Comparison of different superovulation protocols in vitro fertilization and embryo transfer

Objective
To compare the clinical and laboratory results of three different controlled superovulation protocols in vitro fertilization and embryo transfer(IVF-ET), and from the actual situation of patients, provide a basis for clinicians to choose the best superovulation protocol.
Methods
Patients′ data were retrospectively analyzed who underwent IVF-ET for 1996 fresh cycles in our reproductive center from January 2016 to August 2019, and were divided into super long program group (471 cycles) and long program group (818 cycles), and antagonist program group (707 cycles) according to different hyperovulation programs. The differences were statistically analyzed in the general situation, clinical medication, timing of egg retrieval, trigger type and laboratory related data of the three groups of patients.
Results
BMI, bFSH, bLH and bE

of the antagonist program group were all higher than those of the other two groups, and compared with those of the long program group,

<0.05, while bT and bAFC in the antagonist program group were higher than those in the super long program group,

Conclusion
Among the three ovulation induction programs, each has its own indications. According to the existing clinical data, the basic situation of the patient is analyzed to select the unique advantages of each ovulation induction program, the best ovulation induction program and the trigger type, to grasp the proper timing of egg retrieval, to get the right number of eggs as much as possible, and to cultivate a good embryo in order to make the most benefits for the patients.

Constitution and antimicrobial resistance of the pathogens isolated from bloodstream specimens in 2 137 inpatients

Objective
To investigate the constitution and analysis of antimicrobial resistance of the pathogens isolated from bloodstream specimens of inpatients in a hospital.
Methods
Non-repeated specimens of blood culture positive were collected from inpatients with the diagnosis of bloodstream infection from January 2015 to December 2019. Identification and drug susceptibility analysis were adopted by mass spectrometer, API series and Vitek2 Compact. K-B tests and E-tests were used as supplement, and the results were determined according to the CLSI standard 2019. WHONET5.6 software was used for data analysis.
Results
Totally 2 137 strains of non-repeated pathogens were collected, among which 1312 gram-negative bacteria (61.4%), 776 gram-positive bacteria (36.3%), 29 fungi (1.4%) and 20 anaerobic bacteria (0.9%). The detection rates of CRE in Enterobacteriaceae was 0.3%. And 45.5% ESBLs were detected in Enterobacteriaceae. Strains resistant to vancomycin, teicoplanin and linezolid in Staphylococcus species were not found. The rates of MRSA and MRCNS were 21.7% and 78.5% respectively. One isolate of VRE was detected. The resistance rate of fungi to itraconazole was 10.3% and the resistance rate of anaerobic bacteria to penicillin, tetracycline and clindamycin were 15.0%, 50%, and 70%, respectively.
Conclusion
Gram-negative bacteria were the main bloodstream infection pathogens in our district, and the drug resistance to the common antibiotics was different and species were diverse. Blood culture should be emphasized by clinics, and do timely and exactly. Antibiotic agents should be selected reasonably according to the results of drug susceptibility tests in clinical treatment.

Effect of obstructive sleep apnea hypopnea syndrome on early functional deterioration of acute cerebral infarction

Objective
To further explore the effect of obstructive sleep apnea hypopnea syndrome (OSAHS) on early neurologic deterioration (END) in patients with acute cerebral infarction through clinical prospective study.
Methods
Seventy-six OSAHS patients with acute cerebral infarction were selected, and 76 cases of acute cerebral infarction in the same period were selected as the control group. Age, disease history, blood glucose, blood lipid and blood pressure were measured and collected. All patients were examined by head and neck CT vascular imaging to determine the stenosis and stenosis rate. By referring to the definition of END, the incidence and time of END were compared between the two groups, and Cox multivariate regression analysis was performed for the relationship between OSAHS and END.
Results
There was statistically significant difference in the overall incidence of END admission between the two groups

<0.05). The time interval between END and onset, END and admission in THE OSAHS group was smaller than that in the control group (
P
<0.05). After taking diabetes history, coronary heart disease history, hypertension history, thrombolytic therapy, body weight, systolic blood pressure, diastolic blood pressure, A1c, total cholesterol and other covariate factors into account, OSAHS was still a significant risk factor for END,

Conclusion
OSAHS disease is a significant risk factor for END.

Analysis of the effect of quadruple therapy after Pilon fracture operation

Objective
To study the effect of compression bandage, cold compress, plaster external fixation and elevation of the affected limb in the treatment of Pilon fracture.
Methods
Totally 125 cases with Pilon fracture were collected from February 2010 to February 2018, which were treated with internal fixation. The patients were randomly divided into four groups: the observation group (

=32), treated with compression bandage, cold compress, plaster external fixation and elevation of the affected limb; the control group 1 (

=31), treated with compression bandage, cold compress and elevation of the affected limb; the control group 2 (

=31), treated with plaster external fixation, cold compress and elevation of the affected limb; the control group 3 (

=31), treated with cold compress and elevation of the affected limb. The VAS score, swelling, complications and patients' satisfaction were compared.
Results
The VAS score of the observation group was better than that of the control group (

Safety and effectiveness of transcatheter closure of patent ductus arteriosus via right radial artery right femoral vein

Objective
To investigate the safety and effectiveness of transcatheter closure of patent ductus arteriosus (PDA) via right radial artery and right femoral vein.
Methods
A retrospective analysis was made on 45 patients with patent ductus arteriosus occlusion. 14 patients with PDA occlusion via right radial artery-right femoral vein and 31 patients with PDA occlusion via right femoral artery-right femoral vein were divided into two groups. The operation success rate, average operation time, contrast clarity, average bed time and the incidence of operation complications were compared between the two groups.
Results
There was no statistically significant difference between the two groups in average operation time and success rate. Comparison of surgical complications between the two groups showed that there were 0 cases in the radial artery group and 7 cases in the femoral artery group (
P
<0.05).
Conclusion
Both surgical approaches are safe and effective, but the average bed rest time and surgical complications in interventional occlusion via the right radial artery approach are significantly lower than traditional surgical approaches, and are worth further promotion.

Analysis of the effect and safety of combination of labor analgesia with percutaneous
electrical stimulation and Doula delivery on delivery outcome

Objective
To investigate the effect and safety of combination of labor analgesia with percutaneous electrical stimulation and Doula delivery on delivery outcome.
Methods
Totally 160 women who were hospitalized in our hospital were enrolled as subjects. They were randomly divided into the observation group and the control group, with 80 cases in each group. The control group was given Doula delivery, the observation group was given percutaneous electrical stimulation combination on Doula delivery for labor analgesia. The VAS scores, labor time, postpartum hemorrhage, cesarean section rate, maternal satisfaction, neonatal status, and adverse reactions were compared between the two groups.
Results
Compared with before analgesia, VAS score in the observation group decreased in each period after analgesia, with
P
<0.05. Compared with those in the control group, the 2 h postpartum blood loss and cesarean section rate in the observation group reduced, and maternal satisfaction was improved, with
P
<0.05.
Conclusion
Percutaneous electrical stimulation analgesia combination Doula delivery can alleviate maternal pain, reduce cesarean section rate, reduce postpartum hemorrhage, and improve maternal satisfaction, without obvious adverse reactions. It is worthy of clinical application.

Analysis on distribution and drug resistance of pseudomonas aeruginosa in a hospital in 2019

Objective
To explore the distribution and drug resistance of pseudomonas aeruginosa in our hospital in 2019, and to provide reference for rational use of antibiotics in clinics.
Methods
Four hundred and twenty-two Strains of pseudomonas aeruginosa were collected in our hospital in 2019, and the specimen source, department distribution and the drug resistance were analyzed.
Results
Totally 422 strains of pseudomonas aeruginosa were isolated. The primary source included sputum (71.1%), fester (7.3%), alveolar lavage fluid (5.2%); pseudomonas aeruginosa was mainly detected from ICU (19.9%), followed by department of respiratory medicine (17.1%) and department of thoracic surgery ward (8.5%). In 2019 the drug resistance rates of pseudomonas aeruginosa against amikacin was the lowest (0.2%); the pseudomonas aeruginosa had a lower resistance against polymyxin (0.3%) and tobramycin (1.7%), but it had higher resistance rate against ticacillin/clavulanic acid.
Conclusion
Bacterial drug resistance monitoring should be strengthened, and effective measures should be taken to control the spread of drug-resistant strains.

Meta-analysis of the correlation between Vitamin D and diabetes Mellitus with pulmonary tuberculosis

Objective
To use the meta analysis method to study the correlation between vitamin D and diabetes Mellitus with pulmonary tuberculosis(DM-PTB), provide evidence-based medical reference for clinical DM-PTB prevention and treatment.
Methods
All published studies on the case-control experiment of the correlation between vitamin D and DM-PTB retrieved from databases such as CNKI, VIP, CBM, Cochrane, Wanfang, PubMed, and Web of science were collected from the building date of these libraries to April 2020. Data were independently extracted by two reviewers according to the inclusion and exclusion criteria. Literature qualities were assessed by the NOS table and meta-analysis was carried out by revman 5.1.
Results
Eight studies with a total of 1151patients were included. The vitamin D levels in the DMPTB group were significantly lower than those in the PTB group (SMD=-0.73, 95%CI:-1.34~-0.13).
Conclusion
The current study shows that vitamin D has a certain correlation with DMPTB.

Corneal biomechanics of small incision lenticule extraction combined with corneal
collagen cross-linking in the treatment of myopia

Objective
To study the changes of corneal biomechanical parameters of small incision lenticule extraction (SMILE) combined with corneal collagen cross-lingking (CXL) to correct myopia.
Methods
Prospective studies were carried out on thirty-two patients (64 eyes) who underwent SMILE surgery. They were divided into two groups according to eye exam results, including 18 petients (36 eyes ) in the SMILE group and 14 patients (28 eyes) in the SMILE combined with corneal collagen crossing-linking group (SMILE+CXL).The SMILE group received routine SMILE surgery. The SMILE+CXL group first received SMILE surgery, after the separation and removal of the corneal stroma lens, they were given Vibex Xtra riboflavin solution uniformly repeated drops in the corneal capsular bag, with 45 s, Avedro accelerate cross-linking system was used for line across epithelial rapid cross- linking, the wavelength of 365 nm was chosen, with 30 mW/cm
2
ultraviolet irradiation illumination for 45 s of the cornea with the continuous illumination model, and the total energy was 1.35 J/cm
2
. The patients were followed up for 1 month and 3 months after the operation, and the basic preoperative data and postoperative ophthalmic examination parameters, corneal biomechanical parameters before and after operation, and corneal endothelial cell count were compared between the two groups.
Results
All the operations were completed successfully. All patients achieved or exceeded the best corrected visualacuity after the operation. The patients in SMILE+CXL group had more obvious irritation symptoms such as eye pain and tears than SMILE group. LogMAR 1 day, 1 month and 3 months after the operation were compared between the SMILE group and the SMILE+CXL group, and the difference was statistically significant (
P
<0.05). Comparison of spherical equivalent (SE) between the SMILE group and the SMILE+CXL group 1 day, 1 month and 3 months after surgery showed statistically significant differences (
P
<0.05). Intraocular pressure (IOP) was compared between the SMILE group and the SMILE+CXL group 1 month and 3 months after surgery, and the difference was statistically significant (

<0.05). Compared with 1 day after surgery, LogMAR of 1 month and 3 months after surgery was statistically significant between the two groups (

<0.05). Compared with 1 day after surgery, there was a statistically significant difference in SE between the two groups at 1 month and 3 months after surgery (

<0.05). Compared with 1 month after surgery, LogMAR difference was statistically significant in SMILE+CXL group 3 months after surgery,

Conclusion
The corneal biomechanics decreased after SMILE. SMILE combined with corneal collagen cross-linking was safe and effective in early myopia correction. The corneal hardness increased and the corneal deformation rate decreased after corneal collagen cross-linking.

Clinical observation on the changes of astigmatism degree of the original
astigmatism cataract after different incision operations

Objective
To compare the effect of two different incision positions on the original astigmatism of cornea.
Methods
One hundred and eighty-six cases (186 eyes) of age-related cataract with preoperative astigmatism≤2.0 D in our hospital were selected as the study objects. All the patients underwent phacoemulsification combined with IOL implantation with 3.0 mm transparent corneal incision. According to the incision position, the patients were divided into group A (88 eyes) and group B (98 eyes). The incision of clear cornea was made at 10-12 points in group A, while the incision of steep corneal axis was selected in group B according to the preoperative corneal curvature examination. According to whether the preoperative astigmatism degree≤0.75 D, the changes of postoperative corneal astigmatism degree and visual acuity recovery were compared between the two groups.
Results
In patients with astigmatism ≤0.75 D, compared with that before surgery, the astigmatism degree decreased significantly 1 week after surgery, with
P
<0.05, without significant changes in astigmatism 1 month and 3 months after the operation. In patients with astigmatism≤0.75 D, compared with group A, the astigmatism degree of group B decreased significantly at different time points after the operation,
P
<0.05. In group B, the astigmatism degree gradually decreased with the extension of postoperative time,

<0.05.
Conclusion
For the patients with preoperative astigmatism≤0.75 D, the early astigmatism of different incisions decreased, but for the patients with preoperative astigmatism>0.75 D, the postoperative astigmatism decreased, and the better uncorrected visual acuity could be obtained by incision on the steep axis of cornea.

Effects of HU-308 and nHA/PCL electrospun membrane on the biological properties of MC3T3-E1

After incubation for 1, 4, and 7 days, compared with that in the control group, the proliferation activity of cells in each experimental group was enhanced,

Effect of comprehensive nursing intervention in postoperative chemotherapy for breast cancer patients

Objective
To explore the effect of comprehensive nursing intervention on the life quality of postoperative chemotherapy for breast cancer patients.
Methods
One hundredpatientswith breast cancer in postoperative chemotherapy were randomly divided into the observation group and the control group. The control group received conventional nursing, and the observation group took comprehensive nursing intervention measures on the basis of conventional nursing.Health knowledge questionnaire and nursing job satisfaction questionnaire made by SAS and SDS were used to evaluate the effect of intervention.
Results
After the period of comprehensive nursing intervention, the emotionand the life quality in the follow-up one year after dischargein this group significantly improved (

<0.05), while the difference of each index was not statistically significant in patients receiving conventional nursing.
Conclusion
Comprehensive nursing intervention measures can effectively improve the quality of life and social adaptability of postoperative chemotherapy patients with breast cancer.

Application of the theory of enhanced recovery after surgery in postoperative nursing
after laparoscopic treatment of pediatric indirect inguinal hernia

Objective
To study the clinical effectiveness of the theory of enhanced recovery after surgery in the nursing care after laparoscopic high ligation in treatment of pediatric indirect inguinal hernia, and to evaluate the safety and feasibility of early feeding and early activity after general anesthesia.
Methods
Totally 154 children with indirect inguinal hernia admitted by pediatric surgery in a tertiary hospital were selected as the study objects. The patients were randomly divided into the observation group (82 cases) and the control group (72 cases). The children in the observation group were nursed with the concept of rapid rehabilitation surgery, while the children in the control group were nursed with the traditional perioperative nursing method. The nursing effects of the two groups of children were compared by comparing the differences in the degree of pain, hospitalization stays, hospitalization cost, postoperative adverse reactions and complications, family satisfaction and other relevant indicators.
Results
Compared with the control group, the children in the observation group had shorter hospital stays and lower hospitalization costs (

<0.05). The postoperative crying, hunger and family members′ satisfaction with nursing services of the observation group were significantly better than that of the control group (
P
<0.05). The degree of postoperative pain of the observation group was less than that of the control group (
P
<0.05).
Conclusion
The application of the theory of enhanced recovery after surgery in the laparoscopic treatment of pediatric indirect inguinal hernia has reduced the hospitalization time and cost of the children, without increasing the incidence of postoperative complications of the children, and improves family satisfaction. Therefore, it is worthy of promotion and application.

Study on the demand of adult higher education of primary medical staff

Objective
To investigate the basic situation, status of continuing medical education and demand for adult higher education of health technicians, clarify the goal of medical adult education, accelerate the reform of adult education model and improve the quality of primary medical staff.
Methods
A questionnaire survey and interview were conducted among 1233 health technicians (phase 23) from 12 counties, 106 township clinics and 50 community health service centers who participated in basic health training in 2019.
Results
At present, the basic educational background of grass-root medical staff is relatively low, and there is a demand for adult higher education, which is concentrated in the age group of 20-50 years old. However, as the country attaches great importance to grass-root level health, the number of clinical health technicians with bachelor′s degree or above is increasing and the demand for postgraduate education is also increasing.
Conclusion
With the further deepening of the new medical reform, the development scale and quality improvement requirements of grass-root health human resources are constantly improved. In adult higher education, we should make full preparation for further study of grass-root talents, and integrate humanistic education into the education process to stabilize the primary medical team.

Exploration of improving the scientific research innovation ability of clinical medicine students
with master′s degree under the mode of double-track integration mode

Objective
To explore the strategies and methods to improve the scientific research innovation ability of clinical medicine candidates with master′s degree under the double-track integration mode, in order to cultivate compound high-quality medical talents with scientific research innovation consciousness, and meet the requirements of the times.
Methods
Two grades of students with master′s degree in medicine from Binzhou Medical University in 2017 and 2018 were selected. Forty-two students were randomly divided into the control group (20) and the observation group (22). The observation group solved the contradiction between clinical and scientific research by setting up the joint training mechanism of double tutors, optimizing the course setting, building the online course platform, improving the examination and supervision mechanism, and setting up the academic and professional graduate student support group, so as to enhance the scientific research awareness and ability of graduate students majoring in clinical medicine. The number and quality of articles published before graduation was counted, and the graduation thesis of master′s degree students were reviewed and scored by senior professional title personnel of relevant specialties.
Results
The number and quality of articles published before graduation and the scores of master′s degree papers in the observation group were better than those in the control group, and the differences were statistically significant,

Conclusion
The new training method can effectively stimulate the study interest of the master′s degree candidates in scientific research, enhance the enthusiasm of study and the interaction between the tutor and the students, and improve the students′ ability of scientific research innovation.

Research progress of shexiang baoxin pill on angiogenesis of acute myocardial infarction

Acute myocardial infarction (AMI) is due to the acute and continuous decrease of blood flow in the coronary artery, which leads to the injury of myocardial hypoxia. AMI has the characteristics of rapid onset, rapid progress, high mortality and serious threat to human life and health. Shexiang baoxin pill (SBP) is a kind of Chinese patent drug composed of artificial musk, ginseng extract, artificial bezoar, cinnamon, Styrax, toad and borneol. It plays an important role in the treatment of unstable angina pectoris and myocardial infarction. Therapeutic angiogenesis is a promising concept, which can promote the angiogenesis of endogenous collateral vessels, restore myocardial blood supply, reduce infarct area and improve cardiac function after myocardial infarction. In recent years, the effect of Shexiang baoxin pill on the angiogenesis of acute myocardial infarction has been recognized and valued by people. Now, the effect of Shexiang naoxin pill on the angiogenesis of acute myocardial infarction is reviewed to provide reference for the clinical treatment of AMI.

Research progress of sirt1 regulating autophagy in myocardial ischemia-reperfusion injury

Myocardial reperfusion is one of the treatments for acute myocardial infarction, which can significantly improve the prognosis and reduce mortality, but it often increases myocardial cell damage in the early stage, and causes myocardial ischemia/reperfusion (I/R) injury. Autophagy is widespread in eukaryotic cells and plays a protective role in the growth and development of the body. In myocardial I/R, autophagy in the ischemic phase plays a protective role, and reperfusion phase plays a damaging role. In recent years, the mechanism of sirt1's involvement in autophagy has gradually attracted attention. Exploring the role of sirt1 in regulating autophagy in myocardial I/R injury will help to study the pathogenesis of myocardial I/R injury.
