241 Statistical analysis of disease spectrum of death cases in a hospital from 2003 to 2016

Objective To study the distribution and trend of death cases during 14 years in a general hospital in Shenzhen, and to provide reference for reducing the mortality rate and guiding the scientific development of the hospital. Methods The data of 3 975 cases of death from 2003 to 2016 were analyzed retrospectively with Excel 2007 and SAS 9.1 software. Results In the past 14 years, the mortality rate of inpatients in the hospital showed a decreasing trend, with an average mortality rate of 0.72%. The fatality rate of male patients was higher than that of female. The mortality rate of inpatients in December, January and February was higher each year. Among inpatients below 20 years old, the mortality rate of inpatients from 15 years old to 19 years old was the highest (0.41%), followed by those below one year (0.25%). Inpatients over 20 years old showed a gradual increase in mortality rate with the growth of age. The first three causes of death were cancer, circulatory system diseases, and respiratory diseases, accounting for 77.21%. Departments including Intensive Care, Oncology, Minimally Invasive Intervention, Cardiology, Gastroenterology, Neurosurgery, Respiratory Medicine and Hematology were the main departments of death in all clinical departments, accounting for 83.1%.Conclusion The hospital can allocate medical resources in different sex, age, month, disease, department and so on to carry out diversified and multi-level medical service work, so as to improve the survival rate and quality of life of inpatients.

245 Dynamic changes of blood glucose and its influencing factors in the elderly

Objective To monitor the dynamic changes in blood glucose, analyze its influencing factors in elderly patients with type 2 diabetes mellitus in Qiqihar,and provide data support for targeted prevention and control measures. Methods Questionnaires, physical measurements and laboratory tests were used to collect relevant baseline information of the patients. We have monitored dynamically the levels of blood glucose for two years.According to the degree of variation of blood glucose at different time of the same patient,these patients were divided into the stable group and the instable grooup. Logistic regression was used to analyze the influencing factors of the glucostasis in the blood glucose monitoring and SAS 9.2 software was used for statistical analysis. Results The survey involved 486 people, including 264 (54.32%) males, and 222 (45.68%) females, with the sex ratio 1.19:1. The number of stable blood glucose patients was 314, and there were 172 people with unstable blood glucose, with the proportion of blood glucose stability 64.61%. Sleeping time, exercising time and smoking had an impact on blood glucose stability in the multivariate analysis of blood sugar stability in diabetes mellitus. Sleeping time which was too short or too long was a risk factor for blood glucose instability in diabetes mellitus patients. Exercising time was a protective factor for blood glucose instability in diabetes mellitus patients (OR=0.436). Smoking was a risk factor for blood glucose instability in diabetes mellitus patients (OR=5.641). Conclusion Sleeping time, exercising time and smoking were the risk factors for blood glucose instability in diabetes mellitus patients. The staff in the Community Health Service Center should strengthen health education in this part of diabetes patients and advocate a healthy lifestyle.

250 Comprehensive analysis of 120 front sheets of death medical records

Objective To analyze missing items and scoring of front sheets of death medical records , in order to improve the quality of medical records and strengthen quality control. Methods One hundred and twenty front sheets of death medical records from January to December in 2016 were checked according to the quality criteria of Hospital medical record home page data published by the National Health and Family Planning Commission, and the differences of front sheets between the internal medicine and surgery were compared. Results Many missing items of front Sheet of death medical record home page were found. The whole complete rate was 90.00%. General score was 90.68±4.71 points. Medical records rated as best and good accounted for 5.8% and 51.7% respectively. The rating had no difference between internal medicine and surgery. Conclusion The quality of front sheets of death medical records was poor. Various methods should be taken to strengthen staff training, improve the understanding of the knowledge of the medical record home page, and promote quality improvement, so as to ensure the quality of medical information and medical safety.

254 Analysis of risk factors associated with occurrence of dermatitis in ICU patients with fecal incontinence

Objective To investigate the characteristics of incontinence associated dermatitis (IAD) and influencing factors in ICU patients. Methods Totally 105 patients who were admitted into our hospital from Oct. 1, 2015 to Oct. 31, 2016 and met the conditions were selected for the study, and the factors influencing the occurrence of IAD were analyzed. Results The results showed that factors such as APACHEⅡscore, albumin, daily fecal incontinence more than twice, the distribution of stool characteristics over the body, Braden score less than 11 points might be related to the occurrence of IAD (P<0.05). By logistic regression factors, the results showed that watery or loose stool, and Braden score lower than 11 points were independent risk factors (P<0.05). Conclusion A considerable part of ICU patients with fecal incontinence will develop IAD in a short time, especially in patients with low Braden score and shapeless or watery stools, which has some significance for the prevention and treatment of IAD.

257 Effect evaluation analysis on early enteral nutrition support by mouth for colorectal cancer patients in the perioperative period

Objective To explore the effect of early enteral nutrition support by mouth for colorectal cancer patients in the perioperative period. Methods One hundred and ten colorectal cancer patients were randomly divided into control group and observation group, respectively 55 cases. The control group was given parenteral nutrition support method, and the observation group was given enteral nutrition support method. The indexes of postoperative recovery, immunity and nutrition were compared between the two groups after operation. Results Seven days after operation, the indexes of first exhaust time, length of stay, life quality and infection rate of incisional wound in the observation group were much lower than those in the control group (P<0.05). The increasing level of CRP in the observation group was much lower than that in the control group (P<0.01), while the increasing levels of Ig A, Ig G, CD4+ and CD4+/CD8+ in the observation group were much higher than those in the control group (P<0.05). The levels of serum albumin, prealbumin and transferrin in the observation group were much higher than those in the control group (P<0.05). Conclusion The early carrying out of enteral nutrition support by mouth for colorectal cancer patients could raise immune ability, improve nutritional status, and speed up disease recovery. So it deserves promotion in clinical practice.

260 Curative effect analysis of escitalopram combined with group psychotherapy on anxiety disorder of type 2 diabetes patients

Objective To explore the curative effect of escitalopram combinated with group psychotherapy on anxiety disorderof type 2 diabetes patients. Methods Sixty-four type 2 diabetes patients with anxiety disorder were randomly divided into two groups, the first group was only given escitalopram, and the second group was added group psychotherapy. SCL-90 was used to evaluate anxiety-depression symptoms and the improvement, and these indexes of FPG, 2hPG, HbA1c FINS and HOMA-IR were used to evaluate glycometabolism and insulin resistance. Results Before treatment, there was no statistical significance between the control group and the combination group about different factors scores and total score (P>0.05). After treatment, different factor scores and total score in the combination group were much lower than the control group (P<0.05). Before treatment, there was no statistical significance between the control group and the combination group about blood sugar and blood lipid indexes (P>0.05). After treatment, the indexes of 2hPG, HbA1c, FINS and HOMA-IR in the combination group were much lower than those in the control group (P<0.05). Conclusion Escitalopram combined with group psychotherapy on anxiety disorder of type 2 diabetes patients could effectively cut down postprandial blood glucose level and improve anxiety-depression emotion.

263 Clinical value of transvaginal and transabdominal ultrasonography in diagnosis of benign uterine leiomyoma

Objective To research into the clinical value of transvaginal and transabdominal ultrasonography in diagnosis of benign uterine leiomyoma. Methods Totally 413 patients with benign uterine leiomyoma (632 fibroids) in the department of obstetrics and gynecology in our hospital were selected as the research objects. We Analyzed the results of transvaginal ultrasound and transabdominal ultrasound retrospectively. According to the pathological results, we analyzed and compared the results of transvaginal ultrasound, transabdominal ultrasound and combined diagnosis. Results In the diagnosis of different types of uterine fibroids, the diagnostic accuracy of transvaginal ultrasound was significantly higher than that of transabdominal ultrasound besides the subserous myoma, and the difference was statistically significant (P<0.05). The diagnostic accuracy of transvaginal ultrasound and transabdominal ultrasound in the diagnosis of uterine fibroids was 93.51% and 73.10%, respectively, and the diagnostic accuracy rate of combined diagnosis was 99.53%, which was significantly higher than that of single diagnosis (P<0.05). Conclusion In the ultrasonic diagnosis of benign uterine leiomyoma, the diagnostic accuracy of transvaginal ultrasound was significantly higher than that of transabdominal ultrasound. The combined application of the two kinds of ultrasonic diagnosis further improves the accuracy of clinical diagnosis, and avoids the misdiagnosis and misdiagnosis of uterine fibroids to a great extent.

266 Research on the belongingness of medical staff from a tertiary hospital in Xuzhou with Structural Equation Model

Objective To analyze the belongingness of medical staff from a tertiary hospital in Xuzhou with structural equation model (SEM), and to take effective measures to enhance the sense of belonging and promote the harmonious and stable development of hospital medical personnel. Methods The self-designed ″survey scale of belongingness of hospital medical staff ″ was used to investigate the belongingness of medical staff in a tertiary hospital in Xuzhou, and Liser8.7 was used to analyze the influencing factors of belongingness of medical staff with the structural equation model. Results The direct effect coefficient of stress on the belongingness of the medical staff was -0.59, the indirect effect coefficient was -0.137, and the total effect coefficient was -0.73. The direct effect coefficient of the hospital evaluation factors on it was 0.42, the indirect effect coefficient was 0.338, and the total effect coefficient was 0.73.The direct effect coefficient of the satisfaction of medical personnel on it was 0.72. Conclusion We can figure out the influence factors of belongingness of medical staff with SEM discover and quantitatively analyze the existence of direct or indirect effect factors, and provide methods of reference for further study.

270 Research on correlation between research capacity and self-efficacy among clinical nurses

Objective To investigate the status of self-efficacy and research capacity of clinical nurses and analyze the correlation between self-efficacy and research capacity of clinical nurses. Methods Totally 783 clinical nurses were selected with convenience sampling from two tertiary hospitals in Hangzhou. The investigation instruments included Demographic Data Questionnaires, General Self-Efficacy Scale, and Nursing Staff Research Capacity of Self-Rating Scale. Results The average score of research capacity and self-efficacy among clinical nurses were (48.76±13.35) and (24.82±6.21). There was positive correlation between research capacity and self-efficacy (r=0.278,P<0.01), gender, level of education, job title and position were the influencing factors of the research capacity (R2=0.358, F=21.397, P<0.001). Conclusion The self-efficacy of clinical nurses was in higher-middle level, the research capacity of clinical nurses was in lower-middle level, and a positive correlation was found between research capacity and self-efficacy among clinical nurses.
