1 Implementation of the BOX-COX transformation in linear regression with application condition violations

Objective When the application conditions of linear regression are violated, trying to implement the BOX-COX transformation can improve the application effect. This paper introduces the transformation process, which provides a reference for researchers to applying linear regression correctly. Methods The application steps and key points of the BOX-COX transformation were presented. SAS program was utilized to implement data transformation and regression diagnosis in the practical example. Results Before transformation, P value of the normality test was 0.016 and DW statistic was 2.547. Moreover, the distribution of residual plots was not uniform. After proper BOX-COX transformation, P value of the normality test was 0.669 and DW statistic was 2.193. Also, the distribution of residual plots became uniform. These indicated the transformed data basically satisfied the application conditions. Conclusion When the application conditions of linear regression are violated, BOX-COX transformation, while selecting appropriate parameters, can systematically, conveniently and effectively improve the distribution characteristics of residuals, so as to guarantee the correct application of regression analysis.

4 Integrating medical resources vertically and improving medical cooperation

The key points of China′s medical reform are promoting the development of medical cooperations, establishing multi-level treatment pattern and pushing forward with the reform of public hospitals. Integrating medical resources and establishing medical cooperations can effectively alleviate the shortage of medical resources in big hospitals and improve the situation of disordered medical care seeking behaviors. Many ways have been used within some areas around the country, and the reform of realizing the first treatment in community, multi-level treatments and two-way referral step by step have been the lightspots of the medical reform in Shanghai. This paper emphatically analyses the achievements and problems of Shanghai′s medical cooperations, and offers a reference for medical cooperations in China to breaking the predicament and playing a better role.

8 A study on the trend of hand-foot-mouth disease in a district of Dongguan based on SARIMA model

Objective To investigate the posibility of using SARIMA model to predict the incidence of hand foot mouth disease in Dongguan, and to provide evidence for the prevention and control of hand foot mouth disease. Methods SARIMA model was used to model the incidence of foot mouth disease in June 2014 in January 2010, and the predictive effect of the model was verified by using 12 months in July 2014. Results In Dongguan City, a district of hand disease outbreak peak is 2nd quarter of each year, the first quarter is a low incidence. SARIMA (0,1,1) (0,1,0) 12 is well fitted to the time series of the past period, and the prediction of July Dec in 2014 and the actual incidence trend is consistent, the average relative error is 13.09, the average relative error is 9.79%. Conclusion SARIMA model can better fit the trend of the time variation of foot mouth disease in Dongguan City, and can be used to predict the future incidence.

12 Analysis on death cases in a tertiary level hospital in Shandong from 2010 to 2014

Objective To explore the main cause and distribution pattern of death cases in a tertiary level hospital from 2010 to 2014. Methods The basic information of death cases in hospital from 2010 to 2014 were collected from the hospital information system. The causes of death were classified by ICD-10. Excel and SPSS 17.0 statistics software were used to analyze the data. Results The mortality of hospital patients showed a downward trend in recent five years. The mortality of man was higher than woman significantly. Tumor and circulation system disease were in the front rank of death causes sequence. Lung cancer was the first major tumor threatening the health of inpatients. Conclusion We should strengthen the prevention, diagnosis and treatment of the disease to reduce the incidence and mortality of disease and in turn to improve the health level and quality of life.

15 Analysis on deaths and diseases burden of injury in Jinan from 2011 to 2014

Objective To study the mortality and disease burden of injury in Jinan from 2011 to 2014, and provide reference for injury control. Methods Use mortality, standardized mortality, YPLL, average YPLL, YPLL rate to analyze injury death data. Results From 2011 to 2014, the average mortality is 42.49 per 100000 population, the standardized mortality is 34.64 per 100000 population. Traffic fatalities accounted for the most in injury deaths, accounting for 40.52%. Increase with age, injury mortality rises quickly. The YPLL is 63 772 person-years for each year, the average YPLL is 28.76 years, and the YPLL rate is 10.94 years. Traffic accident YPLL account for the largest proportion (46.05%) of injury YPLL. The average YPLL of violence (41.77 years) and the YPLL rate of traffic accident (5.04‰) are both the highest one among other injuries. The death from 15 to 44 years old produce the most disease burden, the YPLL of these deaths accounting for 55.72% of the total. Conclusion Injury has become a major health problem for the residents, the disease burden of injury cannot be ignored. Injury risk factors should be controlled, and reducing the burden of disease.

19 Comprehensive performance evaluation with RSR statistics method based on disease diagnosis related group

Objective To investigate the comprehensive performance evaluation of the hospital with RSR statistics method based the diagnosis related group (DRGs). Methods The home page information of patients from a hospital in Beijing in the first half of 2015 was analyzed with the use of the Beijing edition of the diagnosis related group (BJ-DRGs), and then the DRGs related information of patients were obtained. The RSR statistics method was applied to the analysis of comprehensive performance evaluation on the medical services of the departments. Results The hospital treated disease categories (MDC) was 23 groups, DRGs group was 479, and case mix index (CMI) was 0.96, which was close to the average level of medical technology difficulty in Beijing; cost index and time consumption index were 1.06 and 1.07, which were higher than the average level of the service efficiency. The infectious disease department treated the most diseases, with 71 DRGs groups and the ICU department was with the most difficult treatment, with the case mix index 3.17. The time and pay consumption indexes in nephropathy department were the lowest, while those in obstetrics and gynecology department were 1.28 and 1.46, being the highest. The results of RSR showed the departments with best medical service performance were the department of Infectious diseases, department of Internal Nervous Medicine, department of urology, Chinese and western medicine department and ICU, while the departments with worst medical service performance were obstetrics and gynecology department and pediatrics department. Conclusion The RSR evaluation method based on the DRGs colligated the width, difficulty and efficiency index of the medical service, the evaluation result was objective, and it provided a feasible method for the hospital medical performance evaluation.

22 Influence analysis on pregnancy outcome of twin pregnancy affected by different delivery modes

ObjectiveTo explore the influence of different delivery modes affecting twin pregnancy. Methods We retrospectively collected clinical data of 154 cases of twin pregnancy, and according to different delivery modes, these cases were divided into vaginal delivery group (58 cases) and uterine-incision delivery group (96 cases). These indexes such as gestational weeks, newborn weight, postpartum hemorrhage, Apgar score and complication incidence rates of perinatal fetus were compared between the two groups. Results Gestational weeks, newborn weight and Apgar score of the second fetus in the uterine-incision delivery group were much larger or higher than those of the vaginal delivery group (P<0.05). The complication incidence rates of neonatal asphyxia and perinatal mortality in the uterine-incision delivery group were much lower than those of the vaginal delivery group (P<0.05). Conclusion Twin pregnancy women being carried out uterine-incision delivery, could obviously decrease the rate of neonatal asphyxia and rate of perinatal mortality, but it also could lead to premature delivery, hypoplasia and neonatal asphyxia, so we should properly control the rate of uterine-incision delivery. In the case of the choice of delivery modes of twin pregnancy, we should comprehensively consider cesarean section indication, gestational weeks, body weight of fetus, fetal position and pregnancy complications.

24 Analgesic effect of compound lidocaine cream in lymphocyte immunotherapy

Objective To investigate the analgesic effect of compound lidocaine cream in lymphocyte immunotherapy for women with recurrent spontaneous abortion (RSA). Methods A non-randomized controlled crossover design was adopted including 110 patients with RSA who were receiving lymphocyte immunotherapy. There were two groups in this study. 30 patients, who were in group 1, used compound lidocaine cream in the first two cycles (test group). And they used placebo cream in the next two cycles (control group). The group 2 contained 80 patients. They used placebo cream in the first two cycles (control group) and compound lidocaine cream in the next two cycles (test group). After 1 hour, intradermal injection was conducted. We used numerical rating scale (NRS), Faces pain scale (Wong-Banker score) and behavioral assessment scale to assess the degree of pain. Results For test group, the mean of NRS score, Wong-Banker score and behavioral assessment scale were 5.22±0.14, 5.23±0.14 and 3.98±0.13, which were lower than the corresponding scores in the control group. For the latter, the mean of NRS score, Wong-Banker score and behavioral assessment scale were 5.78±0.14, 5.85±0.15 and 4.78±0.15. For the three assessment methods, the differences of them were statistically significant (P<0.001). However, the effects of compound lidocaine cream or placebo cream in different treatment cycles showed no significant difference (P>0.05). Conclusion For women with recurrent spontaneous abortion, compound lidocaine cream can relieve pain in the treatment of lymphocyte immunotherapy and it is worthy to be popularized in clinical practice.

27 Application of RSR method indicators in the evaluation of the running status of medical index in clinical departments

Objective To apply RSR method to analyze and evaluate the main medical indexes of clinical departments of a tertiary referral hospital in 2013 and 2014, and provide scientific basis for the management decisions of the hospital. Methods The basic data of the clinical departments of a tertiary referral hospital in 2013 and 2014 were collected to make up the comprehensive evaluation indexes. Excel was used to calculate the RSR score and the cumulative probability. SPSS 17.0 software was used to obtain regression equation and conduct sub-file rank test. Results The comprehensive evaluation of each clinical department could be divided into three grades, and there was statistical significance by SNK analysis (2013, F=60.757, P<0.001; 2014, F=67.034, P<0.001). The probit scores of departments such as department of obstetrics and gynecology, pediatrics and cardiovascular department of internal medicine were relatively high (7.326 3, 6.774 4, 6.425 5) in 2013, and the probit scores of departments such as department of obstetrics and gynecology, pediatrics and cardiovascular department of internal medicine were relatively high (7.747 8, 6.786 6, 6.446 6) in 2014, which led to their higher work quality of medical treatment. The probit scores of departments such as infectious disease department and department of thoracic surgery (3.225 6, 3.574 5) in 2013 were relatively low, and the probit score of departments such as infectious disease department and department of thoracic surgery (3.213 4, 3.553 4) in 2014, which led to their lower work quality of medical treatment. Conclusion Using RSR comprehensive evaluation method and statistical software in hospital management can analyze the operation status of clinical departments accurately and quickly, and provide scientific basis for the management of hospital management.

31 Complications related factors in patients with esophageal foreign bodies

Objective A retrospective study was conducted to investigate the related factors of complications in patients with esophageal foreign bodies (EFBs), and to lower the incidence of complications and improve treatment efficacy. Methods From May 1997 to September 2014, 414 cases with EFBs at our hospital were enrolled in the study group. Using the patients with complications as the subjects and patients with no complications as the controls, a case-control study was designed to compare the differences between their characteristics and treatments. Applying multivariate conditional logistic analysis, we probed the risk factors of complications. Results Of all the 414 cases, 60 (15.0%) individuals appeared esophageal mucosa injury and 22 (5.3%) individuals appeared esophageal perforation. Multivariate conditional logistic analysis indicated that type of EFBs and duration of impaction were associated with complications in patients with EFBs (P<0.01). Compared to animal bone, date pit was more likely to cause complications (OR=3.016,95%CI=1.578-5.763). It also showed that the interval time from impaction to seeking medical care was the related factor of complications. Patients who saw a doctor after more than 48 hours were more easily to arise complications (OR=2.186,95%CI=1.134-4.213). For subjects, the ratio of surgical treatment was higher than controls (P<0.01). Compared to controls, subjects improved the feeding (P<0.05) and shortened the time of length of stays and postoperative recovery by about two days, respectively. Conclusion Patients with EFBs should seek hospital treatment as soon as possible. Clinical doctors should consider various data, especially type of EFBs and interval time. Accurate treatment and selective surgery may prevent serious complications and shorten the time of postoperative recovery and length of stays.

35 Investigation and analysis of carrier rate for Neisseria meningitides among healthy population from 2013 to 2014 in Gaomi city

Objective To understand the epidemiological characteristics of carrier rate for Neisseria _eningitides among healthy population from 2013 to 2015 in Gaomi city, and provide a scientific basis for the prevention and control of meningococcal meningitis. Methods Using the stratified cluster sampling, total 996 throat swabs were collected from seven age groups in healthy population in the early, medium and later stage of epidemic cerebrospinal meningitis in the city. The samples were inoculated on bi-antibiotic chocolate agar plate, and Neisseria eningitides strains were separated and cultured. Staining and observating with a microscopy, biochemical identification, and serological agglutination typing were performed. Results Eight positive pharyngeal swabs were detected for Neisseria meningitides with a carrier rate 0. 80%, and the strains were identified to belong to group A, B, C and W135, with four swabs containing group A strain, accounting for 50%. The carrier rate for Neisseria eningitides was high in children aged 1 to 3 years old and aged 7 to 15 years old, accounting for 3. 42% and 2. 07% respectively. The differences in carrier rate for all age groups were statistically significant (P=0.002). The carrier rate in health population in Juchenghe Town and Mishui Street was high, accounted for 1. 81% and 1.22%, respectively, and the differences were not statistically significant in towns and streets. Conclusion The carrier rate of epidemic cerebrospinal meningitis is 0. 80% among healthy population in Gaomi, and the rate is high in children aged 1 to 3 years old and 7 to 15 years old. The major epidemic strain is group A.
