Observation of rabbit corneal stromal lenticule transplantation assisted by femtosecond laser

Objective
To evaluate the safety, refraction changes and tissue repair of stromal lenticule transplantation assisted by femtosecond laser.
Methods
Six New Zealand white rabbits underwent SMILE in left eyes after anesthesia, and underwent femtosecond laser-assisted autologous corneal stromal lenticule surgery in right eyes. We observed the ocular inflammation,anterior segment OCT, refractive status, Pentacam corneal topography and corneal confocal laser microscope 1 day, 1 week, 1 month, 2 months, and 3 months after the operation. After 3 months of observation, the rabbits were sacrificed, the eyeballs were removed, the rabbit corneas were separated, stained with hematoxylin and eosin, and observed under an optical microscope.
Results
During the postoperative follow-up, the lenticule remained transparent and no rejection was seen. The central corneal thickness before lenticule implantation was (357.17±12.97) μm, it was (433.67±12.19) μm, (423.50±13.49) μm, (417.33±12.08) μm 1 week, 1 month, and 3 months after the operation. The spherical equivalent was (0.57±1.13)D before the surgery, and it was (-1.98±1.18)D, (-2.65±0.47)D, (-1.84±0.68)D 1 week, 1 month, and 3 months after the surgery. The anterior surface Km was (47.80±1.02) D before the surgery, and it was (53.85±0.99) D, (52.67±1.32)D, (52.53±0.96)D 1 week, 1 month, and 3 months after the surgery. The posterior surface Km was (-6.18±0.15)D before the surgery, and it was (-5.78±0.19)D and (- 5.73±0.20)D, (-5.50±0.14)D 1 week, 1 month, and 3 months after the surgery. After the overall comparison of the preoperative and postoperative central corneal thickness, spherical equivalent, anterior surface Km and posterior surface Km, the differences were statistically significant (
P<
0.001). The indexes of 1 week, 1 month, and 3 months after the surgery were significantly different from those before the surgery (
P
<0.05), but no significant changes of each index were seen among different time points after the surgery (
P
>0.05). After 3 months of follow-up, the lenticule interface can be observed in the anterior segment OCT examination, but the lenticule boundary gradually becomes blurred with time. At early postoperatively, high reflective particles can be seen at the lenticule interface by corneal confocal laser microscopy. Nerve fibers can be seen to grow into the lenticule at 1 month after the surgery. The morphology of corneal cells was close to normal at 2 months after the surgery, and the morphology and number of corneal cells were basically normal at 3 months after the surgery, with small branches of nerve fibers. HE staining showed that the corneal structure was intact, the transplanted lenticule was in place, and the front and back interface of the lenticule could be observed. The fibers are arranged neatly, distributed in parallel, and the structure is clear.
Conclusion
Autologous corneal stromal lenticule implantation is safe, predictable, and has potential clinical value. Corneal remodeling and changes in refraction after corneal implantation need to be observed for a longer period of time with more samples.

Osteogenesis-related microRNA expression pattern analysis during osteogenic differentiation of murine adipose-derived mesenchymal stem cells under tensile stress

Objective
To investigate the osteogenesis-related microRNA (miRNA) expression pattern during osteogenic differentiation of murine adipose-derived mesenchymal stem cells (ADSCs) under tensile stress.
Methods
ADSCs of the third generation of mice were divided into two groups, the experiment and the control group. The experiment group cells were subjected to a cyclic uniaxial tensile strain generated by a four-point bending apparatus, while the control group cells were without tension treatment. QRT-PCR (quantitative real-time PCR) was used to detect the expression of OCN (Osteocalcin), ALP (alkaline phosphatase), Col1 (type I collagen) and Runx2 (runt-related transcription factor 2) mRNA at different time points. Microarray was used to screen osteogenesis-related miRNA,and the relative expression intensity of miRNAs was detected during the application of tension.
Results
The mRNA levels of osteogenic genes in the experimental group were relatively high. Five up-regulated and 6 down-regulated miRNAs were screened out. On the third day of tensile stress stimulation, all miRNAs showed different changes, and most miRNAs showed a continuous up-regulation or down-regulation trend in the process of augmentation, with the most significant difference on the fifth day.
Conclusion
Tensile stress can induce the osteogenic differentiation of the ADSCs, which may be attributed to the osteogenesis-related miRNA.

Correlation between lymphocyte/monocyte ratio and no-reflow and short-term prognosis in patients with acute ST-segment elevation myocardial infarction

Objective
To assess the correlation between lymphocyte/monocyte ratio (LMR) and no-reflow (NRP) and in-hospital major adverse cardiovascular events after primary coronary intervention (PPCI) in patients with ST-segment elevation myocardial infarction.
Methods
One hundred and thirty-nine patients with acute ST-segment elevation myocardial infarction were included in this study. The patients were divided into the NRP group (
n
=25) and the control group (
n
=114) according to thrombolytic therapy for myocardial infarction (TIMI) blood flow grade. No reflow was defined as TIMI ≤ 2 and the control group as TIMI 3. The risk factors of NRP were analyzed and the application value of LMR was evaluated in the prediction of NRP patients and in-hospital major adverse cardiovascular events (MACE).
Results
Compared with those in the control group, the lymphocyte count and LMR value in the NRP group were lower; the incidence of coronary residual thrombosis signs, the use rate of teicoplanin, and in-hospital MACE after balloon dilatation were higher in the NRP group, and the differences were statistically significant (
P
<0.05); the results of multivariate logistic regression analysis showed that the lymphocyte count rise could increase the risk of NRP after PPCI in STEMI patients, and the LMR rise could reduce the risk of NRP. The patients were divided into two groups according to the LMR value of 2.815, and the incidence of MACE was higher in the low LMR group.
Conclusion
The lymphocyte count rise was independent risk factors for NRP, and the LMR rise was the protective factor for NRP. LMR also has some predictive value for MACE during hospitalization.

Analysis of risk factors related to the progression of brain stem infarction

Objective
To explore the risk factors for the progression of brainstem infarction.
Methods
Totally 173 patients with confirmed brainstem infarction were selected and divided into two groups: the progressive group and the unprogressive group. The risk factors of the two groups were compared and multivariate logistic regression analysis was performed.
Results
Compared with those in the non-progressive group, the proportion of smoking history, diabetes history, fever, unstable plaque, severe vascular stenosis, medulla oblongata infarction and LDL level in the progressive group significantly increased, while the proportion of midbrain infarction significantly decreased (all
P
<0.05). Multivariate logistic regression analysis showed that smoking history, diabetes history, fever, unstable plaque, severe vascular stenosis and medulla oblongata infarction were independent risk factors for the progression of brainstem infarction, while LDL and midbrain infarction were not associated with the progression of brainstem infarction.
Conclusion
Smoking history, diabetes history, fever, unstable plaque, severe vascular stenosis and medulla oblongata infarction are independent risk factors for the progression of brainstem infarction, which can significantly predict the progression of brainstem infarction. Timely identification of the above risk factors and effective intervention may prevent the aggravation of patients with brainstem infarction.

Different expression of lncRNA in peripheral blood of renal cell carcinoma

Objective
To investigate the expression of long noncoding RNA (lncRNA) and mRNA in peripheral blood of renal cell cancer (RCC).
Methods
Five RCC patients diagnosed by pathology were selected as the case group, and the other five healthy persons who were routinely examined in the outpatient clinic were selected as the control group. All the cases were extracted the peripheral blood and the expression of lncRNA and mRNA were detected by gene chip technology. Two upregulated lncRNA ENST00000547549, ENST00000591689 and one down-regulated lncRNA NR_038263 were selected randomly for verification analysis by real-time fluorescence quantitative PCR.
Results
Compared with those in the control group, 3 664 lncRNA were differentially expressed in the peripheral blood, of which 1 511 lncRNA were up-regulated, 2 153 lncRNA were down-regulated (fold change≥2,
P
<0.05). And 2 268 mRNA were differentially expressed, of which 932 mRNA were up-regulated, and 1 336 mRNA were down-regulated (fold change≥2,
P
<0.05). The verification analysis of the three lncRNA ENST00000547549, ENST00000591689 and NR_038263 by real-time quantitative PCR, and the results were consistent with the results of the chip detection.
Conclusion
The lncRNA gene chip is a high-efficiency and rapid detection method for detecting differentially expressed genes in peripheral blood of RCC. The differentially expressed lncRNA and mRNAs in peripheral blood may be potential biomolecules for the diagnosis and follow-up of RCC, and may become therapy targets.

Analysis of pregnancy outcome after multifetal pregnancy reduction under the guidance of transvaginal ultrasound

Objective
To analyze the pregnancy outcome of patients undergoing multiple pregnancy reduction.
Methods
Retrospective analysis was carried out on clinical data of 63 pregnant women with multiple pregnancies through transvaginal fetal reduction. The patients were divided into four groups: four fetuses reduced to twins group, three fetuses reduced to twins group, three fetuses reduced to singles group, and twins reduced to singles group. The pregnancy outcome of patients in each group after pregnancy reduction was analyzed.
Results
Of the 63 patients undergoing fetal reduction, 50 had a successful delivery, 3 had early miscarriage (4.67%), 9 had late miscarriage (14.29%), 1 had induced labor due to fetal cleft lip, and 37 had full-term labor (58.73%), 13 cases of premature delivery (20.63%), and the rate of low birth weight infants was 43.53%. There was no statistically significant difference between the two groups reduced to twins and the two groups reduced to singles in early abortion, premature delivery, neonatal weight, and low-birth-weight infants. Compared with the multiple fetuses reduced to singles, the multiple fetuses reduced to twins groups had lower birth weight, and the difference was statistically significant. There was no statistical difference between the groups in the early abortion rate, late miscarriage rate, premature birth rate, and low birth weight rate.
Conclusion
Pregnancy outcomes after multiple pregnancy reduction are mostly related to the final number of pregnancy, but not related to the number of initial pregnancy sacs. The multiple fetuses reduced to twins groups have higher complications than reduction to singles, so it is recommended that multiple pregnancy reduction be better to single pregnancy

Changes and clinical significance of four plasma coagulation levels in patients with breast cancer

Objective
To investigate the relationship between the changes of plasma prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (FIB) and breast benign tumor, pathological type and distant lymph node metastasis in patients with breast cancer, so as to evaluate the condition and prognosis of patients with breast cancer.
Methods
One hundred patients with breast cancer were selected as the experimental group and 100 patients with benign breast tumor treated in the same period as the control group. The levels of four items of plasma coagulation were detected and compared between the two groups before the treatment, as well as the changes of different pathological types and distant metastasis in patients with breast cancer.
Results
The level of plasma TT in the experimental group was significantly higher than that in the control group, with P<0.05, but there was no significant difference in PT, APTT and FIB. In addition, there was no significant difference in four coagulation levels among breast cancer patients with different pathological types and with or without distant lymph node metastasis.
Conclusion
The level of TT in patients with breast cancer is significantly higher than that in patients with benign breast tumor. Monitoring the level of plasma TT in patients with breast cancer is helpful to better evaluate the condition and prognosis of patients with breast cancer.

Effect of irrigation by different liquids at the same temperature on nasal mucosal healing

Objective
To observe the healing of nasal mucosa after endoscopic sinus surgerywhen irrigated by different liquids at 35-38℃.
Methods
One hundred and forty patients with chronic sinusitis who underwent nasal endoscopic surgery in our hospital were randomly divided into the experimental group and the control group.B oth groups were treated with Dianbang nasal irrigator for nasal irrigation. In the experimental group, the irrigation liquid was 35-38℃ Dianbang nasal irrigation liquid and normal saline. The irrigation liquid of the control group was warm water at 35-38℃.The postoperative cleaning of nasal cavity, healing speed of nasal mucosa and patient comfort were compared between the two groups. The degree of recovery was estimated by the Lund-Kennedy scale and the sino-nasal outcome test-20 (SNOT-20).
Results
The recovery of patients of the experimental group is significantly better than that of the control group, and patients of this group feel more comfortable.
Conclusion
Nasal irrigation by Dian Bang nasal irrigation fluid at 35-38℃ can help accelerate the recovery of nasal mucosa and make patients feel comfortable.

Effects of succinic metoprolol on blood pressure and left ventricular myocardial remodeling in young and middle-aged patients with hypertension

Objective
To investigate the effects of succinic metoprolol sustained release tablets on blood pressure and myocardial remodeling in the treatment of young and middle-aged patients with hypertension.
Methods
The young and middle-aged hypertensive patients with mild to moderate first diagnosed with resting heart rate of >80 times /min were randomly divided into the succinate metoprolol sustained release tablet treatment group (
n
=81) and the losartan control group (
n
=79) for 6 months of treatment. Heart rate (HR), diastolic blood pressure (DBP) and systolic blood pressure (SBP) were observed before and after the treatment in both groups. Before and after the treatment, ultrasonic measurement was conducted on left atrial diameter (LA), interventricular septum thickness (IVST), left ventricular posterior wall thickness (LVPWT), left ventricular systolic immaturity Diameter (LVDs), left ventricular diastolic diameter (LVDd), left ventricular diastolic idle volume(LVEDV), left ventricular ejection fraction (EF), left ventricular myocardial mass index (LVMI), mitral peak early diastolic flow velocity (PFVE), and the largest room velocity (PFVA). Left ventricular mass index (LVMI) and E/A ratio were calculated and analyzed statistically.
Results
IVST, LVPWT and LVMI in both groups were reduced compared with those before the treatment. The comparison before and after the treatment was statistically significant (
P
<0.05), suggesting improvement of left ventricular hypertrophy. There was no change in EF before and after the treatment (
P
>0.05), and left ventricular systolic function was not affected. After the treatment, PFVE and E/A increased, PFVA decreased in both groups, there were significant differences before and after the treatment (
P
<0.05), and left ventricular diastolic function improved.
Conclusion
Succinic metoprolol sustained release tablets can effectively reduce blood pressure and heart rate, reverse left ventricular myocardial remodeling and improve left ventricular diastolic function in young and middle-aged patients with hypertension.

Application and effect of autogenous ipsilateral peroneus longus tendon in the reconstruction of anterior cruciate ligament in youth

Objective
To explore the possibility and early effect of autogenous ipsilateral peroneus longus tendon in the reconstruction of anterior cruciate ligament in youth.
Methods
From May 2013 to May 2018, 40 cases of ACL reconstruction with autogenous peroneus longus tendon under arthroscopy were collected, including 35 males and 5 females, aged 18-35 years old, with the average age 24.8 years old. The anterior cross was reconstructed with autogenous peroneus longus tendon. The femoral end was fixed with adjustable tight rope. The tibial end was fixed with interface screw. The postoperative evaluation included clinical examination, Lysholm knee function score and AOFAS ankle function score.
Results
Forty patients were followed up for 12-36 months, with an average follow-up of 25 months. Lysholm knee function score of the last follow-up (92.65 ± 0.44) was higher than that before the operation (50.65 ± 0.68), (

<0.05).
Conclusion
The application of autogenous ipsilateral peroneus longus muscle in the reconstruction of anterior cruciate ligament in young patients is satisfactory in the early stage, and there is no significant effect on ankle function after the removal of the peroneus longus tendon.

Effect of bioactive glass 45S5 on odontogenic differentiation of apical papilla cells in vitro

Objective
To determine the effect of bioactive glass 45S5 on odontoblast differentiation of human apical dental papilla cells in vitro.
Methods
The human apical papilla cells were co-cultured in the extracted culture medium with 0.1mg/mL 45S5. The inductively coupled plasma analysis was used to test the concentrations of silicon, calcium and phosphorus in the 45S5 extract. Cell proliferation was detected by thiazolyl blue colorimetry. Realtime-PCR was used to detect the expression of DSPP and DMP-1 genes related to odontogenic differentiation of apical papilla cells. With alizarin red mineralized nodule staining, acetylpyridine chloride calcium nodule was used to semi-quantitatively analyze the mineralized nodule formation of apical papilla cells.
Results
Compared with that in the control group, the concentration of silicon ion in 45S5 extraction medium increased significantly in the control group (
P<
0.05). The proliferation of apical dental papilla cells induced by 45S5 extract on the 3rd, 5th and 7th day was significantly higher than that in the control group (
P
<0.05). The expression of
DSPP
and
DMP-1
in apical papilla cells induced by 45S5 extract for 7 days was higher than that in control group. Calcium acetylpyridine chloride nodules were semi-quantitatively analyzed for the formation of mineralized nodule of apical papilla cells in each group. OD values of silicon and OM groups were higher than those in the control group (

<0.05), the OD value of silicon group was higher than that of OM group (

0.05).
Conclusion
Bioactive glass 45S5 in concentration of 0.1 mg/mL promotes the proliferation of apical papilla cells, the high expression of genes related to odontogenic differentiation and the formation of mineralized nodules in vitro. 45S5 promotes dentin differentiation of apical papilla cells in vitro.

Preparation and bioactivity of polycaprolactone/nano hydroxyapatite electrospun fiber membrane

Objective
To study the physical-chemical characteristics, biological properties and osteogenic activity of the polycaprolactone/nano-hydroxyapatite composite materials prepared by electrospinning technology.
Methods
The electrospinning technology was used to fabricate the polycaprolactone/nano-hydroxyapatite composite material, and the physical and chemical characterization were measured. And then MC3T3-E1 cells were used as the model to evaluate the biocompatibility and osteogenic activity of the composite material in order to provide experimental basis for its application in guided bone regeneration.
Results
The polycaprolactone/nano-hydroxyapatite composite material with a disordered non-woven three-dimensional staggered network structure can show low tensile strength and good wettability. MC3T3-E1 cells with pseudopodia can grow well on the polycaprolactone/nano hydroxyapatite membrane, which could promote the expression of alkaline phosphatase and Runx2 gene.
Conclusion
The polycaprolactone/nano hydroxyapatite electrospun membrane could show good physical-chemical characteristics, biological properties and osteogenic properties, which was a potential material for bone regeneration.

Analysis on satisfaction of hospital service demand of the elderly pregnant women in the perinatal period

Objective
To investigate and analyze the satisfaction of hospital service demand of elderly pregnant women in the perinatal period under the two child policy, and to provide the basis for the hospital to improve the service quality.
Methods
Convenient sampling method was used to conduct a questionnaire survey among 450 elderly pregnant women in three tertiary hospitals in Shandong Province.
Results
The overall service satisfaction rate of elderly pregnant women to the hospital service was 72.4%. The highest satisfaction rate was the medical ethics, and the lowest was the price perception; the key improvement parts were price perception, the complaint management, the medical environment and logistics services, admission, and the auxiliary inspection; the admission, the reception, the ward round, the treatment, the nursing, the auxiliary inspection, the service attitude, the complaint management, the medical treatment environment and logistic services, and operation anesthesia were the key factors influencing the hospital service satisfaction of the elderly pregnant women.
Conclusion
Hospital service should start from key medical improvement parts to provide better service for the elderly pregnant women.

It is of great significance for the cultivation of innovative and high-quality postgraduates that how to fully mobilize the enthusiasm of postgraduates′ autonomous learning and guide them to carry out active learning practice. Analysis on the status quo and influencing factors of social anxiety of contemporary medical students

Objective
To investigate the status of social anxiety among contemporary medical students, analyze the influencing factors, and provide evidence for the formulation of relevant intervention measures.
Methods
Totally 1388 undergraduates from a medical university were randomly selected as the research subjects and social anxiety scale (LSAS) was used to evaluate the subjects. The influential factors of social anxiety were analyzed by t test, analysis of variance and multiple linear regression.
Results
The social anxiety scores of 1388 medical students were 40.46±25.44 points. The results of univariate analysis showed that there were differences in the scores of social anxiety among the medical students in gender, age, grade, whether they had been a class leader, whether they had participated in community activities and the frequency of using mobile phones. Multiple linear regression results showed that gender, age, service as a class cadre, participation in community activities, daily mobile phone usage time were the influence factors of medical students′ social anxiety, and the results specifically showed that women above men, the older the age, the higher the anxiety score, without service as a class cadre above service as a class cadre, without participation in community activities above participation in community activities, the longer the daily average time of cell phone use, the higher the anxiety score.
Conclusion
Medical students have heavy academic pressure and relatively large employment pressure, which have an impact on their mental health, and the overall level of social anxiety is high. Especially, it is necessary to guide female students and senior students to take active and effective coping ways to face social challenges, cultivate communication ability and reduce social anxiety.

Research progress of MAPKs signaling pathway in cancer

Nowadays, cancer is a serious threat to human life and health, and the incidence of cancer increases significantly year by year. Therefore, exploring the relevant signal pathways of cancer occurrence and development plays a key role in the treatment of cancer. Mitogen-activated protein kinases (MAPKs) signaling pathway exists widely in most cells in vivo, mainly transducing extracellular signals to the nucleus, and plays an important regulatory role in the proliferation, migration, invasion, apoptosis and autophagy of tumor cells. This pathway mainly includes four classical parallel pathways: JNK/SAPK, ERK1/2, ERK5 and p38 /MAPK signaling pathways, and different pathways play different roles on tumor cells after activation. In this paper, the effects of different MAPK-related signal pathways on tumors will be described.

Research progress of magnetic resonance imaging in the diagnosis of Alzheimer′s disease

Alzheimer′s disease (AD) is an insidious and progressive neurodegenerative disease that is the most common cause of dementia. MRI mainly includes structural MRI, diffusion tensor imaging, magnetic resonance spectroscopy and Arterial spin labeling magnetic resonance imaging. The combination of structural magnetic resonance imaging and functional imaging can more effectively analyze the macroscopic and microscopic changes of the brain and find out the relationship between structural changes and functional changes, which is crucial for understanding the neuropathological mechanism and clinical diagnosis of AD, so as to further provide theoretical basis for clinical treatment.
