Analysis of risk factors of pulmonary infection in the early stage of severe head injury |

Abstract Objective To analyze the risk factors of early pulmonary infection in patients with severe craniocerebral injury.Methods The clinical data of 300 patients with severe head injury were retrospectively analyzed,and the patients were divided into the non-infection group (147 cases) and the infection group (153 cases) according to the occurrence of pulmonary infection.Univariate analysis and multivariate Logistic regression analysis were used to compare the differences between the two groups.Results The incidence of pulmonary infection in 300 patients with severe head injury was 51% (153/300).Multiple factor logistic regression analysis showed that smoking,GCS score and glucocorticoid were independent risk factors of pulmonary infection in patients with severe head injury,and the OR (95%CI) were 0.327 (0.151-0.709),0.292 (0.212-0.403) and 2.196 (1.007-4.786),respectively.Having found that Glasgow Coma Scale (GCS) is a strong predictor outcome(considering favourable outcome),ROC curve was made for finding out the optimal cutoff ( GCS=5) as a predictor.Conclusion The incidence of pulmonary infection in patients with severe head injury is high and prevention measures should be strengthened for high-risk patients to reduce the incidence of infection.
