161 Three-dimensional reconstruction study of the displacement of femoral neck fractures and proposal of modified Garden’s classification

Objective To discuss the disadvantage of Garden’s classification and to propose modified Garden’s classification by measuring the spatial displacement parameters of the femoral head.Methods Totally 160 cases of femoral neck fracture patients were selected, including Garden I,Ⅱ,III,and IV each 40 cases,and Garden Ⅰ included impacted femoral neck fracture 34 cases and incomplete femoral neck fracture 6 cases.The bilateral proximal femurs of each patient were scanned by CT and reconstructed by professional software.Normal femur model was registered with fracture mirror model, key points were marked at the two models,and the parameters of femoral head displacement were measured.Four groups of patients with femoral head space displacement parameters were compared.Results The femoral head displacement angle of impacted femoral neck fracture group was 18.06°±10.77°,and 30% (10/34) impacted femoral neck fracture head space displacement were more than 20°.Six cases of incomplete femoral neck fractures showed on X-ray films,but only 3 cases were incomplete femoral neck fractures by CT scanning and three-dimensional reconstruction.The femoral head displacement angle was 16.79°±9.98° in Garden type Ⅱ,and 25% (10/40) Garden typeⅡ femoral neck fracture head spatial displacement were more than 20°. In the Garden type III and Garden type IV the femoral head center displacement distance were 14.36±5.61 mm and 18.77±5.45mm,respectively.There was not statistical significance of the parameters of femoral head displacement of impacted femoral neck fracture between Garden type I and Garden type Ⅱ (P>0.05) .Femoral head displacement parameters of Garden type IV were compared with those of the other three groups,and the differences were statistically significant (P<0.05).Conclusion Impacted femoral neck fracture is defined as a non-displaced fracture,which was with a large deviation,no There was no significant difference in the head displacement parameters of impacted femoral neck fracture and GardenⅡ type, Garden’s classification has certain limitations,and modified Garden’s classification is more suitable for clinics.

165 Clinical study of LVIS stent in the treatment of intracranial aneurysms

Objective To explore the efficacy and safety of LVIS stent in the treatment of intracranial aneurysms,and to provide basis for the clinical treatment of intracranial aneurysms and the rational selection of the internal stent.Methods From January 2015 to December 2017 in our hospital, clinical data of 40 cases (43 aneurysms) with spontaneous subarachnoid hemorrhage by CTA or DSA were diagnosed as intracranial aneurysms and underwent LVIS and stent assisted coil embolization, and the clinical data were retrospectively analyzed.Results All of the 43 aneurysms were successfully embolized.All the stents were successfully released.Immediately after operation,the results of cerebral angiography showed that 27 cases (67.5%) were completely embolized,11 cases (27.5%) were near total embolism,5 cases (12.5%) were partial embolization,and the effective rate of embolization was 87.5%.Of these,1 was ruptured in the operation,and 1 was all cerebral infarction and died after the operation.Six months follow-up after the operation,mRS score were as follows:0 point 32 cases,1 point 3 cases,2 points 3 cases,3 points 1 cases, and the good rate was 95%.In the reexamination of cerebral angiography,complete embolization was 87.5%, and the recurrence rate was 10%.Conclusion LVIS stent assisted embolization of intracranial aneurysms is a safe and effective method. However, a larger sample needs to be studied and the results of long-term follow-up are required.

168 Expression and intervention study of NF-κB,TNF-α,IL-1 and IL-6 in patients with unstable angina pectoris

Objective To analyze the expression level of NF-κB,TNF-α,IL-1 and IL-6 in patients with unstable angina pectoris (UAP),and to study the clinical application value of Danhong injection combined with clopidogrel.Methods 342 patients with UAP were randomly divided into control group (n=172) and observation group (n=170).The control group received basic treatment and clopidogrel treatment,and the observation group received basic treatment and Danhong injection combined with clopidogrel treatment.The efficacy and clinical indicators of the two groups were observed, and data were processed and analyzed by statistical software. Results The total effective rate of the observation group (96.2%) was significantly higher than that of the control group (85%),and the difference between the two groups was statistically significant (P<0.05).After treatment, the level of blood lipid was obviously improved in the two groups.The level of TNF-alpha,IL-1 and IL-6 and the activity of NF-κB decreased significantly, and the decrease in the observation group was more obvious, the difference was statistically significant (P<0.05).Conclusion UAP application of Danhong injection combined with clopidogrel could significantly improve blood lipid,reduce TNF-α,IL-1 and IL-6 levels and down regulate NF-κB activity.

171 Effect of mNGF in the treatment and levels of serum MBP,NSE,and VEGF in patients with cerebral infarction

Objective To investigate the effect of mouse nerve growth factor(mNGF) in the treatment the patients with cerebral infarction and levels of serum myelin-basic protein(MBP), neuron-specific enolase(NSE) and vascular endothelial growth factor(VEGF).Methods Eighty-eight patients with cerebral infarction were randomly divided into the control group and the observation group,and 30 cases were set up as the healthy group.Patients in the Control group were given conventional foundation treatment,while patients in the observation group were given mouse nerve growth factor besides conventional treatment.Scores of NIHSS (National Institute of Health stroke scale) before the treatment and 14 days after the treatment were recorded,and concentration of MBP and NSE,VEGF were tested 3 days and 14 days after the treatment.Results fourteen days after the treatment, NIHSS scores of two groups reduced, but the change of the observation group was significantly greater (P<0.05) than that of the control group.Serum MBP,NSE and VEGF in cerebral infarction group were higher than those in the healthy group,and 14 d after the treatment the serum MBP and NSE concentration were lower than before, while the VEGF concentration was higher,but the changes in observation group were significantly greater than those in the control group (P<0.05).Conclusion Mouse nerve growth factor can effectively reduce the expression of serum MBP and NSE,increase the expression of VEGF,improve nerve function defect,and is ideal in the treatment of cerebral infarction.

174 Effects of tirofiban on myocardial perfusion and hs-CRP, TNF-,cTnT levels in patients with acute coronary syndrome after Percutaneous coronary intervention

Objective To investigate the effects of tiroofiban on myocardial perfusion and hypersensitive C-reactive protein (hs-CRP), tumor necrosis factor (TNF- a) and troponin T (cTnT) levels in patients with acute coronary syndrome (ACS) after percutaneous coronary intervention (PCI).Methods Eighty patients diagnosed as ACS with PCI in our hospital from August 2013 to August 2016 were enrolled in the study and randomly divided into two groups. The control group (n=40) was treated with clopidogrel,aspirin antiplatelet and low molecular weight heparin calcium anticoagulation.The tirofiban group (n=40) was treated with tirofiban on the basis of conventional treatment.The levels of inflammatory factors and thrombosis were observed before and after the treatment.Results After 24 hours of treatment, the patients' levels of hs-CRP,troponin T (cTnT) and TNF-a increased,but the level of tirofiban group was lower (P<0.05).There was no significant difference between the two groups at 20 min after myocardial perfusion and the incidence of bleeding within 24 h (P>0.05).The total incidence of thrombosis after tirofiban intervention was lower (P<0.05).Conclusion Tirofiban can significantly improve the inflammatory response and reduce the risk of thrombosis in patients with ACS,and is worthy of promotion.

177 Applied research of Dexmedetomidine Hydrochloride Injection and Butorphanol Injection in the process of non-invasive ventilation with respect to chronic obstructive pulmonary disease

Objective To investigate the effect of Dexmedetomidine Hydrochloride Injection and Butorphanol Injection in the process of non-invasive ventilation with respect to acute exacerbations of chronic obstructive pulmonary disease.Methods There were one hundred and twenty chronic obstructive pulmonary disease patients merging with respiratory failure(type II) who accepted non-invasive ventilation.They were randomly divided into two groups:the experimental group for 60 who accepted sedation and analgesia of Dexmedetomidine Hydrochloride Injection and Butorphanol Injection in the process of non-invasive ventilation and the control group for 60 who accepted sedation of midazolam in the process of non-invasive ventilation.The following indicators were observed in the process of non-invasive ventilation: vital signs indicators such as heart rate,mean arterial pressure,respiratory rate and so on,and physical signs indicators such as oxygenation index, partial pressure of carbon dioxide,PH value and so on.The differences of occurrence rate of delirium, trachea cannula and icu stay time of the two groups were compared.Results After six hours of non-invasive positive pressure,partial pressure of carbon dioxide and heart rate in the experimental group were lower than those in the control group,while mean arterial pressure in the experimental group was higher than that in the control group (P<0.05).There was no difference in terms of oxygenation index between the two groups. ICU stay time, mechanical ventilation time, delirium case numbers and trachea cannula cases in the experimental group were less than those in the control group.While everyday average calm object time in the experimental group was more than that in the control group.Conclusion In the process of non-invasive ventilation with respect to acute exacerbations of chronic obstructive pulmonary disease patients, Dexmedetomidine Hydrochloride Injection and Butorphanol Injection can improve man-machine synchronism and compliance and noninvasive ventilation efficiency,which is safe and effective.

180 Clinical significance in combined detection of serum Lipoprotein-associated phospholipase A2 and C-reactive protein in patients with acute cerebral infarction

Objective To explore the clinical significance in combined detection of serum Lipoprotein-associated phospholipase A2 (LP-PLA2)and C-reactive protein(CRP) in patients with acute cerebral infarction.Methods Totally 104 patients with acute cerebral infarction were recruited as the experiment group,and 106 subjects were selected as the control group.The levels of serum LP-PLA2 and CRP were measured for the two groups.The difference of serum LP-PLA2 and CRP levels was investigated between stroke patients and control subjects.The carotid artery color-Doppler ultrasonography and brain MRI examination were done for all stroke patients. All the stroke patients were divided into non-plaque group, stable-plaque group and unstable-plaque group according to the findings of carotid artery color-Doppler ultrasonography,and they were also divided into small area infarction group, middle area infarction group and large area infarction group according to the findings of brain MRI.The difference of serum LP-PLA2 and CRP levels was compared among the different groups above.Finally, the correlation analysis between LP-PLA2 and CRP was investigated.Results The levels of serum LP-PLA2 and CRP in stroke patients were significantly higher than those in control subjects (P<0.01).The levels of serum LP-PLA2 and CRP in stable-plaque group and unstable-plaque group were significantly higher than those in non-plaque group,and in unstable-plaque group they were significantly higher than that in stable-plaque group (P<0.01).Along with the increase of infarction area, the levels of serum LP-PLA2 and CRP were remarkably increased (P<0.01). Furthermore,LP-PLA2 was positively correlated with CRP level in stroke patients (r=0.334,P=0.001).Conclusion The combined detection of serum LP-PLA2 and CRP levels was useful to judge the severity of carotid atherosclerosis and infarct area.

183 Chalazion curettage combined with the Meibomian glands squeezing to prevent recurrence of chalazion in children

Objective To explore the effect of chalazion curettage combined with the Meibomian glands squeezing to prevent recurrence of chalazion in children.Methods Totally 178 cases of chalazion in children treated in our department from January 2015 to December 2017 were randomly divided into 83 cases of chalazion curettage group and 95 cases of chalazion curettage combined with the Meibomian glands squeezing group.The recurrent chalazion in this study includes the following conditions: original cyst didn’t eliminate completely or the cyst reappeared in the original site, or cysts appeared in other sites of the tarsus during the follow-up time. The recurrence rate of chalazion was compared between the two groups.Results The recurrence rate of the chalazion curettage combined with the Meibomian glands squeezing group was significantly lower than that of the chalazion curettage group and the difference was statistically significant (P<0.05).Conclusion For children with chalazion, chalazion curettage combined with the Meibomian glands squeezing can significantly reduce the recurrence rate of chalazion.It is worthy of clinical promotion.

185 Clinical observation of macular pigment optical density changes on visual functions in patients with age-related macular degeneration

Objective To analyze the macular pigment optical density (MPOD) and the multifocal ERG (mf-ERG) changes after pharmacological intervention on variant types of age-related macular degeneration (AMD) patients,and to investigate the impact of macular pigment on visual functions thus providing new clinical indicators in prevention,diagnosis and post-treatment monitoring as well as follow-up visit for AMD patients.Methods Fifty-two eyes(48 cases) patients undergoing fluorescence fundus angiography(FFA)and optical coherence tomography (OCT) were identified as Age-Related Macular Degeneration (AMD) in our hospital from Feb,2015 to May,2016.Twenty-four eyes of 24 cases with dried AMD were treated as group A and 28 eyes of 24 cases with wet AMD were treated as group B.There were 28 male cases and 20 female cases with the eyesight from 0.1 to 0.5.According to whether lutein supplement,All cases were divided randomly into group A1 (lutein),A2,B1 (lutein),and B2.Lutein was administered for 3 months.The MPOD were measured by the MPD mode of the non-mydriatic fundus camera ZEISS Visucam 500 before and after treatment.And The P1 wave and the 1 loop amplitude of the mf-ERG were measured by retinal electrophysiology manufactured by German Roland Company. The impact of macular pigment on visual function of AMD patients were evaluated and statistical analysis was made with SPSS17.0 statistical software.The measurement data were compared with t test and P<0.05 showed the difference was statistical significant.Results The MPOD of group A1 and group A2 before treatment were (0.223±0.135)du and (0.213±0.142)du respectively,with no statistical difference (P>0.05). The post-treatment MPOD of group A1 and group A2 were (0.266±0.189) du and (0.204±0.127) du respectively ,with statistical difference compared with the prior treatment (P<0.05).The prior-treatment MPOD of group B1 and group B2 were (0.169±0.103) du and (0.157±0.137) du respectively ,with no statistical difference (P>0.05).The post-treatment MPOD of group B1 and group B2 were (0.201±0.122) du, (0.132±0.097) du respectively ,with statistical difference compared with the prior treatment (P<0.05).The P1 wave 1 loop amplitude of group A1and group A2 before treatment were (60.42±18.545) and (58.45±20.13) respectively, with no statistical significance of difference (P>0.05).The post-treatment P1wave 1 loop amplitude of group A1and group A2 were (75.53±19.86) and (62.72±20.31) respectively, with statistical difference compared with the prior treatment (P<0.05).The prior-treatment P1wave 1 loop amplitude of group B1and group B2 were (47.44±16.58) and (47.26±16.08) respectively, with no statistical difference in two groups (P>0.05). The post-treatment P1wave 1 loop amplitude of group B1and group B2 were (58.11±16.22) and (50.49±14.41) respectively ,with statistical difference compared with the prior treatment (P<0.05).Conclusion AMD aggravates with the severity of lesions, the density of macular pigment decreases, and mf-ERG amplitude is lowered.The application of lutein can effectively improve the density of macular pigment and protect the visual function of patients with AMD.

188 Clinical application of panoramic ultrasound imaging in superficial tissue lesions

Objective To analyze the panoramic ultrasound imaging’s clinical value in superficial tissue lesions.Methods Totally 200 cases of superficial tissue Lesions were randomly divided into the control group and the experiment group, with 100 cases in each group.The two groups were examined by color doppler ultrasonography and panoramic ultrasound imaging respectively. And surgical pathological examination results were observed and compared.Results Compared with the results of surgical pathology, the diagnostic coincidence rate of 99.0% (99/100) in experimental group was significantly higher thanthat in the control group (86.0%,86/100), and the difference was statistically significant (P<0.05).Conclusion In the diagnosis of superficial tissues and small organs, panoramic ultrasound imaging technology has a relatively high resolution, which can significantly improve the clinical diagnosis rate and has clinical value.

190 Study on the surface modification and antibacterial performance of polyamidoamine on titanium

Objective This article embarks on constructing the anti-bacterial coating by using Polyamidoamine (PAMAM) for modifying the titanium implant surface, then the properties of the coating is accessed.Methods PAMAM was used to construct antibacterial coating on the surface of Titanium. Then, scanning electron microscope (SEM),x-ray energy spectra (EDS) and surface contact angle analyzer were employed to research its characteristics. It was soaked in simulated body fluid (SBF) to detect bioactivity in vitro. Its short/long-term antibacterial property for Staphylococcus aureus and colibacillus were observed.Results PAMAM could modify the surface of Titanium successfully and it has good bioactivity in vitro,The modified Ti sheet possesses good short/long term antibacterial property for Staphylococcus aureus and colibacillus.Conclusion PAMAM can construct a favorable antibacterial coating on the surface of Titanium.

194 Effect of comprehensive community nursing intervention on disease cognition and awareness rates of the elderly patients with hypertension

Objective To explore the effect of comprehensive community nursing intervention on disease cognition and awareness rates of the elderly patients with hypertension.Methods By adopting community health education intervention, the registered?nurses with bachelor or higher degrees and rich clinical experience regularly conducted community health education training for community patients with hypertension, in order to increase patients' cognition and awareness rates of hypertension knowledge.The health education training included hypertension education,hypertension diet,moderate exercise,rational drug use,self-test blood pressure.By distributing the questionnaire,the cognition and awareness rates of hypertension were compared between the two groups after 12 months of comprehensive nursing intervention.Results Comprehensive community nursing intervention significantly improved the awareness rate of hypertension in elderly patients. The difference was statistically significant?(P<0.05).The cognition of hypertension health knowledge of the intervention group, which actively cooperated with community nursing work, was significantly higher than that of the control group. The difference was statistically significant?(P<0.05).Conclusion The comprehensive community nursing intervention can increase the disease cognition and awareness rates of the elderly patients with hypertension, improve their healthy behaviors and quality of life, and reduce complications.

197 Application of laminar sterile bed in the treatment of capillary bronchiolitis

Objective To study the role of laminar sterile bed in the treatment of children with capillary bronchitis,and provide effective new approaches for children with capillary bronchitis.Methods With the list approach,146 children of bronchitis under 6 months old in my department from November 2016 to May 2017 were selected and andomly divided into the intervention group 78 cases and the control group 66 cases.Both groups of children received conventional treatment,Sex,age,hospital infection rate,antibiotic usage rate,medicine proportion,hospitalization days and hospitalization expenses of the two groups were compared.Results The differences of age and gender of the two groups were not statistically significant.The incidence of hospital infection of the intervention group was significantly lower than that of the control group,and the difference(χ2=6.37,P<0.05) was statistically significant.The antibiotic usage (χ2=11.48 P<0.05), medicine proportion (χ2=9.52,P<0.05),hospitalization days (Z=6.58 ,P<0.05),and the hospitalization expenses (Z=5.96,P<0.05) of the intervention group were significantly lower than those of the control group.Conclusion Laminar sterile bed can effectively prevent hospital infection of capillary bronchitis, reduce the antibiotic usage and medicine proportion,shorten the hospitalization days and reduce the hospitalization expenses.

200 Determination of effective components in the fermentation broth of Flos Sophorae Immaturus by HPLC

Objective To establish a high performance liquid chromatography (HPLC) method to determine the content of rutin and quercetin in Flos sophorae immaturus fermentation broth.Methods Flos sophorae immaturus was fermented by Natto fungus and the broth was separated on a Phenomenex Gemini 5μ C18 110A (250 mm×4.6 mm I.D.5μm) eluted with methanol-water (0.05% formic acid) in linear gradient condition [0min→15min→20min→25min→26min→ 36min,elute rate of methanol-water (0.05% formic acid) was 35:65→50:50→55:45→ 60:40→35:65→35:65].The detection wavelength was 257nm for rutin and 254nm for quercetin.The flow rate was 1.0 mL/min;the column temperature was set at 25℃.Results There was a good linear relationship between the peak area and concentration of rutin and quercetin.The linear range was 0.031 ~ 1 mg/mL (R12= 0.9997,R22=0.9991).The precision RSD of rutin was 0.997% and quercetin was 0.392%.The stability RSD of rutin was 0% and quercetin was 0.05%.The repeatability RSD of rutin was 1.405% and quercetin was 1.649%.The average recovery rate of rutin was 93.90%,with RSD 1.55% and quercetin was 98.55%,with RSD 1.65%.The contents of rutin and quercetin were 19.9199±0.1436 mg/g and 0.6501±0.0089 mg/g, respectively.Conclusion The method can accurately and efficiently determine the content of effective components of the fermentation broth of Flos sophorae immaturus.

204 Preparation and quality control of compound spironolactone gel

Objective To prepare compound spironolactone gel and to establish a method for its quality control.Methods The gel was prepared using carbomer-940 as the matrix,and spironolactone and erythromycin as the main ingredients,and the contents of spironolactone and erythromycin was detected by high performance liquid chromatography.Results The gel was exquisite,colorless and transparent,the viscosity was moderate,and the pH value was 7.0~7.5.The linear range was 0.251 6~5.032 μg·mL-1 for spironolactone (r=0.999 9) and 0.577 2~11.54μg·mL-1 for erythromycin (r=0.999 9).The average recoveries were 97.80% (RSD=0.74%,n=9) and 96.7% (RSD=2.60%,n=9),respectively.Conclusion The method is simple,and the quality is stable and controllable.
