161 Relationship between the expression of PRKCI and pancreatic cancer

Objective To analyze the expression of PRKCI in pancreatic cancer, to clarify its role in the prognosis of pancreatic cancer, and to analyze the relationship between PRKCI and immune cell infiltration in tumor microenvironment.Methods Oncomine, GEPIA, HPA and CCLE databases were used to analyze the expression of PRKCI in pancreatic cancer tissues and cell lines; GEPIA was used to analyze the relationship between PRKCI expression and the prognosis of pancreatic cancer patients; String database was used to analyze the protein interacting with PRKCI; TIMER was used to detect the correlation between PRKCI expression and tumor purity and immune cell infiltration.Results The expression of PRKCI in pancreatic cancer tissues was significantly higher than that in normal pancreatic tissues. PRKCI was also highly expressed in pancreatic cancer cell lines. The overall survival and disease-free survival of pancreatic cancer patients with high PRKCI expression were significantly lower than those with low PRKCI expression (P<0.01) and the expression of PRKCI was significantly correlated with the stage of pancreatic cancer. Proteins interacting with PRKCI include CDC42, NUMB, SQSTM1 and so on, which are mainly involved in inflammatory reaction and protein phosphorylation. The expression of PRKCI was negatively correlated with CD4+ T cells, PD-L1 and CTLA-4 in pancreatic cancer.Conclusion PRKCI is highly expressed in pancreatic cancer. The expression of PRKCI is significantly correlated with the prognosis and immune cells. PRKCI may act as a potential therapeutic target for pancreatic cancer.

167 Predictive value of transient elastography combined with NLR and PLR for the complications of hepatitis B liver cirrhosis and its relationship with the severity of the disease

Objective To explore the application value of transient elastography combined with neutrophil-lymphocyte ratio (NLR) and platelet-lymphocyte ratio (PLR) in predicting the complications of hepatitis B cirrhosis, and to study the relationship between the above indicators and the severity of hepatitis B liver cirrhosis.Methods Retrospective analysis was conducted on 76 patients with hepatitis B liver cirrhosis who were diagnosed in the Binzhou Medical University Hospital from January 2018 to September 2020, and it is divided into CPT A, CPT B, CPT C according to the Child-Pugh (CPT) score; at the same time, it is divided into ≤9 group and ≥10 group according to MELD score; according to the presence or absence of complications, 76 patients with hepatitis B liver cirrhosis were divided into the complication group and the non-complication group; and according to the number of complications, the complication group was divided into the single complication group and the multiple complication group. The changes of NLR, PLR and liver hardness value (LSM) among the groups were compared and analyzed, and the correlation between each index and CPT score and MELD score was further analyzed, and the strength and significance of the correlation were evaluated. Finally, the ROC curve was used to analyze whether each index and the combined application had predictive value for the complications of hepatitis B liver cirrhosis.Results NLR and LSM were positively correlated with CPT score and MELD score, and PLR was negatively correlated with CPT score and MELD score. The NLR and LSM of the complicationgroup were higher than those of the non-complication group, and the PLR was lower than that of the non-complication group. The NLR and LSM of the single complication group were lower than those of the multiple complication group, and the PLR was higher than that of the multiple complication group. The predictive value of NLR, PLR, LSM, NLR combined with PLR, NLR combined with LSM, PLR combined with LSM were expressed by AUC as 0.715, 0.691, 0.777, 0.770, 0.811, 0.830, and 0.839, respectively, where the best cut-off values for NLR, PLR, and LSM were 2.101, 76.92, and 19.3 respectively.Conclusion 1. NLR, PLR, LSM are all related to the severity of hepatitis B liver cirrhosis, and can be used to assess the severity of liver cirrhosis. 2. NLR and LSM are positively correlated and PLR is negatively correlated with the severity of liver cirrhosis. 3. NLR, PLR, LSM, the combination of two and the combination of the three have certain predictive value for the complications of liver cirrhosis, and the combination of NLR, PLR and LSM has the best predictive value for the complications of liver cirrhosis.

173 Effects of endometrial receptivity indexes on pregnancy outcomes of embryo transfer during freeze-thaw cycles in patients of different ages

Objective To study the effects of endometrial thickness, patterns and blood flow types on the clinical outcome of frozen embryo transplantation in patients of different ages.Methods The clinical data of patients who received frozen embryo transplantation were retrospectively analyzed and 853 cycles were included. The patients were divided into different age groups. The endometrium thickness, patterns and blood flow types were evaluated by vaginal color Doppler ultrasound. The influence of endometrium thickness, patterns and blood flow types on pregnancy outcome in patients of different ages was studied. The predictive value of endometrial thickness, patterns and blood flow types on clinical pregnancy outcome was analyzed by using ROC curve.Results With the increase of age, the embryo implantation rate and pregnancy rate of patients showed a downward trend. The pregnancy rate is the highest when the endometrial thickness is 8-14mm, followed by <8 mm, and the pregnancy rate ≥14 mm is the lowest, but the trends are different for patients of different ages. There was no pregnancy in the type C group of patients of all ages, and the pregnancy rate and implantation rate of the endometrium in the type A group were not different from those in the type B group. With the increase in the abundance of endometrial blood flow, the pregnancy rate of patients gradually increased. The implantation rate of type II group was the highest, followed by type III group, and the implantation rate of type I group was the lowest. However, there was no statistical difference in pregnancy rate and implantation rate among the three groups of data in each age group. The ROC curve suggests that the endometrial blood flow types can be used as indicators to predict clinical pregnancy outcome.Conclusion The clinical pregnancy rate and implantation rate of embryo transfer during freeze-thaw cycles decrease with age. Endometrial thickness, patterns and blood flow types can affect the implantation rate and clinical pregnancy rate of embryo transfer. Endometrial blood flow types on the day of endometrial transformation can predict the clinical pregnancy outcome of embryo transfer patients during the freeze-thaw cycle.

178 Analysis on the status quo and influencing factors of medication compliance in patients with depression

Objective To explore the influencing factors of medication compliance of patients with depression and improve their medication compliance.Methods With the method of random investigation, 200 patients with depression were selected as the objects of investigation who were treated in the Psychological Outpatient Department of Bin Medical University Hospital from April 2020 to September 2020, and general data questionnaire and the self-made medication compliance questionnaire were used for investigation and evaluation. Chi-square test and Logistic regression analysis were used for data analysis to explore the influencing factors.Results Among the collected 195 patients, the score of medication compliance was (1.91±1.37), and 59 patients had good medication compliance (total score was 0), accounting for only 30.26%. In terms of the influencing factors of medication compliance, occupation (medicine/non-medicine), monthly income, duration of medication, understanding of disease, satisfaction with doctors, and the presence or absence of adverse drug reactions in the past had significant differences (P<0.05). Logistic regression analysis showed that occupation, duration of medication, monthly income, understanding of the disease and satisfaction with doctors were all influencing factors of medication compliance.Conclusion The medication compliance of patients with depression is not optimistic. Occupation, medication duration, monthly income, understanding of the disease and satisfaction with doctors will affect medication compliance.

183 A meta-analysis of the clinical efficacy of arthroscopic surgery and open surgery for the treatment of popliteal cyst

Objective To investigate the clinical efficacy of arthroscopic surgery and open surgery in the treatment of popliteal cyst by meta-analysis.Methods PubMed, EBSCO, the Cochrane Library, CNKI and Wanfang databases were searched for randomized controlled trial (RCT) and semi randomized controlled trials (qRCT) of arthroscopic and open surgery for popliteal cyst of the knee joint from January 2000 to September 2017. According to the inclusion and exclusion criteria, literature screening and quality evaluation were conducted. Then, Newcastle Ottawa scale (NOS) document quality evaluation scale was used to evaluate the quality of the literature, and rev man 5.3 software was used for meta analysis.Results Eight articles were included, including 499 patients, 259 patients in arthroscopic surgery group and 240 patients in open surgery group. The meta-analysis showed that compared with the open surgery group, the arthroscopic group had a certain advantage over the open surgery group in the postoperative recurrence rate, postoperative infection rate, postoperative improvement rate and incision length between the two groups; however, there was no significant difference between the two groups in the operation time and the postoperative Lysholm score.Conclusion Compared with open surgery, arthroscopic surgery for popliteal cyst of the knee joint has the advantages of less surgical trauma, less postoperative complications, low recurrence rate and rapid recovery, and it also has the advantages of dealing with concomitant intra-articular lesions.

189 Treatment of lumbar spondylolisthesis by oblique lateral lumbar interbody fusion combined with lateral nail plate internal fixation

Objective To explore the clinical curative effect of inclined lateral lumbar intervertebral fusion (oblique lateral interbody fusion, OLIF) joint lateral plate fixation for the treatment of patients with lumbar olisthe disease.Methods A retrospective study was carried out on 37 patientswith lumbar olisthe disease in I or II degrees in our hospital who received OLIF joint lateral nail plate internal fixation treatment.The subsidence of the fusion cage in the CT lesion was observed at the last follow-up.Operative time, intraoperative blood loss and postoperative complications were recorded.Clinical efficacy was assessed by visual analogue Scale (VAS) and Oswestry Disability Index (ODI).Results Compared with preoperation, the VAS and ODI scores of 37 patients decreased at 1, 3 months after operation and the last follow-up, P<0.01; Compared with 1 month after surgery, VAS and ODI scores were decreased at 3 months after surgery and at the last follow-up, P<0.05; Compared with 3 months after operation, the VAS and ODI scores in the last follow-up were decreased, P<0.05.Conclusion OLIF combined with lateral nail plate internal fixation can effectively open the vertebral space, enhance the stability of affected vertebral segment and prevent the fusion cage from sinking.The operation is safe and reliable with few postoperative complications.

192 Expression of IL-17 and IL-23 and significance in rectal cancer patients before and after radiotherapy

Objective To investigate the level of IL-17 and IL-23 in peripheral blood of patients with rectal cancer to analyze their association with clinicopathological characteristics and prognosis before and after radiotherapy.Methods Blood samples were collected from 24 patients with rectal cancer before and after radiotherapy, and the levels of IL-17 and IL-23 in peripheral blood of the patients were detected by ELISA. All patients received 50Gy radiotherapy after surgery.Results Protein expression level of IL-17 was correlated with tumor differentiation and lymph node metastasis in rectal cancer patients (P<0.05), whereas serum IL-23 level showed correlated only with lymph node metastasis (P<0.05). Significant differences were observed between serum IL-17 and IL-23 levels in rectal cancer patients before and after radiotherapy (P<0.05). Serum levels of IL-17 and IL-23 can be used to effectively diagnose tumor differentiation and lymph node metastasis of rectal cancer patients. Multivariate Cox proportional hazards regression analysis showed that the expression of IL-23 may be independent risk factor for poor prognosis (P<0.05).Conclusion The concentration of IL-17 and IL-23 obviously increased in peripheral blood of patients with rectal cancer, both of them were correlated with radiotherapy. Detection of IL-17 and IL-23 levels has certain clinical significance in the diagnosis and prognosis of rectal cancer patients.

196 Value of CHA2DS2-VASc score in the differential diagnosis of cardioembolic transient ischemic attack

Objective To explore CHA2DS2-VASc score and ABCD2 score values in differential diagnosis of cardiogenic transient ischemic attack (TIA).Methods Clinical data and ABCD2, CHA2DS2-VASC score results of 60 patients with clinically confirmed cardiogenic TIA (group A) and non-cardiogenic TIA (group B) were reviewed and compared.Results There were no significant differences in baseline characteristics between the two groups, such as gender, age, associated onset and duration, clinical symptoms, and previous history of stroke, but there were significant differences in CHA2DS2-VASc scores (t=2.70, P<0.05), and the area under the ROC curve of the CHA2DS2-VASC score for the diagnosis of cardiogenic TIA was 0.682 (95%CI: 0.526-0.837, P<0.05).Conclusion CHA2DS2-VASC score can be used as a reference index in the actual management of clinical TIA to help identify the possibility of cardiogenic TIA, so as to guide early intervention in hospitals.

201 Diagnostic value of shear wave elastography for BI-RADS 4 Breast lesions

Objective To evaluate the value of shear wave elastography in diagnosing BI-RADS 4 breast lesions.Methods One hundred and sixty-four breast nodules of BI-RADS 4 breast lesions were examined by conventional ultrasonic and shear wave elastography, and their elastic modulus Emax and ERatio values were determined, and their diagnostic threshold was calculated.Results The elastic modulus value of breast lesions were statistically significant, including Emax and Eratio. As the ROC curve displayed, The diagnosing boundary value of Emax and Eratio was 66.9kpa and 3.88, respectively. The sensitivity of Emax and Eratio were 87.8% and 92.9% , and the specificity of Emax and Eratio were 87.7% and 96.0%.Conclusion Shear wave elastography plays more important role in the differential diagnosis of BI-RADS 4 breast lesions.

204 Treatment of diabetic macular edema by Buqilishui prescription combined with conbercept

Objective To observe the treatment of diabetic macular edema by Buqilishui prescription combined with conbercept.Methods Fifty-two cases (64 eyes) of diabetic macular edema patients were randomly divided into the combined treatment group and the control group. The combined treatment group was treated with Buqilishui prescription combined with intravitreal injection of conbercept, while the control group was treated with only intravitreal injection of conbercept. The BCVA and CMT were observed at 1 month, 3 months and 6 months after the initial treatment, and the times of conbercept injections were countedand compared.Results There were no significant differences in BCVA and CMT before the treatment between the two groups (P>0.05). BCVA and CMT in 2 groups were significantly improved after the treatment compared with those before the treatment, and the difference was statistically significant (P<0.05). But there were no significant differences in BCVA and CMT after the treatment at the same time points (P>0.05). There were significant differences in average times of conbercept injections between the two groups (P<0.05).Conclusion Conbercept in the treatment of DME can effectively improve the visual acuity of patients with DME, and when it is combined with Buqilishui prescription, can reduce times of intravitreal injection of conbercept.

207 Correlation analysis of sleep disorder of diabetic patients with Yin deficiency and Huowang and blood glucose fluctuation, 25 hydroxyvitamin D

Objective To analyze retrospectively the clinical data of diabetes mellitus with Yin deficiency and fire hyperactivity in order to understand the correlation of sleep disorders in diabetic patients.Methods One hundred and fifty cases of diabetes mellitus with sleep disorder and 150 cases without sleep disorder were collected as the control group. The general clinical data, blood glucose fluctuation and incidence of hypoglycemia were compared between the two groups.Results SBP, DBP, fasting blood glucose, in the sleep disorder group were significantly higher than those in the normal sleep group, with statistical significance, P<0.05; the HbA1c, SDBG, PPGE and LAGE in the two groups were compared, with statistical significance, P<0.05. The results of logistic regression showed that HbA1c, PPGE, SBP, FPG and hypoglycemia were positively correlated with sleep disorder in Yin deficiency Huowang type diabetes mellitus, P<0.05, and 25(OH)D was positively correlated with sleep disorder in Yin deficiency Huowang type diabetes mellitus, P<0.05.Conclusion As the blood glucose and blood pressure of patients with Yin deficiency and Huowang type diabetes can not be effectively and actively controlled, the blood glucose fluctuates greatly, patients are prone to hypoglycemia, and the incidence of sleep disorders increases gradually.

211 Optimization of extraction process for volatile oil from compound Corydalismicroemulsiongel cataplasmaby central composite design-response surface methodology

Objective To optimize the extracting condition of volatile oil by central composite design-response surface methodology.Methods Analytical method was established, and volatile oil was extracted by the method of steam distillation. Yield of water, soaking time and extracting time were selected as study factors. The extraction rate of volatile oil, the content of ligustilide and cinnamaldehyde was as the optimized index to abtain the optimal extraction for verifying tests.Results The optimal extracting process was as follows: adding10 times of water, soaking for 100 minutes and extraction time was 8 hours. The expected extraction rate of volatile oil was 0.413%, the content of ligustilide was 39.20%, and cinnamaldehyde was 23.30%. The error between the actual value and the predicted value was less than 0.05.Conclusion The central composite design-response surface methodology has a high accuracy in the extraction of volatile oil, and the parameters are reasonable and reliable.

218 Prediction of anti-inflammatory targets of withanolides from Physalis angulata L

Objective To predict the anti-inflammatory effect of withanolide constituents from Physalis angulata L. and their target proteins with molecular docking technology and to evaluate the potential anti-inflammatory activities of these compounds in vitro by lipopolysaccharide (LPS)-induced RAW 264.7 macrophage cells.Methods A small molecule database based on 9 withanolide constituents isolated from Physalis angulata L. was established, and molecular docking technology in computer-aided drug design (CADD) was used to dock the small molecules with target proteins (COX-2, iNOS, ERK, p38, JNK, NF-κB) to predict the target and the anti-inflammatory mechanism of withanolide constituents. Furthermore, the inhibitory activities on the overproduction of inflammatory mediator NO were measured by Griess method.Results The main active sites of 9 withanolide compounds acting on each target were compared and analyzed, compounds 1, 2, 3, 4 inhibited the overproduction of NO induced by LPS.Conclusion With anolides from Physalis angulata L. may exert anti-inflammatory founction through blocking the ERK transduction pathway.

223 Multiple logistic regression analysis on the level of physical and mental health of parents of children with cerebral palsy and on influencing factors

Objective To analyze the physical and mental health of parents of children with cerebral palsy by the status survey to study the quality of life and its influencing factors.Methods Totally 104 cases including children with cerebral palsy and healthy babies were as the research object, and they were divided into the experiment group and the control group. The questionnaire survey were conducted, All the parents were issued with SF-36 scale, and the experimental group still needed to fill in the family general situation questionnaire. The results of the two groups were analyzed to get the influencing factors affecting the quality of life of parents of children with cerebral palsy.Results Compared with the parents of normal children, the quality of life of the parents of children with cerebral palsy was significantly lower. The scores of the 8 dimensions of the parents of children with cerebral palsy all decreased to different degrees. The scores of each dimension of the parents of children with cerebral palsy were correlated with the main factors to different degrees. The influencing factors were included in the regression equation and the results showed that the older the children were, the more serious the disease was, and the older the parents were, the lower the quality of life of their parents.Conclusion The quality of life of the parents of children with cerebral palsy has different degrees of decline, and it is affected by a variety of factors, while the most important factors are the age of the children, the severity of the children′s condition, and the age of the parents.

226 Design and application of module teaching based on PBL in oral clinical medicine teaching

Objective To discuss the design and application of common symptoms module in clinical stomatology teaching based on PBL modular teaching method, so as to provide a new teaching model for the cultivation of applied and innovative medical talents.Methods Based on the main symptoms of oral clinic, several modules were designed, and the disease system and related cases of the modules were established. Sixty stomatological students including 30 undergraduates and 30 postgraduates in clinical practice were randomly selected as the research objects. They were randomly divided into the experimental group and the control group with 15 undergraduates and 15 postgraduates in each group. The experimental group learned two modules of pain and swelling, while the control group used LBL teaching method system to explain the diseases involved in the two modules. At the end of the study, theoretical examination and questionnaire survey were conducted, and the results of the two groups were statistically compared.Results According to the experimental design, this study constructed the common symptom module and corresponding disease system in oral clinical practice. The Application results showed that the theoretical test scores and questionnaire scores of the experimental group were higher than those of the control group (P<0.05).Conclusion The modular teaching mode based on PBL, which combines the advantages of PBL and MES training modes, is a competency-based education model suitable for stomatological science, and its comprehensive effect is better than that of traditional teaching methods.

Objective To explore the teaching methods suitable for basic nursing practice course by applying mixed teaching to basic nursing practice course, so as to improve the teaching quality and improve the teaching effect.Methods Two hundred and forty-eight nursing students of grade 2018 in a university were selected by cluster sampling and divided into the experimental group (128) and the control group (120). The experimental group adopted mixed teaching method and the control group adopted traditional practice teaching method. The results of in-class skill operation, skill operation at the end of the semester and average scores of the two groups were compared to evaluate the effect of mixed teaching.Results In the experimental group, the scores of in-class skill operation (86.94±4.98) and the skill operation at the end of the semester (87.51±4.29), and the average score (87.23±3.23) were higher than those in the control group (79.68±6.53), (78.93±6.69) and (79.31±5.79), and the difference was statistically significant (P<0.05).Conclusion The application of mixed teaching method in basic nursing training course is conducive to students′ mastering of the difficulties in operation technology and stimulating students′ interest in autonomous learning. It is a preferred teaching method for students at present.
