321 Construction, phenotype and preliminary application of SUMO1 T95R mutation mouse model

Objective To identify the SUMO-modified sites of myosin by constructing a T95R mutation SUMO1 mouse model in mice.Methods Homologous recombination was used to estabish mutations in ES cells and construct the T95R point mutation SUMO1 mouse model. Mouse gene identification technology was used to identify the T95R mouse genetic mutation.The weight and blood glucose were detected of T95R mutation SUMO1 mouse. The myosin purification and proteomics were used to identify SUMO-modified sites.Results The T95R mouse model was established successfully by the mouse gene identification technology. The T95R site mutation homozygous mice and wild-type mice had the same trends in body weight and blood glucose through quantitative analysis. Proteomics technology showed that T95R homozygous mice could be detected different novel SUMO-modified myosin sites.Conclusion This study constructed a special SUMO1 mutation mouse model to provide a simple and feasible model for the identification of SUMOylation.The discovery of new SUMOylation sites is helpful to study the physiopathological function of myosin.

326 Expression and clinical significance of JAK1 in laryngeal squamous cell carcinoma

Objective To detect the expression of Tyrosine kinase1(JAK1) in laryngeal squamous cell carcinoma and analyze its correlation with clinicopathological features, in order to explore the clinical significance of JAK1.Methods The expression of mRNA and protein of JAK1 in laryngeal squamous cell carcinoma and adjacent tissues was detected by Real-time quantitative polymerase chain reaction (RT-qPCR) and Western-blot (Western blotting), the expression of mRNA and protein in laryngeal squamous cell carcinoma and paracancerous tissues was detected by immunohistochemical method, and the relationship between the expression and clinicopathological features was analyzed according to the method of immunohistochemical score.Results The expression of JAK1 in mRNA in laryngeal squamous cell carcinoma was significantly higher than that in paracancerous tissues, P<0.05; the expression of JAK1 in protein in laryngeal squamous cell carcinoma was significantly higher than that in paracancerous tissues, P<0.05; the results of immunohistochemical showed the expression of JAK1 in laryngeal squamous cell carcinoma was related to the degree of differentiation, clinicopathological stage and lymph node metastasis, P<0.05.Conclusion The increased expression of JAK1in laryngeal squamous cell carcinoma plays an important role in the occurrence, development and metastasis of laryngeal carcinoma.

330 Effect of apatinib mesylate tablets in the third-line treatment of extensive-stage small cell lung cancer

Objective To evaluate the clinical effect and safety of apatinib mesylate tablets in the third-line treatment of extensive-stage small cell lung cancer (SCLC).Methods All extensive-stage SCLC patients involved in this study were admitted and treated from January 2016 to June 2018, who were randomly divided into two groups: the apatinib group who received apatinib (500 mg/day) until the disease progressed or became intolerable, and the paclitaxel group who were treated with Paclitaxel (175 mg/m2, 3 weeks). Patients were matched for sex, age, smoking history, ECOG, organ metastasis, hemoglobin, etc. Progression-free survival (PFS), overall survival (OS) and adverse effects were evaluated.Results The median PFS in the apatinib group and the paclitaxel group were 3.7 months and 1.9 months, respectively, and the median OS were 6.6 months and 3.0 months, respectively. The PFS and OS were both significantly longer in the apatinib group than in those in the paclitaxel group, P<0.05. The adverse effects in the apatinib group such as hypertension, fatigue and elevation of transaminase were grade 1-2, and there were no grade 3 or above.Conclusion Apatinib in the third-line treatment is possible to improve the prognosis for extensive-stage SCLC patients, and there are no obvious adverse effects.

333 Clinical efficacy of raltitrexed by transcatheter arterial chemoembolization
n the treatment of advanced primary liver cancer

Objective To investigate the clinical efficacy and safety of the transcatheter arterial chemoembolization with raltitrexed and lipiodol emulsion in the treatment of advanced primary liver cancer.Methods The clinical data of 80 patients with inoperable primary and advanced liver cancer admitted to our department were studied. According to the treatment plan, they were divided into the treatment group and the control group, with 40 patients in each group. The short-term (1 month after surgery) efficacy of the two groups of patients was observed, including the median disease progression time, 1-year and 2-year survival rate, serum tumor markers (AFP, CA19-9), changes of transaminase and bilirubin, and adverse reactions.Results Three months after operation, RR and DCR in the treatment group were higher than those in the control group (P<0.01); the 1-year and 2-year survival rates in the treatment group were 81.6% and 57.5%, respectively, which were significantly higher than those in the control group (67.0% and 35.9%, respectively, P<0.05); median survival time in the treatment group was 11.6 months, higher than 7.3 months in the control group (P<0.05); the number of AFP, CEA, transaminase and bilirubin decreased in the treatment group 1 month after operation was higher than that in the control group (P<0.05); the comparison of the incidence of adverse reactions between the two groups showed no statistical significance (P<0.05).Conclusion The clinical efficacy and safety of retetroxel and iodized oil in the treatment of advanced primary liver cancer by intra-arterial embolization were significant.

336 Effect observation of gastrodia elata granule combined with nifedipine controlled-release tablet
in the treatment of cervical hypertension

Objective To observe the effect of gastrodia elata granule combined with nitradipine controlled-release tablet in patients with cervical hypertension.Methods One hundred and twenty patients with cervical hypertension were selected and randomly divided into 3 groups. In the experiment group, both gastrodia elata granule and nifedipine controlled-release tablets were taken orally, while in the first control group, nifedipine controlled-release tablets were taken orally, and in the second control group, gastrodia elata granule was taken orally. The treatment period was 4 weeks and the observation period was 8 weeks. Blood pressure, neck NDI score and neck NRS pain score before and after treatment were compared among the three groups after week 4 and week 8 respectively.Results Four weeks after the treatment, NRS pain score and NDI score in the neck of the three groups were all decreased, and the difference was not statistically significant. Eight weeks after the treatment, NRS pain score in the experiment group and the second control groups was lower than that in the first control group, P<0.05. Systolic blood pressure and diastolic blood pressure in the experiment group decreased 4 days after the treatment, and the antihypertensive effect of the experiment group and the first control group 8 weeks after the treatment was better than that of the second control group, P<0.05.Conclusion Gastrodia elata granule combined with nifedipine controlled-release tablets can significantly improve the clinical symptoms of neck and shoulder pain in patients with cervical hypertension. The antihypertensive effect is significant, and the side effects of western medicine are reduced. The patients′ satisfaction is high, so it is worthy of clinical promotion.

340 Analysis of laboratory indicators predicting the risk of death for severe fever with thrombocytopenia syndrome

Objective To analyze the correlation between laboratory indexes and prognosis in patients with severe fever and thrombocytopenia syndrome, and to establish a death model for verification. Methods Cases of severe fever with thrombocytopenia syndrome were collected. The case data from January 2011 to December 2015 were used as the modeling group to establish the death risk model. The case data from January 2016 to December 2016 were used as the validation group to verify the death model. The first time 11 laboratory examination results were selected after admission to ICU for analysis. Through ROC curve, laboratory indexes with area under the curve (AUC)>0.6 were selected to determine the best cutoff value of each index. Through the logistic regression analysis, the laboratory indexes that had influence on the prognosis were selected, and the original β value was assigned to them. At the same time, after the β value was rounded, the risk model was established and verified, and the sensitivity and specificity of different total scores were analyzed.Results The AUC of ALT, AST, CKMB, Cr, UA, BUM, Ca2+, K+ and Na+ were all >0.60. Logistic regression analysis showed that the increase of AST, K+, and BUN was directly related to the predicted death, respectively. The area under ROC curve was 0.908 and 0.907 respectively. The sensitivity and specificity were 97.6% and 95.2%.Conclusion The simplified scores of AST, K and BUN are of high value in predicting the risk of death in patients with severe fever and thrombocytopenia syndrome. Patients with a total score≥3 should be highly alert to the clinicians.

344 Observation of the complications after the myopic surgery corrected by transepithelial photorefractive keratectomy

Objective To explore the causes, prevention and treatment of complications in patients after transepithelial photorefractive keratectomy (transPRK).Methods The cases were retrospectively studied. One hundred and sixteen eyes of 58 patients were followed up and reexamined. The patients with complications were examined and treated. The patients were followed up for 3 months.Results The side effects and complications of transPRK were pain within 3 days, haze, delayed healing (1.72%), dry eye symptoms or original dry eye symptoms aggravation (18.97%), transient or persistent glare (5.17%), and hormone induced ocular hypertension (5.17%). After proper treatment of all the complications, the visual acuity of naked eyes reached or exceeded the corrected visual acuity before operation.Conclusion The causes of postoperative complications are related to many factors. Standardized preoperative evaluation and procedures can reduce the incidence of complications.

348 Comparative analysis of chin soft tissue angle before and after orthodontic treatment with extraction
in skeletal class Ⅱ and class Ⅲ Malocclusion adults with different skeletal vertical patterns

Objective To evaluate and compare the change of chin soft tissue in skeletal class Ⅱ, Ⅲ malocclusion adults with different skeletal vertical patterns before and after orthodontic treatment, and to evaluate the sensitivity of improvement of soft tissue profile after orthodontic treatment between the different skeletal malocclusion and vertical patterns.Methods Skeletal class Ⅱ and class Ⅲ malocclusion adult patients were randomly selected, and divided according to the vertical growth pattern, with 10 cases in each group. X-ray lateral cephalograms were taken at pre-and post-orthodontic treatment, and the related variables of chin soft tissues were measured and statistically analyzed.Results After treatment, the LL-B′-Pog′ was significantly reduced in the groups of high-angle and normal-angle in skeletal class Ⅱ and class Ⅲ malocclusion, P<0.01, but the high-angle group showed a more significant reduction than the normal-angle group, P<0.05. There were increases in Pog-Pog′, Pog′B′-FH, and B′LL-FH in the group of high-angle after treatment, P<0.05, whereas the LL-Pog′ showed a reduction, P<0.01. In the group of low-angle, the LL-E was markedly reduced, P<0.01. Obvious reductions showed in LL-E, B′LL-FH, and Pog-Pog′ in the group of high-angle in skeletal class Ⅲ malocclusion after treatment, P<0.05, meanwhile, distinct increases showed in B′-LLPog′ and Pog′B′-FH, P<0.01. There were no statistical differences in the normal-angle group except the reduction of B′LL-FH, P<0.01. In the group of high-angle, the skeletal class Ⅲ malocclusion showed a more significant variation than the class Ⅱ malocclusion in the LL-B′-Pog′, Pog-Pog′, and B′LL-FH, P<0.01, the change of mentolabial angle was more distinct in the skeletal class Ⅲ malocclusion than the skeletal class Ⅱ malocclusion in normal-angle group, P<0.05.Conclusion The chin soft tissue and lateral appearance tend to be in harmony after orthodontic treatment with extraction in adults. There was a more significant change of the chin soft tissue in the high-angle group on account of orthodontic treatment with extraction, especially in the skeletal class Ⅲ malocclusion.

353 Experimental study of hyaluronic acid combined with metronidazole
or periocline in the treatment of chronic periodontitis

Objective To study the anti-inflammatory effect of hyaluronic acid combined with metronidazole or periocline on chronic periodontitis.Methods The periodontitis model of rats was established. According to the negative control group, the positive control group, the metronidazole group, the hyaluronic acid group, the metronidazole combined with hyaluronic acid group, the periocline group, and the periocline combined with hyaluronic acid group, the rats were divided into 7 groups, with 10 rats in each group. The expression levels of TNF-α and IL-1β protein in gingival crevicular fluid were measured by ELISA 2 h, 6 h, 24 h and 5 d after the treatment, then MMP8 and TIMP1 expressed in gingival epithelium were measured 24 h and 5 d after the treatment.Results Two hours after the treatment, the expression of TNF-α and IL-1 β in all experiment groups was significantly inhibited, P<0.01, especially in hyaluronic acid group, metronidazole combined with hyaluronic acid group, and periocline combined with hyaluronic acid group, P<0.01; 24 hours after the treatment, the inhibition of IL-1 β in metronidazole combined with hyaluronic acid group was significant; 5 days after the treatment, periocline combined with hyaluronic acid group still had a slow inhibition of TNF-α. Twenty-four hours after the treatment, MMP8 expression in all experiment groups showed a downward trend, P<0.01, while the expression of TIMP1 significantly increased, P<0.01, especially in metronidazole combined with hyaluronic acid group, periocline group and periocline combined with hyaluronic acid group; 5 days after the treatment, the expression of TIMP1 in Metronidazole combined with hyaluronic acid group showed an upward trend, P<0.01.Conclusion Local application of metronidazole, hyaluronic acid and Periocline has obvious effect on the early stage of inflammation, but the long-term maintenance effect is not good; the combination of metronidazole and hyaluronic acid, periocline and hyaluronic acid is better than single application. In contrast, the combination of metronidazole and hyaluronic acid has more obvious advantages in five-day continuous anti-inflammatory effect.

359 Diagnostic value of high frequency ultrasound in traumatic neuroma

Objective To study the diagnostic value of high frequency ultrasound in traumatic neuroma.Methods Thirty-two cases with traumatic neuroma were retrospectively collected and analyzed. High-frequency ultrasound was used to observe the location, size, morphology, number, internal echo status, internal blood flow, and the relationship with peripheral nerve and surrounding tissues.Results Thirty-two cases were collected, and the average age was (43.1±16.6) years old, with 29 cases under 60 years old; the largest lesion was 2.1 cm ×2.7 cm, and the smallest was 0.4 cm ×0.2 cm; 23 cases were oval, 6 cases were round-like, and 3 cases were irregular; 23 cases were with clear boundary and 9 were with blurred boundary; 21 cases showed uneven hypoechoic inside, 9 cases had a little hyperechoic inside, and 2 cases had a mixed echo inside; there were 27cases with nerve continuation, but no significant neurological continuity was found in 5 cases; color Doppler examination showed no obvious blood flow signal in 20 cases, 6 cases of peripheral blood flow signal, 5 cases of internal blood flow signal and 1 case of peripheral and internal blood flow signal; there were 20 patients with pain, numbness and other sensory abnormalities after pressure.Conclusion High frequency ultrasound can effectively evaluate traumatic neuroma and provide effective information for clinical treatment plan.

362 Comparative study of multimodal magnetic resonance imaging and ultrasonography
in evaluating the diagnosis staging and radiotherapy effects of cervical cancer

Objective To explore the comparative study of multimodal magnetic resonance imaging and ultrasonography in the diagnosis of cervical cancer and the efficacy of radiotherapy.Methods Totally 396 patients with cervical carcinoma who received assessment by multimodal MRI or color Doppler ultrasonography in The Third People′s Hospital of Linyi from October 2015 to October 2018 were recruited in this study. They were the multi-modality MRI group (n=201 ) and the color Doppler ultrasound inspection group (n=195 ). The results were analyzed and compared. The multi-modal MRI findings of cervical cancer before and after treatment were compared, and the effect of enhancement on multimodal magnetic resonance imaging was evaluated.Results The diagnostic coincidence rate of multi-modality MRI in detecting stage I and stage II cervical cancer was higher than that of color Doppler ultrasound (P<0.05), but there was no significant difference between the diagnostic coincidence rate of stage III and stage IV. The overall sensitivity of multi-modality MRI to evaluate the staging of cervical carcinoma was 94.6% and specificity was 92.8%. The overall sensitivity of color Doppler ultrasonography was 83.4% and specificity was 82.6%. There was a significant difference between the two (P<0.05). In roc curve analysis, the area of the multi-modal MRI curve was significantly larger than that of color Doppler ultrasonography, and the sensitivity and specificity were significantly higher than that of color Doppler ultrasonography (P<0.05). The accuracy of CR and PR in the evaluation of radiotherapy efficacy in patients with cervicalcancer by multi-modal MRI were lower than that of color Doppler ultrasonography (P<0.05). The enhancement of multi-modal MRI examination was significantly correlated with the evaluation of tumor reduction rate (r=0.649, P<0.01) and radiotherapy effect (r= 0.679, P<0.01). Comparing the multi-modal MRI findings of cervical cancer before and after treatment, it was found that multi-modal MRI could clearly show the size of the tumor and its relationship with the surrounding tissues and organs, and the lesions disappeared after radiotherapy.Conclusion Compared with color Doppler ultrasound, multi-modality MRI is of great value in the accurate diagnosis and staging of cervical cancer and provides a strong basis for the evaluation of radiotherapy effect of cervical cancer.

367 Study on treatment mechanism of heart failure for astragali radix based on network pharmacology

Objective To study the mechanism of astragali radix against heart failure based on network pharmacology.Methods The potential target of astragali radix against heart failure and signal pathways were predicted by active ingredient screening and target prediction technology, combined with bioinformatics. Finally, we explored the molecular mechanism of astragali radix in the treatment of heart failure.Results Twenty corresponding chemical constituents in astragali radix were screened through the TCMSP data, and 121 targets of these components were found using target prediction technology. Meanwhile, 10 962 known genes related to the occurrence and development of heart failure were retrieved from the gene databases. Eight key targets were selected by string database screening and network topology analysis in Cytoscape software. GO enrichment analysis showed that astragali radix played an important role in anti-heart failure by participating in 10 key signal pathways such as PI3K/AKT, p38MAPK, and MAPK/NF-κB, which were related to 10 biological functions including Cellular energy metabolism response, changes in blood vessel environment, hormone activity, nerve activity and receptor function.Conclusion As a traditional Chinese medicine, the effect of astragali radix against heart failure has the characteristics of multi-component, multi-target and mutual synergy.

373 Study on antioxidant capacity of paeonia extract

Objective To determine the contents of Paeoniflorin in radix paeoniae rubra, and to analyze the antioxidant activities in vivo.Methods Paeoniflorin, an antioxidant component in radix Paeoniae rubra, was extracted by ultrasonic extraction method. An extraction agent with strong antioxidant activity was selected through in vitro antioxidant experiments. The effective component was further separated and purified by silica gel column chromatography. Paeoniflorin content was determined by HPLC.Results Seventy percent ethanol solution was selected and ultrasonic extracting was conducted from radix paeoniae rubra for 50 minutes according to the ratio of material to liquid of 1∶30. After extracting with n-butanol, silica gel column chromatography was used to obtain 2.34 % paeoniflorin. The in vitro experiments of the extracted part proved that the antioxidant activity of the product obtained by n-butanol extraction was the most optimal.Conclusion Paeoniflorin isolated and purified after ultrasonic extraction has been proved to have antioxidant effects in vitro experiments.

377 Study on the allocation of public health human resources in community hospitals
under novel coronavirus epidemic situation

Under the influence of the new coronavirus pneumonia epidemic, public health human resources in community hospitals are one of the main forces in the emergency state of epidemic prevention and control. The quantity, quality and distribution of public health human resources are the influencing factors of epidemic prevention and control. Establishing an information sharing mechanism, rationally arranging public health human resources, and implementing grid management, and applying for transfer if necessary are issues that every community hospital needs to think about. By investigating the public health human resources of 9 representative community hospitals, the common problems and causes are analyzed, and corresponding suggestions are made.

381 Study on influencing factors of health equity of middle-aged and elderly residents in China

Objective To analyze the current situation of the health equity of middle-aged and elderly residents in China and its contributions of its influencing factors.Methods Data came from the CHARLS 2015 survey, the middle-aged and elderly people aged 45 and above were selected as the research objects, and the self-rated health status was used as the result variable of health status. The concentration index and concentration index decomposition were used to evaluate and explore the health equity and its influencing factors.Results The good proportion of self-rated health of middle-aged and elderly residents in China was 29%, the concentration index was 0.0614, and the horizontal inequity index of the health level fairness index was 0.0102. the main influencing factors of health equity are the economic level, education level, and urban-rural distribution.Conclusion There is a certain degree of inequity in the health status of middle-aged and elderly people in China. Economic factors are the most important contribution of inequity. Therefore, improving the economic status of low-income middle-aged and elderly people will help promote the health and health equity.

384 Investigation and research on the health status of elderly people living at home in rural area of Binzhou

Objective To understand the health status of elderly people living at home in rural area of Binzhou, and further explore the factors affecting the health status of elderly people, so as to provide reference for relevant government departments to formulate health promotion policies for elderly people living at home in rural area.Methods Four hundred and sixty-five elderly people in rural area of Binzhou were investigated by questionnaire and SF-36 health survey.Results The health status of elderly people living at home in rural area of Binzhou was not very ideal. Scores of the six dimensions including physiological function (PF), role physical (RP), bodily pain (BP), general health (GH), social function (SF) and role emotional (RE) were all lower than the norm scores, P<0.05, while scores of the two dimensions of vitality (VT) and mental health (MH) were higher than the norm scores, P<0.05. The health status of rural residents in Binzhou was related to gender, age, marital status, living style and monthly income in 8 dimensions, P<0.05, and the difference was statistically significant, P<0.05.Conclusion The general health status of the elderly in rural areas of Binzhou is not very ideal, and relevant government departments should give reasonable health promotion according to the actual situation of the elderly in the area.

388 Study of compound localization in PICC ectopic in tumor chemotherapy patients

Objective To investigate the clinical effect of combined localization in peripherally inserted central venous catheter (PICC) chemotherapy of tumor patients.Methods One hundred and twenty patients with PICC were selected as the research objects, and they were randomly divided into the control group and the observation group, with 60 cases in each group. PICC external measurement and catheterization were used in patients in the control group, and PICC combined positioning method was used in patients in the observation group. The success rate of PICC catheterization (including single catheter success rate, tip position optimal rate, catheter final placement rate) and patient satisfaction were compared between the two groups.Results The success rate of PICC in the observation group was much higher than that in the control group, P<0.05. The satisfaction of the PICC in the observation group was much higher than that of the control group, P<0.05.Conclusion Tumor chemotherapy patients use the PICC composite positioning method to measure high accuracy, the success rate of one-time catheter placement, the optimal rate of the tip position, and the final catheter placement rate are high, which can effectively improve the success rate of PICC catheterization, prevent and adjust ectopic PICC catheter, improve patient satisfaction. It is safe and effective, so it is worthy of clinical promotion.

392 A review of the correlation between psoriasis and cardiovascular disease

Psoriasis is a chronic, recurrent, inflammatory disease of unknown etiology, mainly manifested as skin damage. In recent years, cardiovascular disease has gradually ranked first in the global burden of disease, and has become a public health issue that governments of all countries focus on. The cases with high risk of psoriasis complicated with cardiovascular disease show that psoriasis may be one of the risk factors for cardiovascular disease. Studying the correlation between psoriasis and cardiovascular disease is helpful to further understand these two complex chronic diseases and provide them with more diverse diagnosis and treatment. This article briefly summarizes the correlation between psoriasis and cardiovascular disease in recent years in terms of pathogenesis-related mechanisms.

395 Advanced progress of mutp53 in diffuse large B-cell lymphoma

Diffuse large B-cell lymphoma is one of the heterogeneous hematological malignancy accounting for approximately a third of non-Hodgkin lymphomas, with versatile clinical, morphological and molecular characteristics. P53 is an important tumor suppressor gene, which inhibits the occurrence and development of tumors by inducing cell cycle arrest, DNA repair and apoptosis, and regulating multiple signal transduction pathways. The cell cycle disorder and the inactivation of signal transduction pathway caused by p53 mutation are factors in the occurrence and progression of many malignant tumors. This article reviews the role of mutationtype p53 (mutp53) in diffuse large B-cell lymphoma and its possible mechanisms.
