摘要目的 了解不同性别、年龄组的疾病负担及其变化趋势,为厦门市胰腺癌防治工作提供科学依据。 方法利用死因监测系统收集2010—2014年厦门市居民胰腺癌死亡数据,运用早死所致的健康生命损失年(years of life lost, YLL)、残疾所致的健康生命损失年(years lost due to disability, YLD)、伤残调整寿命年(disability adjusted life year, DALY)对胰腺癌疾病负担进行测量、分析。 结果2010—2014年厦门市居民胰腺癌死亡率3.06/10万,标化率3.13/10万;男性死亡率3.90/10万,标化率4.02/10万;女性死亡率2.23/10万,标化率2.25/10万;男性死亡率、标化率均高于女性(P1<0.001,P2<0.001)。每千人口胰腺癌YLL、YLD、DALY分别为2.261 7、0.000 6、0.262 3,其中,男性(0.320 2、0.000 7、0.320 9)高于女性(0.203 6、0.000 4、0.204 0),疾病负担主要集中在50~69岁人群,60~69岁组(1.055 2、0.003 2、1.058 4)负担最重。 结论YLL是厦门市居民胰腺癌疾病负担的主要来源;30岁以上男性及50~69岁老年群体是重点防治人群。
Abstract:Objective To provide scientific basis for prevention and treatment of pancreas cancer in Xiamen City by understanding the burden of the disease and its changing trend in different gender and age groups. Methods In Xiamen city, the data of pancreas cancer death of residents were collected from the death cause surveillance system. The years of life lost (YLL), years lost due to disability (YLD) and disability adjusted life year (DALY) were used to calculate and analyze the burden of patients with pancreas cancer from 2010 to 2014. Results From 2010 to 2014, the average mortality rate of pancreas cancer was 3.06 per 100 000 persons, and the standardized mortality rate was 3.13 per 100,000persons; for the male, the average mortality rate was 3.90 per 100 000 persons, and the standardized mortality rate was 4.02 per 100 000 persons; for the female, the average mortality rate was 2.23 per 100 000 persons, and the standardized mortality rate was 2.25 per 100 000 persons. The YLL, YLD and DALY of Xiamen residents were 2.261 7, 0.0006 and 2.262 3 per 1 000 persons; those of the male (0.320 2, 0.000 7, 0.320 9 per 1 000 persons) were higher than those of the female (0.203 6, 0.000 4, 0.204 0 per 1 000 persons); the disease burden was centered upon 50~69 year-old people and the burden of 60~69 year-old people (1.055 2, 0.003 2, 1.058 4 per 1 000 persons) was the heaviest. Conclusion The main cause of the disease burden was YLL; we should pay more attentions to preventing and curing pancreas cancer especially on the male over 30 years old and the elderly population between 50 and 69 years old.
