321 Forecast analysis of body mass index of the newly enrolled students of one university in Weifang

Objective To understand the development law of the body mass index to the Newly Enrolled Students of a university in weifang, and to provide theoretical basis and data support for the rational development of physical health promotion measures for university students. Methods Totally 30 609 newly enrolled students from 2008 to 2016 were selected from a university in weifang, and the data of their entrance examination were adjusted to calculate the average mass index of the year. The grey-Markov method was used for forecast and analysis. Results We Built the grey-Markov modified model: G(k)=Y(k)-12(Ai+Bi). The prediction accuracy was in the first level after the posterior error ratio (C=0.088) and the small error probability (P=0.987) test. The average body mass index of the newly enrolled students of one university during the 2017-2020 years were 20.69, 21.33, 21.39, and 21.45, and the figures of Markov status were 3, 1, 2, and 3. Conclusion The university students′ body mass index is in normal range for the period of 2017-2020, but the increase trend is clear. Therefore, it is necessary to strengthen physical monitoring, reasonable intervention and health literacy, to guide college students to exercise scientifically and control their weight reasonably in case of the overweight and obesity risk caused by the rise of BMI.

324 Quality evaluation of rural primary medical service in Yantai under the new rural cooperative system

Objective Through the analysis of input and output indicators of township hospitals, to master the status quo of rural primary health care service under the new rural cooperative medical system, and to better evaluate the quality of primary health care services, in order to promote the quality of rural primary health care services. Methods Through the use of random sampling method, 16 township hospitals in 18 counties in Yantai were selected, and the relevant data were for DEA analysis. Results In the three years from 2013 to 2015, the mean value of DEA was above 0.95, which showed that under the new rural cooperative medical system, the township hospitals could better serve the local people. But the development of health care service in Yantai was not balanced. In 48 calculations of 16 township hospitals in the 3 years, a total of 27 DEA was effective, and DEA effective rate was 56.25%. And the medical service quality of county a and g within three years was at an effective level, while that of county c did not reach the effective level. To a certain extent, this could reflect that the more economically developed the area, the better the rural primary health care service quality. Conclusion There are regional differences in the quality of service in township hospitals of Yantai, and there is promotion space, so we need to further strengthen the management to improve the efficiency of health resource allocation.

329 Changes of hemoglobin in the treatment of moderate and severe traumatic brain injury by hyperbaric oxygenation combined with progesterone and prognostic relevance

Objective To study the effects of hyperbaric oxygenation combined with progesterone on hemoglobin and prognosis in moderate and severe traumatic brain injury. Methods Ninety patients with moderate and severe traumatic brain injury (TBI) from January 2016 to June 2016 were randomly divided into three groups. Hyperbaric oxygenation, progesterone, and hyperbaric oxygenation combined with progesterone treatment were used respectively in three groups. We compared changes of progesterone, estradiol and testosterone in the serum, Glasgow coma (GCS) score, and Glasgow prognosis (GOS) score among groups. Results After the treatment, the levels of progesterone and estradiol in hyperbaric oxygenation combined with progesterone group were significantly higher than those in progesterone group and hyperbaric oxygenation group, while testosterone level was significantly lower than that in progesterone group and hyperbaric oxygenation group, with significant difference (P<0.05). After the treatment, the GCS score in hyperbaric oxygenation combined with progesterone group was significantly higher than that in progesterone group and hyperbaric oxygenation group, and the difference was significant (P<0.05). In the six-month follow-up, the GOS score in hyperbaric oxygenation combined with progesterone group was significantly higher than that in progesterone group and hyperbaric oxygenation group, with significant difference (P<0.05). Conclusion Hyperbaric oxygenation combined with progesterone can effectively improve the levels of progesterone and estradiol, decrease the testosterone level, and improve prognosis for the treatment of moderate and severe TBI.

332 Path analysis on career development of new nurses

Objective To reveal the causal relationship among vocational values, career maturity and career development including job selection and self career choice, and to provide intervention basis for career development of new nurses. Methods With self-design questionnaire, we collected data of 148 cases of new nurses, mainly including general data, vocational values, career maturity, and career development. Path table and path diagram were used to reveal the causal relationship among different dimensions. Results According to the size of the path coefficient, the dimensions of self-affirmation (0.340), occupation recognition (0.323) and information use (0.153) of career maturity directly affected job selection, but the three dimensions of vocational values (0.131, 0.129, and 0.182 respectively) indirectly affected job selection through self-affirmation. These dimensions such as self-affirmation (0.311), occupation recognition (0.145), the use of information (0.100), independence (0.141) and economic returns (0.125) directly affected the self career choice. Besides, the dimensions of independence (0.166) and economic returns (0.129) through self-affirmation indirectly affected self career choice. Conclusion Many factors affect the career development of novice nurses. Path analysis method is required to reveal the causal relationship.

336 Relationship of quality of life, social support and life events in menopausal women

Objective To explore the related factors of quality of life among menopausal women, and to examine the interaction effect of stressful life events and social support on quality of life. Methods Totally 413 menopausal women were selected using convenient sampling method. Menopause-specific quality of life questionnaire, social support rating scale, and life event scale were used for evaluation. Results The total score of quality of life was 63.91±29.57, and physical symptoms was the most important health problem of menopausal women. The total score of social support was 42.72±6.95, and that of life event was 34.6±8.72. Life events and social support had direct effect on life quality of menopausal women and life events had indirect effect on life quality of menopausal women through social support. Conclusion The results suggest improvement of social support, particularly among those who have recently experienced significant life events, may be important to improve the quality of life in menopausal women.

340 Statistical analysis of the application of continuity nursing management measures after resection of vocal cord polypectomy

Objective To research into the application of continuity nursing management measures after vocal cord polypectomy. Methods Totally 130 patients with vocal polyps polypectomy in our hospital were selected as the research objects. According to the order of surgery, they were divided into the observation group (69 cases) and the control group (61 cases). The control group received routine nursing management measures, and the observation group was given continuous nursing management measures from the 7th after the operation on the basis of the treatment in the control group. The vocal cords recovery, voice acoustic recovery and the satisfaction of the patients in both groups were compared and analyzed. Results Fourteen days, 30 days and 90 days after the operation, the recovery rates of vocal cords in the observation group were 20.29%, 82.61% and 95.65% respectively, significantly higher than those in the control group (4.92%, 57.38% and 83.61%, respectively), and the differences were statistically significant (P<0.05). There was no significant difference in voice acoustic parameters between both groups (P>0.05) 7 days after surgery. Fourteen days, 30 days and 90 days after the operation, the levels of Jitter and Shimmer in the observation group were significantly lower than those in the control group, the levels of MPT and dSI were significantly higher than those of the control group, and the difference was statistically significant (P<0.05). The overall satisfaction degree of the patients in the observation group was 91.30%, significantly higher than that in the control group (77.05%, P<0.05). Conclusion Carrying out continuity nursing management measures for patients with vocal cord polyp after the operation can significantly improve the patients′ self-management ability, promote the vocal cord and voice function of the rapid recovery effectively, so that patients′ satisfaction with the nursing work greatly improved.

344 Effectiveness evaluation analysis of comprehensive intervention on children with attention deficit-hyperactivity disorder

Objective To explore the intervention effect of comprehensive intervention on children with attention deficit-hyperactivity disorder (ADHD). Methods Eighty children with ADHD were randomly divided into the control group and the observation group, 40 cases respectively. The control group was given Ritalin treatment and usual care, and the observation group was added comprehensive intervention measures on the basis of the treatment in the control group. After 3 months of intervention, these indexes such as behavior scores, sustained attention and total effective rate of treatment were compared between the two groups. Results After intervention, the scores of behavior, learning, anxiety and impulsive hyperactivity in the observation group were much lower than those in the control group (P<0.01). The total effective rate of improvement of sustained attention in the observation group was 87.5%, much higher than that in the control group (42.5%, P<0.01). And the total effective rate of treatment in the observation group was 92.5%, higher than that in the control group (70.0%, P<0.05). Conclusion Carrying out comprehensive intervention measures could obviously improve behaviors of children with ADHD, raise sustained attention, and increase therapeutic efficacy. So it deserves promotion in clinical practice.

347 Risk factors and nursing strategy of hospitalized patients with hospital acquired infection in the department of gastroenterology

Objective To explore the risk factors and the nursing strategy of hospitalized patients with hospital acquired infection (HAIs) in the department of gastroenterology. Methods Totally 600 hospitalized patients in the department of gastroenterology were selected as research objects. 42 cases were with HAIs and belonged to the infection group, and 558 cases were not with HAIs and belonged to the non-infection group. The common parts of infection was counted. Single factor analysis and logistic analysis were used to analyze the relations of 13 factors (age, diabetes, digestive tract tumors, digestive system surgical procedures, invasive procedure, hormonotherapy, combination use of antibiotics, length of stay, hypersensitive C-reactive protein (hs-CRP), serum albumin, hemoglobin, hyponatremia and hypokalemia) with HAIs, in order to identify risk factors. The nursing strategy was made according the risk factors. Results The total infection rate was 7%, including 24 cases in respiratory tract (57.14%), 7 cases in urinary system (16.67%), 6 cases in digestive tract (14.29%), 2 cases in blood (4.76%), 2 cases in skin and mucosa (4.76%), and 1 case in others (2.38%). Single factor analysis showed that hyponatremia and hypokalemia were irrelevant to HAIs (P>0.05). The logistic analysis showed that invasive procedure (OR:3.400), the level of serum albumin<40g/L (OR= 2.536), combination use of antibiotics (OR= 2.494), diabetes (OR= 2.427), digestive tract tumors (OR=2.300), age≥60 (OR= 2.176) and hs-CRP≥10mg/L (OR=2.014) were the risk factors of HAIs (P<0.05). Conclusion Inpatients in the department of gastroenterology have a low ratio of HAIs. The nursing strategy made according the risk factors can effectively reduce the ratio of HAIs.
