我国60岁及以上居家老年人失能情况及影响因素分析

Abstract Objective To learn about the disability status of the elderly aged 60 and above at home in China and its influence factors, and provide evidence for improving the health status of the elderly population and making corresponding policies for the elderly. Methods Data of Chinese national health service survey and Katz index scale were used to analyze the disability status of the elderly population in different regions and different social demographic characteristics, different family economic status and different health status and the factors influencing their disability. Results The survey results showed that 6.7% of the elderly people had different degrees of disability; rural disability was more serious than urban disability; with the increase of age, the degree of disability increased, and 20.1% of the population aged 80 and above had different degrees of disability. Multivariate logistic regression analysis showed that factors affecting disability were sex, age, marital status, education, economic status, chronic illness status, physical exercises, hearing, vision, dementia etc. Men (OR=1.369), old age (OR_{group 70-79}=1.568, OR_group≥80=3.472), widowed (OR=1.248), with chronic diseases (OR_{with 1 chronic disease}=2.140, OR_{with 2 chronic disease}=3.478, OR_{≥3 chronic disease}=5.764), visual difficulty (OR_{moderate difficulty}=1.249, OR_{extremely difficulty}=1.538), hearing difficulty (OR_{need to raise voice}=1.647, OR_{can′t hear clearly}=3.271), dementia（OR=6.559）were the risk factors. High education level (OR_{Primary school}=0.876, OR_{junior middle school}=0.861, OR_{senior high school or above}=0.757), high income (OR_{middle income group}=0.833, OR_{higher income group}=0.856, OR_{highest income group}=0.858), Active physical exercise every week (OR_{1-2 times}=0.405, OR_{3-5 times}=0.357，OR_{≥6 times}=0.331) were the protective factors of disability of the elderly. Conclusion In order to reduce the occurrenceof disability, it is necessary to strengthen health education, strengthen exercises, provide suitable working and living environment for the elderly to avoid falls and maintain social adaptability, and effectively prevent chronic diseases and strengthen chronic disease management to delay the loss of function of the elderly.
