81 Effects of zika virus NS1 on immune response in mice

Objective To use Zika virus nonstructural protein 1 (NS1) expressed in prokaryotic system, to observe the effect of NS1 on immune response of mice, so as to explore the potential of NS1 as Zika virus vaccine.Methods The recombinant NS1 protein was obtained by expression pET28a-ZIKV-NS1 plasmid in prokaryotic expression system, then the protein was be used to immunize the mice. The immunoglobulin G (IgG) in mouse serum was determined by means of enzyme-linked immunosorbent assay. And the flow cytometry and enzyme-linked immune assay were applied to detecting related factors including the mice spleen's CD4+ cells, CD8+ cells numbers and cytokine interferon gamma (IFN-γ), interleukin 4 (IL-4) secretion levels.Results We constructed the prokaryotic expression plasmid pET28a- ZIKV-NS1 successfully and got the ZIKV-NS1 protein. The serum IgG titer of mice increased when NS1 was immunized 2 and 3 times, and increased with the increase of inoculation times, and when compared with the blank control group, P<0.001; the number of CD4+ and CD8+ cells as well as the level of IFN-γ and IL-4 secretion in the spleen of mice increased, and when compared with the blank control group, P<0.001.Conclusion ZIKV-NS1 can activate the humoral and cellular immunity of mice, which may have the potential to become a Zika virus vaccine.

88 Feasibility of transfection of rat adipose-derived stem cells with recombinant adenovirus Ad-DDAH2

Objective To explore the feasibility of transfection of rat adipose-derived stem cells with adenovirus vector (Ad-DDAH2).Methods Ad-DDAH2-GFP plasmid was constructed. Adipose-derived stem cells of the original generation in rats were cultured. The expression of immunophenotype CD29, CD45 and CD90 was detected by flow cytometry. Adipose stem cells were transfected with Ad-DDAH2-GFP with different titers. The transfection efficiency and cell morphology were counted by fluorescence microscope. The expression of DDAH2 in adipose-derived stem cells was detected by RT-PCR and Western blot.Results Ad-DDAH2-GFP recombinant adenovirus vector was successfully constructed and packaged, and the virus titer reached 2E+10PFU/ML. The growth curve of the 4th generation adipose-derived stem cells is S-shaped. The results of flow cytometry showed that the positive rates of CD29, CD90 and CD45 were (95.83 0.53)%, (91.32 0.27)% and (1.59 0.06)%, respectively, which accorded with the immunophenotype characteristics of adipose-derived stem cells. When the virus infection complex values are 80 and 100, the transfection efficiency can reach more than 90%. However, when the virus infection complex value is 100, the adipose-derived stem cells have obvious cytopathological phenomena, so the virus multiplicity of infection 80 is regarded as the best multiplicity of infection, and positive amplification bands can be seen by RT-PCR and Western blot.Conclusion Ad-DDAH2-GFP recombinant adenovirus vector was successfully constructed, which can efficiently transfect rat adipose-derived stem cells. The long-term stable expression of DDAH2 at mRNA and protein levels has laid the experimental foundation for subsequent vivo experiments.

92 Effects of alga polysaccharide on inflammatory response and oxidative stress in rats with endometriosis

Objective To investigate the therapeutic effect of alga polysaccharide on endometriosis (EMS) and its effects on serum levels of follicle stimulating hormone (FSH), luteinizing hormone (LH), malondialdehyde (MDA) and interleukin-6 (IL-6).Methods The rats were divided into five groups: the control group, the model group and three alga polysaccharide groups with different doses. Four weeks later, the rats were treated with alga polysaccharide at different doses. The pathological changes and serum levels of FSH, LH, MDA and IL-6 in rats with endometriosis were observed after 2 weeks of administration.Results The therapeutic effect of alga polysaccharide on endometriosis in rats was dose-dependent. The degree of atrophy and decrease of the glands and the serum levels of FSH, LH, MDA and IL-6 significantly decreased in alga polysaccharide groups compared with the model group.Conclusion Alga polysaccharide achieves the therapeutic effect of EMS in rats, and the underlying mechanisms may be through significantly inhibiting the proliferation of ectopic endometrium, accelerating the regression of inflammation, and reducing the levels of FSH, LH, MDA and IL-6 in serum.

96 Relationship between VEGF and severity of acute epidural hematoma

Objective To detect the expression of vascular endothelial growth factor (VEGF) in the blood of patients with acute epidural hematoma (EDH) and explore its relationship with severity.Methods Three hundred and forty-two adult patients admitted to the hospital with EDH whose VEGF values were detected were selected for research. Patients were divided into low,normal and high groups according to the VEGF values detected at admission. New injury severity score (NISS), Glasgow Coma Scale (GCS), hematoma volume, and midline shift were recorded at admission. Early neurological decline (END), surgical procedure, death toll, intensive care unit (ICU) and total hospital stay were recorded at discharge. Pearson correlation analysis was used to analyze the relationship between VEGF levels and various indicators among different VEGF groups. The relationship between VEGF levels and mortality was calculated by analysis of variance. Unconditional logistic regression analysis was used to determine the risk factors associated with mortality.Results Totally 342 patients with EDH were studied, and the mean VEGF at admission was (74.585±37.686) pg/mL. One hundred and forty-seven patients (43.0%) underwent surgery. Sixteen patients (4.7%) died before discharge. Pearson method showed no correlation between VEGF and each index. Analysis of variance showed statistically significant correlation between VEGF reduction and mortality (P<0.05). Logistic regression analysis showed that NISS increase and GCS decrease at admission and VEGF reduction were risk factors associated with mortality.Conclusion In the early phase of admission, the serum VEGF values of more patients with EDH increased, and a few cases decreased. NISS increase, GCS decrease and VEGF reduction during admission were risk factors for mortality.

100 Correlation between electrolyte changes and postoperative death in 320 patients with cerebral contusion

Objective To investigate the electrolyte changes in patients with cerebral contusion and analyze the risk factors of postoperative death.Methods Three hundred and twenty adult patients with cerebral contusion who required emergency craniotomy were enrolled in our hospital from November 2017 to October 2019. They were divided into three groups according to GCS scale at admission (mild group, medium group and severe group). The types of electrolyte disorders at the time of admission and within 24 hours after operation were analyzed, and the mortality was recorded. Multivariate logistic regression was used to analyze the risk factors of electrolyte disorders and mortality.Results Patients with cerebral contusion were more common in men; hypokalemia was common before operation (65.63%, 210/320), with an average of (3.27±0.91) mmol/L. Acidosis was common after operation (67.81%, 217/320), with an average of (20.57±2.82) mmol/L. Logistic regression analysis showed that hypernatremia and acidosis were risk factors for increased mortality within 24 hours after operation. The risk degree was 20.241 (27.550-6.497) and 4.012 (2.921-1.023) respectively.Conclusion The most common types of electrolyte disorders before and after operation are hypokalemia and acidosis respectively in patients with cerebral contusion. Hypernatremia and acidosis significantly increase the mortality within 24 hours after operation during perioperative period.

104 Clinical characteristics and prognostic analysis in acute myeloid leukemia with isocitrate dehydrogenase 1 gene mutation

Objective To assess the frequencies and clinical characteristics and prognostic significance of the isocitrate dehydrogenase 1(IDH1)mutations in acute myeloid leukemia (AML).Methods Two hundred and twenty patients with AML newly diagnosed in our hospital from January 2015 to September 2018 were selected. PCR was used to amplify the exon 4 of IDH1 gene, gene sequencing was used to detect the mutation of IDHL gene, and the clinical characteristics and prognostic significance of patients with IDH1 mutation were analyzed.Results IDH1 gene mutation was detected in 9 of 220 AML patients, and the mutation rate was 4.09%, all of which were R132H mutation. The median age was 50 years in the mutation group and 43 years old in the non-mutation group, and there was a difference between the two groups, P<0.05. Blood platelets median level was 68×109/L in the mutated group and 43×109/L in the non-mutation group, and there was a difference between the two groups, P<0.05. IDH1 gene mutations all occurred in AML-M4/M5, and they were not observed in other AML subtypes. The rate of IDH1mutations was 6.48% in AML with normal karyotype, higher than 1.43%in AML with abnormal karyotype, without signifi cant differences. IDH1 gene mutations were associated with NPM1mutations, P<0.05, but not associated with FLT3-ITD muta -tions. Patients with IDH1 mutations were prone to CD34-、CD33+、CD64+ (averge P<0.05). IDH1 mutated patients hada lower complete remission rate than unmutated patients (50% vs 80.92%), P<0.05. In all follow-up patients, the 2-year over all survival rate in IDH1 mutation group was lower than in the non-mutation group (25% vs 67.32%), P<0.05. In the patients with normal karyotype, the 2 years overall survival rate in the IDH1 mutation group was lower than in the non-mutation group (28.57% vs 65.91%), P<0.05. In the patients without mutated NPM1, the same result was obtain (0% vs 67.20%), P<0.05. In the patients with mutated NPM1, the 2 years overall survival rate was 40% in the IDH1 mutation group and 67.86% in the non-mutation group, without statistical differences.Conclusion IDH1 gene mutations are more common in AML patients with older age, higher platelets level. IDH1 gene mutations are associated with normal karyotype, AML-M5/M4 and NPMl gene mutations. IDH1 gene mutation was correlated with NPM1 gene mutation. IDH1 gene mutation has an adverse effect on the prognosis of AML patients and is a molecular marker of poor prognosis. In the patients with mutated NPM1, IDH1 gene mutations have no effect on prognosis.

108 Expression and clinical significance of LncRNAGASL1 in patients with sepsis

Objective To analyze the diagnostic value of blood GASL1 in patients with sepsis by collecting and detecting the difference of GASL1 expression between patients with sepsis and patients in the control group.Methods Two hundred and fourteen patients were enrolled, 134 patients with sepsis and 80 patients without sepsis, and 120 healthy people were enrolled. The blood samples of patients were collected within 24 hours after admission. RT-PCR was used to detect the expression level of GASL1.Results The relative expression levels of LncRNAGASL1 were 5.33±4.08, 2.01±1.75 and 1.72±1.40 in sepsis patients, non sepsis patients and healthy trontrols, respectively. The relative expression level of GASL1 in the blood of sepsis patients was significantly higher than that of non sepsis patients (P<0.05) and healthy controls (P<0.05). Compared with the healthy controls, the area under curve (AUC), sensitivity and specificity of blood GASL1 in the diagnosis of sepsis were 0.866, 0.885 and 0.735, respectively. Compared with non sepsis patients, the AUC, sensitivity and specificity of blood GASL1 were 0.861, 0.695 and 0.900. The relative expression level of blood GASL1 in sepsis patients with Apache>15 and sofa>7 was significantly higher than that in sepsis patients with Apache≤15 and sofa ≤ 7, average P<0.05; the relative expression level of GASL1 in sepsis patients was significantly lower than that in severe sepsis and septic shock patients, P<0.05.Conclusion The expression level of GASL1 is increased in patients with sepsis, and it has a better diagnostic value for sepsis.

112 Correlation between cystatin C and cerebral infarction and carotid atherosclerotic plaque

Objective To investigate the correlation between serum cystatin C (CysC) levels and cerebral infarction and carotid atherosclerosis.Methods Three hundred and nineteen patients with cerebral infarction in our hospital were selected as the experimental group and 323 healthy physical examinees as the control group, and Carotid ultrasound was performed on all patients. According to the results of carotid ultrasound, the patients were divided into the plaque group and the non-plaque group. The clinical data of the two groups were compared, and the risk factors of cerebral infarction and plaque formation were analyzed by logistic regression.Results Age, CYS-C level and plaque detection rate in the experimental group were significantly higher than those in the control group, P<0.05. Age, CYS-C and carotid artery plaque were independent risk factors for cerebral infarction, P<0.05. The average age, number of smokers and cYS-C level in the plaque group were higher than those in the non-plaque group, P<0.05. Smoking and CYS-C levels were independent risk factors for carotid plaque formation, P<0.05.Conclusion The level of cystatin C is closely related to the formation of cerebral infarction and carotid artery plaque. Smoking cessation and control of cystatin C level can prevent the formation of carotid artery plaque, and it is of clinical significance to further prevent the occurrence of cerebral infarction.

116 Distribution and drug resistance of pathogenic bacteria in urinary tract infection in a hospital from 2014 to 2019

Objective To analyze the distribution and drug resistance of pathogenic bacteria generated from urine culture in a Hospital from 2014 to 2019, so as to provide basis for the correct clinical application of antimicrobial agents.Methods The pathogenic bacteria found in urine culture from 2014 to 2019 were collected, and their distribution and drug sensitivity were analyzed.Results Totally 2 363 strains of pathogenic bacteria were isolated, including 1 480 (62.6%) strains of gram-negative bacteria, 502 (21.2%) strains of gram-positive bacteria, and fungi 381 (16.1%); the detection rate of Escherichia coli was the highest, accounting for 37.7% of the total number of pathogens detected. The drug resistance rate of most cephalosporins and quinolones was above 40%, with no significant change in the last six years. The drug resistance rate of enzyme inhibitors and carbapenems has not increased significantly in the past six years and is still at a low level, below 6%. The ESBL strains in Klebsiella pneumoniae and Proteus singular were 49% and 50% respectively, and the drug resistance rates of Escherichia coli, Klebsiella pneumoniae and Proteus singular to imipenem were 0.8%, 1.4% and 2.4% respectively. The drug resistance rates of Pseudomonas aeruginosa to imipenem and amtriaxone were 11% and 14% respectively. The drug resistance rate of Enterococcus faecalis was higher than that of enterococcus faecalis, and all of them were more sensitive to linezolid, tekoranin and vancomycin. The drug resistance rates of Staphylococcus epidermidis to benzoxicillin and penicillin were 60% and 86.7% respectively. The sensitivity to vancomycin was 100%. The highest resistance rate of fungi to itraconazole was 17.6%, and no resistance to 5-fluorocytosine was found.Conclusion The results of this study showed that gram-negative bacteria were dominant in urine culture, mainly Escherichia coli, and cefoperazine, piperacillin/tazobactam and cefoperazone/sulbactam could be used as initial empirical drugs in clinical practice.

121 Clinical study of triple therapy for adult early necrosis of femoral head

Objective To investigate the clinical effect of the triple therapy of extracorporeal shock wave combined with hyperbaric oxygen and blood circulation drugs in the treatment of adult early necrosis of femoral head, so as to provide valuable experience for clinical work.Methods Thirty patients with femoral head necrosis in the department of joint surgery were chosen. ARCO stage was Ⅰ period of 17 cases, Ⅱ period 13 cases, and patients received triple therapy treatment of extracorporeal shock wave combined with hyperbaric oxygen and blood circulation drugs. The patients were followed up for 3 months, and before the treatment and after three periods of treatment, the calculation was performed on VAS scores, Harris scores, limb medial femoral circumflex artery blood flow resistance index (RI), and the percentage of femoral head necrosis volume in overall femoral head volume.Results VAS scores and Harris scores were lower after the same stage of treatment than those before the treatment, P<0.01. Compared with those of stage I, the VAS scores of stage II decreased and Harris scores of stage II increased before and after treatment, P<0.05. The blood flow signal of the medial circumflex femoral artery of the femoral head could be detected in 20 of the 30 patients, includingⅠperiod 13 cases and Ⅱperiod 7 cases. Compared with that before the treatment, RI of medial rotated femoral artery decreased at the same stage, P<0.05. The percentage of femoral head necrosis in femoral head volume was lower after the same stage of treatment than that before the treatment, P<0.05. Compared with that of stage I, the percentage of femoral osteonecrosis volume in stage II increased before and after the treatment, P<0.01.Conclusion Triple therapy for the treatment of early adult femoral head necrosis has clinical curative effect, which was better in Ⅰ period than in Ⅱ period, so it has great clinical application value.

125 The mechanism of Yuanqi capsule improves sleep latency and sleep time of mice

Objective To explore the effect of Yuanqi capsule on mice sleep improvement and to explore its potential mechanism.Methods 150 Kunming mice were randomly divided into 5 groups, 30 in each group; they were normal group, model group, Yuanqi Capsule low-dose group, Yuanqi Capsule medium-dose group, and Yuanqi Capsule high-dose group. Except for the normal group, the remaining 4 groups of normal Kunming mice were prepared into insomnia model mice by the para-chlorophenylalanine (PCPA) method. After the model was built, normal group and model group were given normal saline, Yuanqi capsule low-dose group was given Yuanqi capsule 0.39 mg/g, and Yuanqi capsule middle-dose group was given Yuanqi capsule 0.78 mg/g, Yuanqi Capsule high-dose group was given Yuanqi Capsule 1.56 mg/g. After 5 days of intragastric administration, the pentobarbital sodium sleep experiment was conducted, and the levels of γ-aminobutyric acid (GABA)and protein expression in serum and brain tissues of mice were detected by ELISA and Western Blot.Results After Yuanqi capsule treatment, the sleep latency of mice in low-dose, medium-dose and high-dose groups was shortened, the sleep time was prolonged, the serum GABA content and the expression of GABA in brain tissue were increased.Conclusion Yuanqi Capsule can improve the sleep of PCPA model mice, and its mechanism of treating insomnia is related to the regulation of GABA expression.

129 Correlation between mammographic characteristics and molecular subtypes of breast cancer

Objective To verify the feasibility of predicting molecular subtypes based on mammography before surgery by analyzing the direct signs of breast cancer and the tumor characteristics and different molecular subtypes in immunohistochemistry.Methods The X-ray features and molecular typing of 139 cases of breast cancer in our hospital were analyzed statistically.Results Among all the mammographic characteristics of breast cancer, tumor was the most common type, and Luminal B was the most common molecular subtype. The direct signs of breast cancer were statistically significant with different molecular typing, P<0.01, and there was a significant correlation between the edge characteristics of breast cancer mass and whether there was calcification in the mass and molecular typing, P<0.01. Most breast cancers of the triple negative type and Luminal A type showed mass type, calcification was rare in the triple negative type, and marginal burrs were more common in the Luminal A type. Images with her-2 overexpression were not the same.Conclusion Mammography of breast cancer is helpful to differentiate its molecular subtypes, and to a certain extent can guide the clinical determination of treatment plan and prognosis judgment.

133 Comparison on the effect of cone-beam CT and orthopantomography in locating impacted teeth of anterior maxilla

Objective To investigate the role of cone-beam computer tomography(CBCT)in the diagnosis and orientation of impacted teeth in the anterior maxilla.Methods Forty-five impacted teeth in the anterior maxilla of 39 patients were selected, which were respectively examined by orthopantomography and cone-beam CT. The direction of crown growth, the position of the crown, root curvature and the development were observed and measured with the software of the system to analyze and compare the differences between the two examinations.Results CBCT was more effective than orthopantomography in diagnosing the direction of crown growth, impacted types, root curvature and the development,P<0.05.Conclusion CBCT can accurately locate the spatial position and the shape of impacted teeth.

136 Influence factor of quality of life, psychological health in lung cancer patients during chemotherapy

Objective To explore obvious influence factor quality of life, mental health in patients with lung cancer during chemotherapy, so as to provide basis for making targeted nursing plans.Methods Two hundred and eight patients with lung cancer in our hospital were selected and analysis process was mainly by using QLQ-C30 quality of life scale, as well as MCMQ and Kessler scale.Results The univariate analysis showed that QLQ-C30 showed significant differences in marital status, smoking and alcohol consumption (P<0.05), and Kessler scale showed significant differences in occupation, smoking and alcohol consumption (P<0.05). There was no statistical significance in the evaluation of MCMQ. Multivariate analysis results showed that QLQ-C30 scale showed significant differences in age, education level, marital status and whether drinking or not; Kessler scale showed that annual income, smoking, drinking or not, self-rated health were statistically significant; medical coping scale showed that drinking was statistically significant. The correlation coefficient between QLQ-C30 and Kessler scale indicated that the correlation coefficient of body function was significantly increased; the correlation coefficient between MCMQ total score and QLQ-C30 indicating health-related economic situation significantly increased.Conclusion The quality of life of patients with lung cancer is closely related with their age, education background, marital status, drinking or not. In clinical work, nurses can formulate individualized nursing programs according to the above contents, so as to further improve the quality of life of patients.

Objective To explore the application effects of PICC catheterization in the ZIM area under ultrasound guidance in two positions, and to seek the best position suitable for puncture in the ZIM area.Methods From June 2018 to November 2019, 80 patients with PICC intubation diagnosed with hematological diseases were randomly divided into the control group and the observation group according to the hospitalization number. All patients underwent the ZIM best area method as the best puncture point. In the control group, the conventional catheterization of the lateral extremity and the trunk at 90 degrees were used for puncture. When the catheter reached the shoulder, the patient's head was instructed to turn to the puncture side, and the lower jaw was close to the shoulder. The observation group used the position of the catheterized limb to lift the arm and hold the head for puncture. When the catheter reached the shoulder, the catheter was sent to the required length without turning the head. The comfort, internal jugular vein ectopic, and success rate of the catheter were compared between the two groups.Results When the PICC was placed by PICC under the guidance of ultrasound, the pain score in the control group (8.425±0.808) was significantly higher than that in the observation group (7.825±1.111), and the position discomfort score (7.288±0.996) in the control group was significantly higher. In the observation group (4.2±1.072), the difference was statistically significant (P<0.05). The incidence of internal jugular vein ectopic was 2.50% (2/80) , significantly lower than thta in the control 12.5% (10/80). The difference was statistically significant, with P<0.05. The success rate of one-time puncture and catheterization was 90% in the control group and 87.5% in the observation group, and the difference was not statistically significant.Conclusion Raising the arm and holding the head is the ideal puncture position for PICC catheterization in the ZIM area under ultrasound guidance.

Objective To explore effects of external use of dweco combined with TDP irradiation on symptom improvement in patients with craniocerebral trauma with skin abrasions.Methods Two hundred patients with craniocerebral trauma and skin abrasions admitted to the department of neurosurgery of our hospital from July 2019 to November 2019 were selected. According to the random number table method, they were divided into the control group and the observation group, with 100 cases in each group.The control group received TDP irradiation, and the observation group received TDP irradiation combined with dweco. The two groups were compared in terms of wound healing time, pain score, pressure sore, pigmentation and scar occurrence.Results The healing time of facial and trunk wounds in the observation group was shorter than that in the control group,P<0.05.There was no statistically significant difference in VAS between the two groups before the treatment.VAS score of the two groups after treatment was lower than that before the treatment, P<0.05, and VAS level score of the observation group was lower than that of the control group,P<0.05.The incidence of pressure sore, pigmentation and scar in the observation group was lower than that in the control group,P<0.05. The wound infection rate of the observation group was lower than that of the control group ,P<0.05.Conclusion Combined with TDP irradiation, dwecois helpful to shorten the wound healing time of craniocerebral trauma patients with skin abrasions, reduce the pain of patients, the wound infection rate and reduce the incidence of pressure ulcers, pigmentation and scar.

148 Advances in the application of routine cytological examination based on cerebrospinal fluid in the diagnosis of central nervous system diseases

Routine examination of cerebrospinal fluid is a common clinical examination item of cerebrospinal fluid. It is an indispensable monitoring item in the diagnosis, treatment and evolution of various diseases of the central nervous system. However, in the aspect of cell classification and counting, because routine examination is wet-film microscopy, it only relies on refractive power to observe and classify cells roughly and is easily disturbed by various factors to produce errors. The cerebrospinal fluid cytology examination is a meticulous microscopic examination of cells after cell collection and staining by various methods. It not only makes up for the shortcomings of routine examination, but also has its unique advantages in the identification of central nervous system diseases and cancer cells. With the development of technology, cerebrospinal fluid cytology is also developing. Many new technologies, such as the detection of lymphocyte subsets in cerebrospinal fluid by immunology and the accurate diagnosis of central nervous system tumors by cytochemical methods, have emerged. The future research route of CSF should start from three aspects: structural research, information research and hierarchical research.

2019-nCoV is a highly contagious respiratory RNA virus. Since there is no approved vaccine for prevention, in order to improve the diagnosis and treatment efficiency of the disease, and to avoid the spread of the virus, the virus detection is extremely critical. This article reviews the nucleic acid detection technology, immunological detection technology and laboratory and clinical methods of joint diagnosis of 2019-nCoV, with a view to providing reference for virus detection and disease prevention.

157 Efficacy of CD38 monoclonal antibody in the treatment of relapsed refractory multiple myeloma

The incorporation of proteasome inhibitors and immunomodulatory drugs into the standard of treatment has improved outcomes in patients with multiple myeloma over the years, but MM is still incurable, and all patients will eventually relapse. CD38 monoclonal antibody has shown certain clinical efficacy in the treatment of relapsing refractory multiple myeloma. This article will review the mechanism of action of CD38 monoclonal antibody, and review the progress of two kinds of CD38 monoclonal antibodies, isatuximab and daratuumab, in the treatment of RRMM.
