP1 were in accord with cancerous fibrous response around PTC cells.In PTC,the expression of FAP was positively related to tumor diameter,lymph node metastasis and TNM staging, but the expression of FSP1 was negatively related to the clinicopathological parameter.Conclusion FAP and FSP1 were the senstive markers of carcinoma-associated fibroblasts(CAFs) in PTC.FAP may be certain significant to predict the malignant biological behavior in PTC.

343 Improvement and application of a preoperative location method of percutaneous endoscopic lumbar discectomy

Objective To explore a novel preoperative location method which was more time-saving and effective than traditional preoperative location method of percutaneous endoscopic lumbar discectomy (PELD).Methods 60 patients with lumbar disc herniation (LDH) received surgical treatment were selected.According to the difference of preoperative location method,30 patients of group A were surgically treated by traditional method of preoperative location method,while 30 patients of Group B underwent operation by novel method. The fluoroscopy times, preoperative location time and operation time were thoroughly recorded and analyzed to compare with the different preoperative location method.Results The fluoroscopy time of Group A was (15.63±3.61) and that of Group B was (12.20±3.67). The preoperative location time of Group A was (16.62±2.45) min and that of Group B was (11.51±1.50) min.The operation time of Group A was (65.73±4.09) min and that of Group B was (58.49±3.87) min.Conclusion Compared with traditional preoperative location method,the fluoroscopy times,preoperative location time, and operation time of patients with novel preoperative location method were significantly shorter.

347 Investigation on short-term compliance and influencing factors of young and middle-aged discharged patients with coronary heart disease

Objective To investigate the current status of short-term treatment compliance and related factors in young and middle-aged patients with coronary heart disease (CHD).Methods A total of 258 young and middle-aged patients with coronary heart disease were followed up 6 months. Questionnaires were carried out to assess general conditions,social supports, medication compliance, etc. The relationship between above indexes and treatment compliance status were analyzed.Results 102 patients were with better treatment adherence and 156 patients with poorer. The single factor analysis showed that the compliance of the patients with coronary heart disease was significantly related to the monthly per capita income, the education level, the knowledge of coronary heart disease,and the social support.The incidence of major adverse cardiovascular events (MACE) in patients with poor compliance was significantly higher than that in patients with good compliance. Multivariate logistic regression analysis showed that the low score knowledge of CAD , poor social support and low per capita income were the independent factors that led to the poor compliance of the middle-aged and young patients with coronary heart disease.Conclusion The short-term treatment compliance of young and middle-aged patients with coronary heart disease after discharge was not optimistic. Cognition and knowledge of coronary heart disease,social support and family per capita income were independent factors that affect the treatment compliance of young and middle-aged patients with coronary heart disease.

351 Analysis of the characteristics of hospitalized patients with systemic lupus erythematosus in the Yellow River Delta

Objective To analyze the clinical characteristics,treatment and progonsis of hospitalized patients with systemic lupus erythematosus (SLE) in the Yellow River Delta.Methods The clinical characteristics,laboratory test,pathology and therapeutic regimen of 351 cases with SLE were retrospectively analyzed.Results The ratio of male and female was 1∶10.7. The most common clinical symptoms was joint pain, which accounted for 41%,and followed by fever accounted for 35%, rash accounted for 30%,swelling accounted for 10%,fatigue accounted for 9%.Kidney was the most commonly affected organ, the largest number of pathological types was type Ⅳ lupus nephritis.Total mortality of patients was 2.2%, and the death in patients with infection accounted for 1.4%.Conclusion SLE primarily occurs in childbearing aged women,and its clinical presentation is complex and varied.Preventing the infection can improve the prognosis of SLE.

354 Application of optic capture in microphthalmos congenital cataract surgery

Objective To evaluate the outcomes of 3-piece hydrophobic acrylic IOL optic capture for children with microphthalmos and congenital cataract.Methods In this retrospective study,outcomes and complications of primary or secondary 3-piece hydrophobic acrylic IOL optic capture (11 eyes of 6 patients had primary IOL implantation and 11eyes of 7 patients with secondary IOL implantation) for children with microphthalmos and congenital cataract were analyzed.Results with mean follow-up period of (26.55± 11.03) months, all eyes maintained round pupils, and visual acuity improve to different degrees. Posterior capsule opacification (PCO) was detected in both eyes of a patient and partial posterior synechia detected in both eyes with secondary IOL implantation. There was no IOL dislocation, pupillary?capture or postoperative glaucoma observed.Conclusion IOL optic capture implantation could be a promising solution for children with microphthalmos and congenital cataract.With this procedure,the IOL could be implanted in microphthalmic eyes, with no regard to the size of the capsular bag and artificial crystal size. The occurrence of intraocular lens dislocation or papillary capture of the IOL was minimized.

359 Significance of HPV E6/E7 mRNA combined with thinprep cytologic test in cervical cancer screening

Objective To explore the significance of the combined application of cervical E6/E7 mRNA and thinprep cytologic test (TCT) in cervical cancer screening.Methods 311 patients with cervical high risk HPV infected were chosen to carry on TCT,E6/E7 mRNA and histological examination,histological pathology Results as the gold standard.Results The sensitivity to high-level squamous intraepithelial lesion (HSIL) was 72.97%, the specificity was 90.08%,the positive predictive value (PPV) was 81.82%,and the negative predictive value (NPV) was 85.51%. The sensitivity of E6/E7 mRNA test in HSIL diagnosis was 69.64%,the specificity was 59.30%,and the PPV and NPV were 47.56% and 80.27%,respectively.The sensitivity of TCT to HSIL diagnosis was 56.25%,the specificity was 88.94%,and the PPV and NPV were 74.12% and 81.94%,respectively.Conclusion The sensitivity of E6/E7 mRNA was higher than that of TCT,and the specificity of TCT was higher than that of E6/E7 mRNA.The combination of E6/E7 mRNA and TCT significantly improved the sensitivity and specificity,and the PPV and NPV were more than 80%.Combined E6/E7 mRNA and TCT could improve the sensitivity and specificity of cervical cancer screening.

362 The effects of the ICB on walking ability of children with hemiplegic cerebral palsy

Objective To approach the effects of the ICB(international college of biomechanics,ICB) on walking ability of children with hemiplegic cerebral palsy.Methods Forty cases of hemiplegic cerebral palsy were randomly divided into therapy group and control group,20 cases in each group.All patients received conventional rehabilitation,and the therapy group were given the ICB in addition.Before and after training 12 weeks,the GMFM-88-E scores were assessed on the change of walking ability of the children.Results After treatment for 12 weeks,the GMFM-88-E scores were notably increased in both groups as compared with those before treatment(P<0.05),more significantly in therapy group than in control group(P<0.05).Conclusion The ICB can significantly improve the walking ability of children with hemiplegic cerebral palsy.

365 The clinical experience of open reduction and internal fixation for the treatment of 12 talar fracture-dislocation

Objective To explore the clinical experience via open reduction and internal fixation for the treatment of 12 talar fracture-dislocation.Methods Retrospective analysis of 12 cases of talar fracture-dislocation from March 2010 to September 2016 had undergone open reduction internal fixation.The adverse events such as infection of incision, cutaneous necrosis of the surgical area,breakage failure of the internal fixation,talar avascular necrosis and traumatic arthritis were recorded during the follow-up period.AOFAS ankle-foot evaluation standard and VAS grade were adopted to assess patients’ function.Results The following-up ranged from 14 months to 84 months with an average of 37.6 months.The union time ranged from 11 weeks to 13 weeks with an average of 12.2 weeks.The recovering of normal activity ranged from 14 weeks to 17 weeks with an average of 15.8 weeks.Though there were 5 cases of traumatic arthritis and 2 cases of talar avascular necrosis without collapse,the adverse events such as infection of incision, cutaneous necrosis of the surgical area, breakage failure of the internal fixation, and nonunion were absent.The VAS value ranged from 0 to 8 with an average of 3.5.The AOFAS value ranged from 43 to 92 with an average of 87.2 with 4 cases of excellent,6 cases good,1 case tolerable and 1 case poor,the excellent to good rate was 83.3%.Conclusion Symptomatic treatment in the early stage, sufficient surgical exposure, deliberate protection of the blood supply of talus so as to minimize the surgical damage and personal function exercise of the patients were the important therapeutic methods.

369 Hypoglycemic effect of total flavones of Angelica Keiskei on type 2 diabetic mice

Objective To analyze the hypoglycemic effect of total flavonoids from the leaves of Angelica Keiskei on type 2 diabetic mice.Methods The model of type 2 diabetic mice was maded by intraperitoneal perfusion streptozotocin (STZ) and high fat diet.The mice were divided into the control group,the model group,the positive group and the low,medium and high dose group of the total flavonoids of Angelica Keiskei.The effects of the body weight,fasting blood glucose,glucose tolerance,serum total cholesterol (TC),serum glycerin three fat (TG) and free fatty acid (NEFA) were investigated.Results When the drug was given 2 and 4 weeks, the improvement of body weight and fasting blood glucose in medium and high dose groups of the total flavonoids were better than those in the low dose group,and the difference was statistically significant,while there was no significant difference between the middle and high dose groups. The blood glucose of each group reached peak value at 0.5 h after the gavage, and then gradually decreased. 1 h after gavage, in addition to the control group, the order of decrease of blood glucose in other groups was as follow:Positive group>high dose group>middle dose group>low dose group>model group.When the drug was given 4 weeks,there was no significant difference in TC,TG and NEFA in the low,middle and high dose group of total flavonoids of Angelica Keiskei,but all were lower than that of the model group.Conclusion The total flavonoids of Angelica Keiskei could improve the fasting blood glucose and abnormal lipid metabolism in type 2 diabetic mice.

372 Analysis of pathogenic bacteria of patients with stroke and ventilator associated pneumonia in intensive care unit

Objective To explore pathogens and drug resistance of patients with stroke and ventilator associated pneumonia (VAP) in intensive care unit (ICU).Methods Sixty cases of stroke in ICU were diagnosed with VAP.Medical records such as pathogens distribution and drug resistance were reviewed retrospectively.Results Among 60 patients,123 strains were detected,including 97 Gram-negative bacteria (78.86%),8 Gram-positive bacteria (6.50%) and 18 fungi (14.60%).The most common Gram-negative bacteria were Acinetobacter baumannii,Pseudomonas aeruginosa,Klebsiella pneumoniae and Stenotrophomonas maltophilia. The most common Gram-positive bacteria was Staphylococcus aureus.The drug resistance tests demonstrated that bacteria mentioned above were antibiotic resistant.The fungi were Monilia albican and Candida glabrata.Conclusion The predominant pathogens of ICU stroke patients with VAP were Gram-negative bacteria with highly antibiotic resistance.

376 Outcomes analysis of neonatal and maternal when sevoflurane combined with remifentanil for induction of cesarean delivery in general anesthesia

Objective To investigate the effects of sevoflurane combined with remifentanil on neonates undergoing cesarean section under general anesthesia and to analyze the maternal outcomes.Methods One hundred full-term pregnant women undergoing cesarean section were randomly divided into two groups, the control group and the observation group,50 cases in each group.Remifentanil was used only in the control group,while sevoflurane combined with remifentanil was used in the observation group.Apgar score and anesthesia effect of the two groups were recorded,and the bleeding situation within 2,6 and 24 hours after operation was compared.Results In the observation group,37 cases were excellent,11 cases were good, the excellent and good anesthesia rate was 96.0%;in the control group,23 cases were excellent,10 cases were good, and the excellent and good anesthesia rate was 66.0%. The excellent and good anesthesia rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant. The Apgar scores of neonates in the two groups increased gradually at 1, 5 and 10 minutes after birth. The amount of postpartum hemorrhage in the two groups increased gradually 2,6 and 24 hours after operation.Conclusion Sevoflurane combined with remifentanil could induce general anesthesia for parturients undergoing cesarean section, and it had high safety and lower postoperative bleeding.

378 Application of quality control circle in quality control of peripherally inserted central catheters in oncology patients

Objective to investigate the application effect of quality control circle (QCC) in peripherally inserted central catheters (PICC) control of oncology.Methods The quality control circle group was established.The targets were “to improve the success rate of PICC catheterization,to reduce the complications during PICC and improve the quality of PICC”.226 patients with PICC tube were selected as control group without the implementation of QCC,and 215 patients with PICC tube were selected as observation group with the implementation of QCC.The success rate of tube insertion,the incidence of complications,the knowledge of PICC theory and operation skills,and the patient’s satisfaction in two groups were compared.Results After implementation of QCC,the success rate of tube placement at the first time in the control group and the observation group were 85.84% and 96.74% respectively.The incidence of PICC complications were 31.42% and 15.81% respectively.The PICC theoretical knowledge and operational skills of the nurses were excellent,and the satisfactions of patients were 91.59% and 97.21%.Conclusion The application of QCC could effectively improve the success rate of PICC catheterization at the first time,reduce the incidence of complications, improve patient’s satisfaction, and improve the quality of PICC nursing management and the professional accomplishment of nursing staff.
