241 A method for establishment of experimental periodontitis model in SD rats

Objective To establish an animal model of rat periodontitis by optimization method and observe the degree of alveolar bone resorption in animal models of periodontitis.Methods The maxillary second molars of the rats in the periodontitis group were ligated with silk. During the modeling period, the periodontal condition of the rats was observed. The alveolar bone resorption was detected by imaging. The alveolar bone resorption was measured by stereo microscope. The relative expression levels of TNF-α and MMP-8 mRNA in gingival tissues were detected by RT-qPCR.Results In the experimental group, the periodontal tissue inflammation of the maxillary second molars gradually increased, the alveolar bone absorption increased, and the looseness gradually increased. The relative expression of TNF-α and MMP-8 mRNA in the experimental group was higher than that in the control group. Conclusion The animal model of SD rat periodontitis was successfully established by optimizing the modeling method.

243 Effects of chloroquine on neurotransmitter expression in rat brain with epilepsy induced by pentylenetetrazole

Objective To observe the effects of chloroquine on adenosine kinase (ADK) and Glutamate (Glu)in cerebral cortex and hippocampal of rats with epilepsy induced by pentylenetetrazole, to explore the role of chloroquine in epileptogenesis. Methods Thirty male adult Sprague-Dawley (SD) rats were randomly divided into control group (n=10), pentylenetetrazole (PTZ) group (n=10) and chloroquine intervening group (n=10). The behavior and electroencephalogram of rats were observed and recorded. The expression of ADK and Glu in cerebral cortex and hippocampus was examined with immunohistochemistry. Results The control group showed no seizure activity. Severe seizure activity was observed in PTZ group (Ⅳ-Ⅴdegree), while slight seizure activity(Ⅰ-Ⅲdegree) appeared in the chloroquine intervening group. Electroencephalogram studies showed high amplitudes with fast frequencies in the PTZ group, low amplitudes and slow frequencies in the chloroquine intervening group. The expressions of ADK and Glu in the PTZ group were stronger than that of the control group (P<0.01), and significant differences were observed between the chloroquine intervening group and the PTZ group (P<0.05). Conclusion Chloroquine could alleviate the seizure activities induced by PTZ in rats, inhibit the expression level of ADK and Glu.

247 Effects of EGCG on pain behavior of mice with neuropathic pain

Objective To investigated the anti-nociceptive effects of EGCG on neuropathic pain induced by chronic constriction of sciatic nerve injury (CCI) in mice.Methods Establish the models of the chronic constriction injury of sciatic nerve, using the von Frey, plantar, cold-plate, to assess the effect of different doses of EGCG (100, 65, 20 mg/kg) on mechanical, radiant, thermal respectively.Results Compared with CCI group,EGCG of 100, 65 mg/kg could improve the operated lateral paw withdrawal threshold, prolong the operated lateral paw withdrawal latency and reduce the counts of the operated lateral paw withdrawal (P<0.05). Conclusion EGCG can alleviate CCI-induced neuropathic pain.

250 Effect of electrospinning film stent on proliferation and differentiation of MC3T3-E1 cells

Objective To observe the effect of electrospinning membrane scaffold on proliferation and differentiation of MC3T3-E1 cells. Methods The membrane scaffolds were prepared by electrospinning. The reticular network was observed by scanning electron microscopy. The membrane scaffolds were used as carriers to observe the proliferation and differentiation of osteoblasts and observe the morphological changes of cytoskeleton. Results Scanning electron microscopy showed that the electrospun membrane network structure was conducive to cell adhesion; CCK-8 cell proliferation assay and alkaline phosphatase activity persisted to the results, and the difference between the control group and the experimental group was 1 d, 4 d and 7 d. All of them were statistically significant (P<0.05); cytoskeletal staining showed that the cell spreading area of the electrospinning group increased and the number increased. Conclusion Electrospun membrane stents can promote osteoblast proliferation, differentiation and cell adhesion.

253 Clinical significance of serum CXCL-10 level and the correlation between severity in children with bronchial asthma

Objective To investigate the clinical significance of CXCL-10 and correlation between serum CXCL-10 level and severity of bronchial asthma in children with bronchial asthma. Methods The expression levels of fasting serum CXCL-10, IL-4 and INF-γ in the control group and asthma group were detected by enzyme-linked immunosorbent assay (ELISA) and statistical analysis was performed using Graphpad Prism. Results The serum level of CXCL-10 in children with asthma group was significantly higher than that in the control group (P<0.01). The serum level of CXCL-10 in severe asthma group was significantly higher than that both in the diapause/mild group and the moderate group (P<0.01). The serum level of CXCL-10 is positively correlated with the IL-4 level (P<0.001). Conclusion CXCL-10 is involved in the pathogenesis of bronchial asthma in children by promoting the activation of Th2 lymphocytes.

257 Effect of ecological immune enteral nutrition support on prognosis and liver function of patients with severe acute pancreatitis

Objective To study the effect of ecological immunity enteral nutrition on prognosis effect and liver function in the severe acute pancreatitis(SAP) patients. Methods The 58 patients with severe acute pancreatitis were randomized to ecological immunity enteral nutrition(EIEN group) 64 and enteral nutrition (EN group) 32 respectively. The changes of prognosis effect and liver function in both groups before and post nutrition support in 3 days and 8 days was determined. Results The white blood cell count, liver function (AST, ALT, γ-GT) and therapeutic effects of two groups of SAP patients were detected in the 3th days and 8th days after EIEN nutrition support. The two groups were statistically significant (P < 0.05). Conclusion Ecological immunity enteral nutrition can protect liver function and reduce the SAP complications and enhanced treatment effect in the SAP patients,it was one of the important treatment Methods for SAP patients and would be used widely in the practice.

260 Investigation and analysis of mental health status of Medical Postgraduates

Objective To understand the mental health status of medical postgraduates and to provide basis for improving the mental quality of medical postgraduates and carrying out mental health education. Methods A psychological test was conducted on 309 medical postgraduates using survey tools such as beck anxiety scale (BAI), flow calling self-rating depression scale (CES-D), and symptom checklist (SCL-90). Results There are positive symptoms of anxiety among a few medical postgraduates in our school, accounting for 0.97% of the total number of students surveyed. It is sure that students with depression account for 8.74% of the survey. With a total score of 160 as the threshold, 16 people with mild mental problems accounted for 5.18% of the total number of respondents. Taking a factor of more than 2 in 10 factors as a diagnostic criterion for mild mental problems, postgraduates with mild psychological problems account for 14.56% of the total number of respondents. The scores of SCL-90 for medical postgraduate students in our university are lower than those of the National Youth norm group, differences are statistically significant (P<0.001). Conclusion The overall mental health of medical postgraduates in our school is good, but a few students have mild mental health problems.

263 Effect analysis of cortical bone trajectory screw for osteoporotic lumbar degenerative disease

Objective The aim of this study was to evaluate the outcomes of cortical bone trajectory (CBT) screws in the posterior lumbar fusion surgery of lumbar disease with osteoporosis by a retrospective study, a relatively new technique. Methods This is a retrospective cohort study. All patients that underwent circumferential lumbar interbody fusion with CBT screws in our hospital from 2015 to 2018 were reviewed. Patient age, clinical outcome with visual analogue scale (VAS) and Oswestry Disability Index (ODI), operative blood loss, hospital stay and the fusion rate were evaluated. Results All patients obtained follow-up 16 to 24 months postoperative, with an average of 19.2 months. Oswestry Disability Indexes and Visual Analogue Scores improved significantly at 3 months postoperative. Mean procedural time was 183.2 min. The mean operative blood loss was 375.7 mL. The mean hospital stay was 4.56 days. The fusion was obtained in 100% of cases. Mean ODI and VAS index postoperative improved with statistical significance. Conclusion The use of cortical bone trajectory (CBT) screws for interbody fusion is safe and effective to patients with osteoporosis. While the technique has less trauma, shorter operative time, hospital stays and good fusion rate.

267 Expression of autophagy-associated protein LC3II in non-small cell lung cancer and its correlation withradiotherapy sensitivit

Objective To study the expression of autophagy-related protein LC3II in non-small cell lung cancer (NSCLC, including lung adenocarcinoma and squamous cell carcinoma), and elucidate the influence and significance of autophagy expression on lung cancer radiotherapy curative effect. Methods Immunohistochemical method was used to detect the expression of LC3-II in 243 cases of non-small cell lung cancer based on retrospective study and 48 cases of normal lung tissue, and analyze the the relationship with expression of LC3II and clinicopathological factors. Dividing the patients into four groups according to the curative effect, to explore the relationship between the expression of LC3II and radiotherapy sensitivity. Results ① The positive expression of LC3II in the adenocarcinoma group and the squamous cell carcinoma group was lower than that in normal lung tissue.② High LC3II expression was found to be significantly correlated with tumor size, necrosis and TNM grade (P<0.05) , but not with age, sex and pathological type. ③ Compared with all patients with positive expression of LC3II (52.7%), the positive rate of LC3II expression in CR group was the lowest (40.9%), whereas PD group was the highest (71.9%). Conclusion Autophagy-related protein LC3II was down-regulated in lung adenocarcinoma and squamous cell carcinoma, and associated with large size, necrosis and TNM grade, which may be related with the development of lung cancer. The high expression of autophagy is closely related to lung adenocarcinoma, squamous cell carcinoma and the radiation resistance, which would affect the efficacy of radiotherapy.

271 Study on the risk factors of surgical site infection and artificial network model in a class a teaching hospital

Objective The incidence rate of surgical site infection of surgical patients in a hospital was calculated, the related factors of postoperative infection were found out, and the probability of postoperative infection of surgical patients was predicted scientifically. Methods Research object selected a top three teaching hospitals in 2012—2015 postoperative patients, 275 cases patients with surgical site infection cases, the other in accordance with the 1∶1 ratio to choose 266 patients as control group, postoperative infection cases retrospective survey study, statistical detection pathogenic bacteria and surgical site infection situation, to probe into comprehensive factors of surgical site infection combined with statistical analysis data to construct artificial network model of postoperative infection. Results The Results showed that surgical site infection rate was 0.35%. The Results of multi-factor analysis showed that the independent risk factors for surgical site infection were the type of operation, the presence of primary disease, the type of incision, the ASA grade, the patient's age, the duration of operation, and the number of cases (OR=11.043, 9.587, 2.136, 1.818, 1.299, 1.293, 1.041, P<0.05). Finally, the operation resultsof the network model are determined as follows: the average error is 0.040% and the network error rate is 0.038%. Area under the curve (ROC) is greater than 0.9. Conclusion The top three risk factors of surgical site infection were surgical type, primary disease and incision type. The prediction Results of the artificial network model for the risk of ssis were satisfactory, and the related prediction information platform system was established to provide guidance for the clinical hospital infection control decision.

275 The value of the sFlt-1∶PlGF ratioin predictionin women with suspected preeclampsia

Objective To investigate the predictive value of soluble fms-like tyrosine kinase 1 (sFlt-1) and placental growth factor (PlGF) ratio in women with suspected preeclampsia. Methods We performed a prospective, multicenter, observational study to prove in the short term in women with singleton pregnancies,whether low sFlt-1∶PlGF ratios (at or below a derived cutoff) predict the absence of preeclampsia within 1 week and whether high ratios (above the cutoff) predict the presence of preeclampsia within 4 weeks. Results In a subsequent validation study among an additional 550 women, an sFlt-1∶PlGF ratio of 38 or lower had a negative predictive value (i.e., no preeclampsia in the subsequent week) of 99.3% (95% confidence interval [CI], 97.9 to 99.9), with 80.0% sensitivity (95% CI, 51.9 to 95.7) and 78.3% specificity (95% CI, 74.6 to 81.7). The positive predictive value of an sFlt-1∶PlGF ratio above 38 for a diagnosis of preeclampsia within 4 weeks was 36.7% (95% CI, 28.4 to 45.7), with 66.2% sensitivity (95% CI, 54.0 to 77.0) and 83.1% specificity (95% CI, 79.4 to 86.3). Conclusion An sFlt-1∶PlGF ratio of 38 or lower can be used to predict the short-term absence of preeclampsia in women in whom the syndrome is suspected clinically.

278 Application of accurate fracture line injection vertebroplasty in vertebral osteoporotic fracture

Objective To compare the clinical effect between traditional percutaneous vertebroplasty(PVP)and precise fracture line puncture vertebroplasty in the treatment of osteoporotic vertebral compression factures (OVCFs).Methods A retrospective controlled study was designed to review 112 cases of single level OVCFs hospitalized from January 2015 to October 2017.The patients were divided into the observation group and the control group according to random number table method, with 56 cases in each group.The control group was treated with conventional vertebroplasty , and the observation group was treated with precise fracture line puncture. Visual analogue score (VAS) were performed before the surgery and at 1 d,3 d after surgery.Operating time,cement injection volume,rate of cement leakage,cement distribution in the fractured area and VAS of back pain were compared between the two groups.Results At 1 d、3 d after surgery, the VAS of the observation group was significantly lower than that in the control group(P<0.05). In the observation group, the fracture area was filled with sufficient bone cement, and 8 cases in the control group showed insufficient filling. The difference was statistically significant (P<0.05). There were no significant differences in operating time, cement injection volume and rate of cement leakage between the two groups(P>0.05). Conclusion Compared with traditional percutaneous vertebroplasty, precise fracture line puncture vertebroplasty can ensure the full filling of bone cement in the fracture area and reduce the occurrence of poor postoperative efficacy. It is a safe and effective new technology.

281 Postoperative adjuvant chemotherapy for stage IB non-small-cell lung cancer:A systematic review and meta-analysis

Objective We systematically reviewed the literature to investigate the effect of postoperative adjuvant chemotherapy in patients with completely resected stage IB non-small-cell lung cancer. Methods Pubmed, Embase, Cochrane Library and Web of Science were searched regarding the effect of postoperative adjuvant chemotherapy for completely resected stage IB non-small-cell lung cancer,and randomized controlled trials(RCT) were collected according to the inclusion criterias. Data were analysed with RevMan 5.3 software. Results Six studies with 1 286 patients were included. Pooled analysis suggested that 5-year overall survival between the two groups was similar, and the difference had no statistical significance (HR: 0.83,95% CI: 0.66~1.04,P=0.11). However, the subgroup analysis demonstrated that 5-year overall survival had statistical significance for patients who had tumors ≥ 4 cm in diameter (HR: 0.51,95% CI: 0.26~0.99,P=0.05). The 5-year disease-free survival in the adjuvant chemotherapy group was greater than the control group(HR: 0.58, 95% CI: 0.35~0.94,P=0.03). Conclusion Postoperative adjuvant chemotherapy can’t increase 5-year overall survival in patients with stage IB non-small-cell lung cancer. However, postoperative adjuvant chemotherapy can increase 5-year overall survival and 5-year disease-free survival for patients who had tumors ≥ 4 cm in diameter.

285 Detection and analysis of biochemical markers of bone metabolism in diabetic osteoporosis

Objective To investigate the relationship between the metabolic levels of fibroblast growth factor-23 (FGF-23), osteocalcin (OCN), collagen type I C-terminal peptide (CTX) and osteoporosis in diabetic osteoporosis. Methods Mice were randomly divided into three groups: control group, STZ group and US group. The changes of biomarkers of bone metabolism in serum, urine and bone were detected. Results Ursolic acid derivative (UAD) can significantly increase bone calcium content and bone mineral density, promote the expression of fibroblast growth factor-23 and OCN, and reduce the expression of PTH and CTX in diabetic mice. Conclusion Ursolic acid derivatives can improve STZ-induced osteoporosis by promoting new bone formation and inhibiting osteoclast bone resorption.

288 Comparasion of uniportal video-assisted and single utility port video-assisted pulmonary lobectomy in the treatment of pulmonary nodules

Objective To compare the clinical effects of uniportal video-assisted and single utility port video-assisted thoracic surgery in the treatment of pulmonary nodules, and explore the clinical value of uniportal video-assisted thoracic surgery in the diagnosis and treatment of pulmonary nodules. Methods From August 2017 to July 2018, department of thoracic surgery of Binzhou Medical University Hospital performed a total of 90 cases of uniportal video-assisted thoracic surgery, compared with 81 cases of single utility port video-assisted thoracic surgery at the same time. Results All the patients completed the operation successfully, no one was changed to open operation, no death or serious surgical complications during perioperative period. There were no significant differences in operation time, number of lymph node dissection, the incidence of postoperative complications between two groups (P>0.05). There were significant differences in blood loss, chest drainage time, hospital stay, postoperative 24 h and 72 h pain scores between two groups(P<0.05). There was no recurrent metastasis during 0~11 (7.8±3.6) months follow-up time. Conclusion Uniportal video-assisted thoracic surgery is helpful for the early diagnosis and treatment of pulmonary nodules. The wound is smaller, the postoperative recovery is quicker with less postoperative pain , which is worthy of clinical application.

291 Effect of ursolic acid derivatives on bone remodeling in diabetic osteoporosis

Objective The aim of this study was to investigate the effects of ursolic acid derivatives (UAD) on bone remodeling in diabetic osteoporosis. Methods The mice were randomly divided into three groups: control group, STZ group and US group.The tibia was taken for gene and protein expression determination and histomorphologicalanalysis. Bone volume/total volume (BV/TV), trabecular number (Tb.N), trabecular thickness (Tb.Th) and bone mineral density/total volume (BMD/TV) were measured.Detection of osteoprotegerin/nuclear factor-kappa B receptor activator (OPG/RANKL) ratio and expression of key regulators of bone metabolism. Results Compared with the control group, OPG/RANKL ratio and OPG in STZ group decreased significantly and RANKL increased significantly.The OPG/RANKL ratio increased significantly in US group.The expression of MMP9 and CAI in STZ group was significantly higher than that in control group, while the expression of Runx2 and TGF-beta in STZ group was significantly lower than that in control group.US group reversed the expression of MMP9, CAII, RUNX2 and TGF-beta in diabetic mice at the same time. Conclusion UAD reverses STZ-induced trabecular bone injury by promoting osteoblast differentiation, new bone formation and inhibiting osteoclast bone resorption.

295 Relationship between fluid resuscitation under PICCO in different time and the prognosis of patients with septic shock

Objective To observe and evaluate the effect of different fluid resuscitationstandard time on the prognosis of patients with septic shock under the guidance of PICCO monitoring. Methods A retrospective study was used. The patients divided into 0~6 h standard group and 6~12 h standard group according to the different time when ITBVI reached 850~1 000mL/m2 monitored by PICCO. Blood gas analysis, lactic acid, SOFA scorewere recorded at 6 h, 12 h, 24 h and 48 h after admission, length of stay in ICU, mechanical ventilation time, and the fatality rate at 7 and 28 days were recorded. Results After 24 h of treatment, lactic acid and SOFA scores in the 0~6 h group were significantly lower than those in the 6~12 h group, and the fatality rate was significantly reduced at 28 days. Compared with the 6~12 h standard group, patients in the 0~6 h standard group had shorter mechanical ventilation time, shorter time in ICU, and less 7-day mortality, but the difference was not statistically significant. After treatment, the indexes of oxygenation index and lactic acid in both groups were better than those at 0 h. Conclusion Fluid resuscitation therapy for patients with septic shock under the guidance of PICCO monitoring can significantly reduce SOFA score and 28-day mortality after reaching the standard at 6 h compared with 12h. Both 6 h and 12 h ITBVI reaching 850~1 000 mL/m2 can improve the prognosis of patients.

298 Analysis of medical treatment behavior and influencing factors of rural residents in shandong province under the background of graded diagnosis and treatment

Objective To understand the medical behavior of rural residents, analyze the influencing factors of medical behavior, and the impact of hierarchical diagnosis and treatment system on medical behavior, so as to provide policy suggestions for system optimization. Methods Convenient sampling and self-designed questionnaire were used. Results Only 44.39% of residents choose to seek medical treatment when they fall ill, and the main places for medical treatment are clinics/village clinics, pharmacies, etc. The reasons for choosing the above places mainly include medical habits, short distance, low medical costs, and short waiting time. The main reasons for not seeking medical treatment include limited economic conditions, self-medical treatment, self-belief that the disease is not serious, and long waiting time. Residents of different ages, family annual income and education level showed statistically significant differences in medical treatment behavior (P<0.05). Conclusion The majority of rural residents do not seek medical treatment due to illness, mainly due to the impact of economic conditions and hierarchical medical system.

300 Determination of Fluconazole Ophthalmic Thermosensitive Gels by HPLC

Objective To establish an HPLC method for the content of Fluconazole Ophthalmic Thermosensitive Gels . Methods The method was performed on a C18. column, using mobile phase consisted of acetonitrile-0.063% ammonium formate soltuin(20∶80). The detection wavelength was 260 nm. Results Fluconazole in Fluconazole Ophthalmic Thermosensitive Gels can be effectively separated under this chromatographic condition. The linear ranges was 5.9~239.8 μg·mL-1(r=0.999 9) for assay. The mean recovery was 98.96%, RSD was 1.29%. Conclusion The method is accurate and stable for content determination of Fluconazole Ophthalmic Thermosensitive Gels .

303 Comparison of ginsenosides from Panaxquinquefolium L. by two different extraction methods

Objective To select the best extraction method of ginsenosides in Panaxquinquefolium L. Methods Methanol ultrasonic method and N-butanol reflux method were used to optimize the best extraction method of Ginsenosides. High Performance Liquid Chromatography (HPLC) was established to determine the content of ginsenosides. Results The average content of Rg1 and Re extracted by ultrasonication of methanol is significantly higher than that of n-butanol reflux extraction(P<0.05); while the average content of Rb1 extracted by ultrasonication of methanol is significantly lower extracted by n-butanol reflux extraction(P<0.05).There is no significant difference between the average content of Rc and Rd within the two methods. The content of Rg1 extracted by methanol is higher than that of n-butanol extraction. Methodological investigation showed the best that the inter-day precision and the average recovery rate of Rg1, Re, Rb1, Rc and Rd. Conclusion Rg1 and Re extracted by methanol ultrasonication are significant higher than n-butanol reflux extraction. While, Rb1 extracted by methanol ultrasonicationis significant lower than n-butanol reflux extraction. Thus, the optimal method for ginsenosides extraction of American ginseng should be chosen by different ginsenosides component.
